Skin Care Tips- Treating hormonal acne
Hormonal acne is extremely common and affects one in four Australian women.
Before going into treatment, lets explore how to tell if you suffer from hormonal acne.
1. You are no longer a teenager. While we’d love to leave acne behind once we enter our 20s, the reality is flare-ups are possible at any time. Hormonal acne is the type that’s most likely to present in your 20-something years - all the way to your late20s. That because those are the years when women are most hormonally active. Your 20s is often peak childbearing age, making women more prone to the intense hormonal fluctuations conditions such as polycystic ovarian syndrome, childbirth, pregnancy and breastfeeding. Age alone won't determine whether or not you will suffer from adult acne. Genetics can dictate when hormonal breakouts start and stop. This is modified by diet, stress and the products you use.
2. One of the telltale signs of a hormonal breakout is its location on the face. If you’re noticing inflamed cysts around your lower face-especially your chin and jawline area-you can bet your bottom dollar that it’s probably hormonal. This is due to the fact that acne glands, known as sebaceous glands are concentrated in this area.
3. Period cycles and patterns. Adult hormonal acne often occurs in a cyclical pattern, much like women’s menstrual cycles. This is true even in postmenopausal women, because these women still experience monthly fluctuations in their estrogen and progesterone levels, albeit lower than pre-menopausal women. Adult hormonal acne tend to occur in the same place each month.
4. Stressors play a major role in adult acne. With our stressful jobs, cortisol or cortisone levels rise, this in turn stimulates the oil gland to enlarge and produce more sebum, clogging the oil gland to produce painful cysts. That is why taking a vacation resets our cortisol patterns and acne usually improves.
5. Deep painful cysts are the predominant lesion of adult acne, with very little in the way of zits, and blackheads. These painful cysts manifest as deeper bumps that are under the skin’s surface and cannot be extracted with normal manual extraction.
How to treat? Here is the truth- this form of acne is one of the hardest DIY acne sub types to treat. Why? Because cysts lie deep in the dermal layers of your skin, beyond normal therapies such as creams, chemical peels and even laser light! Try these hints before seeing a dermatologist.
Firstly review your skin care- use a simple cleanser, and non-comedogenic products. This goes across the board from your vitamin A, B, C creams and serums, sunscreen, moisturiser to your make up range. Mineral make up is considered the best for any form of acne, including hormonal acne.
Secondly identify and rectify your flare up factors. Flare factors include foods such as dairy and refined foods high in sugars. Eat a balanced diet, and supplement this with zinc.
Thirdly add vitamin A creams. In Australia buy a high strength retinol serum or cream and apply nightly to acne prone areas. In the US you can buy Differin. The trick is to constantly use this retinoid- its designed to prevent acne and not to be used as a spot treatment. If you after a natural solution- tea tree oil may help.
The use of anti-inflammatory antibiotics may help some patients. Additionally the use of BHA or AHA peels and Blue/ Red light one week before breakouts can help some patients. Kleserca can be successful in some cases.
So, how do medical dermatologist treat hormonal acne? Firstly they may investigate your condition and conduct a full hormone profile. Secondly they may treat you with the pill. OCP or oral contraceptive pills such as Diane, Estelle, Yaz or Yazaim may help. It often takes 3 period cycles to gain hormonal control. Medication such as spironolactone or cyperterone acetate are highly effective, and in my option are much safer than Accutane or Oratane. Oral retinoids are the last line of defense in the fight to control adult hormonal acne, they have many side effects and regular blood tests are required.

What frustrates me is when some pimples fade away, it grows on another spot. For example I got pimples on forehead, when it disappears, it switches to my chin, nose, and it's so annoying

I can tell you it made zero difference whether I wore my liquid foundation or not to my hormonal acne. I tested this out in quarantine by not wearing any makeup for months. Had the worst cystic acne of my life.

I know this is a year old comment, but I also think it's stress and dairy for me. I just stopped drinking milk so I hope it helps. I'm also super stressed with school and that's when my acne started. Anyway, I hope your skin journey has been going better!

Jessica Heidi Melo I wish that worked for me. I went all the way vegan and got rid of gluten and caffeine and alcohol for three years. Only thing that happened was my acne got worse and I was mentally feeling horrible due to enjoying Nothing. I just manage it now with topical products instead of aiming for flawless skin.

Ah this makes so much sense 😫 I had no acne as a teenager then randomly got it a few years ago, must have been around 25, for a while it went away and I forgot all about it but it’s back worse than ever now I’m 29! Every month I break out loads during my PMS week then fade away during the month just for them to all come back again before they’ve even fully gone 😔

Yes, and also skin regiment. I started washing my face two times a day and than applying moisturising cream, that helped me a bit, however the biggest thing for me was pepfactor treatment done at a clinic. My skin looks like before young and clean with no foundation required. Just try, you will definitely love it.

I have been dealing with acne for 15 years now and I've seen a lot of doctors and dermatologist and none of them checked my hormones levels they would just prescribed drugs and creams. I remember the first time I saw a doctor for acne I was 11 years old and he told me it would go away with time, just be patient. Ya right i'm 26 years old now and I've been dealing with really painful cystic acne it is so depressing my face is covered in scars

I'm so sorry. I've been dealing with acne for about 15 years too. I;m 29 years old. My cousins have beautiful skin, and I'm stuck with weight gain and acne. I got rid of acne when I started taking birth control. I had to stop taking it because I'm trying to conceive. My hormonal acne returned. It is frustrating! So I changed my diet to a KETO (no refined carbs, high fat, moderate protein) for weight loss. My skin cleared up, my nails are stronger, my hair is thicker. So now I know my acne is attributed to diet. I hope you find what is causing yours. It sucks, acne sucks because you cannot be your best self.

f0repl4y Oh wow.... I’m 31 years old and it has been soooo long also... 16 years!! I cant deal with it anymore.... Did your skin clear up entirely going keto?? Did your parents/siblings have acne too? In my case a lot of my brothers had it in their teens, my older sister still has it (but not as bad as mine) and my father had it in his teens.... Im starting to fear its never going to get better.... :( I even had Accutane 3 different rounds through my teens! Worked amazingly, but the acne still came back.... I hope it didnt cause any damage...

I had serious acne all over my face, neck, chest and back. It was extremely painful. I was given accutane for a year and my acne disappeared completely. Now it is 10 years later and I'm getting a bit of hormonal acne. After watching this video I think it could be because of stress and sugar. Thank you for a good video!

I love this video! I have hormonal acne at 28😒 I quit sugar and cut back white grains and saw improvement. I then added a sunscreen and differin gel to my regimen and saw more results. Drink tons of water and keep your hands away from your face and a clean pillow case. It’s been a few weeks and my skin looks better.. not the best. But tolerable.

Try ketogenic diet and intermittent fasting. It permanently cleaned my acne. No sugar and I mean it - totally no sugar. Just meat, healthy fat and green veggies.

I am 38 years old and one year ago, my skin was flawless. The i decided to avoid washing face for 24 hours and my face full of zits for the first time, instant regret. Now its much better but i still have some red bumps over the cheeks and one red bump on my nose, i resorted to using Oxy5 to sped up process. I don't use any retonoid or hi-fi item yet, only some benzyl benzoate and sea cucumber exfoliator.

Sis I’ve been doing a sugar and honey scrub and it works.. regularly steaming my face, reduces the inflammation. Get rid of anything causing you stress!

When I eat healthy (keep sugar and processed food to a minimum), control stress level, get good sleep and use good quality natural oils (my favourite are from Herbivores and Prim Botanicals) I am able to keep my acne under control. Also, drinking adequate water helps with hydration. However, I am not always disciplined enough to stick to this lifestyle but I do try.. always keep trying again!!! When I get acne on my jaw, chin or basically anywhere else on my face I put body shop Himalayan charcoal purifying mask on the spot so that the zit comes to the surface faster and the mask does help a bit in controlling the pain. But this can be a bit drying so I only put the mask on and closely around the zit when I plan to leave it on for long hours. This really helps me a lot.

This is the BEST VIDEO IVE WATCHED EVER CONCERNING HORMONAL ACNE. Very informative. Elementary, Middle school, High school, and college had drop dead gorgeous clear beautiful skin. I didn't start wearing makeup until I was 28 and very bars minimum. Until I hit 32 and right now I've been to 2 dermataligist and not satisfied with the results. Docycyline is very good but only last until it's done and noncosmegnic products has been a life saver for me. Hoping this video helps. Leave it to the pros to do a good job!

I've been beginning to focus more on the digestive aspect of hormonal acne, but you gave me a better idea of my next step which is to see a derm and possibly get on the right birth control pill.

Wow I'm so impressed with the authentic information and objective approach here. I rarely see this perspective from dermatologists and this is genuinely helpful as opposed to pushing all cases of acne to prescription drugs.

I suddenly had a spike of acne around my jaw after 1 year of having good skin. I didn't change anything from my diet or skin care, which made me wonder why I was breaking out. I think this is definitely hormonal and due to stress (bumps started to appear few weeks from my finals and graduation). AHA doesn't seem to work for me, it flares my acne more. What I do is put aloe on my face to cool it down and reduce the warm sensation. Thanks for your advise Dr. Lim, I will eliminate sugar and dairy from my diet first.

Thank you so much for taking the time to explain hormonal acne. I have been to see so many dermatologists and frankly I have been flabbergasted at their seemingly indifference to a very horrible and upsetting condition. Many of them explain nothing and just write a prescription for the “usual” antibiotics which they probably know won’t work. If you get a demo like this then don’t put up with it. Shop around and get a good one. Ps the thing that finally worked amazingly well for me was Spiroloctane which was prescribed by a lovely dermatologist that took the time to listen to my history of the condition. Game changer!

This was very useful! I had no idea DHEA was an androgen, I've seen it in so many vitamin formulations. I wish more physicians were as a knowledgeable as you about acne. Sad to even have to say that. Most of them just tell you to change your pillow case more often and want to put you on spiral, birth control, or antibiotics. Never diving deeper, never helping to break the cycle.

I really enjoyed this video. I have hormonal acne along the jaw line and was happy that you addressed causes, and even went into how medications work. I think I will try reducing my dairy and sugar intact to see if that works.

Thanks for sharing this! I wish more dermatologists were aware of the dietary impact and also stepwise therapy before throwing out accutane. I was put on 3 rounds of accutane at 15 which caused so many side effects down the road. When my acne returned at 25 did research myself and went on a dairy free plant based diet and I barely get cysts anymore

Accutane is the only thing that worked for me. Have been off for about a year. Face looks alot better. I went vegan as well because I was at my wits end. No dairy, minimal sugar, and lots of water to keep my face clear.

Dr. Lim...I take my hat off to you. I have struggled with acne since the 4th grade and I'm 36. I also have a background in nursing and pharmacy. You reminded me to stop trying to fix my hormonal acne all on my own with OTC products and get back to the dermatologist. Isotretinoin has been tossed around as an idea and you actually hit close to home on my feelings about it and reminded me that if it's necessary...it's okay. This was informative, dead on accurate and a breath of fresh air compared to the other videos that just push gluten free, consuming coconut oil only, and pretty much only eating nothing but chia seeds and kale. Thank you. Keep up the awesome insight!

For me refined sugar is a major offence. Everytime I eat too much bread, cookies I get big painful deep pimples around my mouth.

Thank you Dr for a really clear explanation, I love it. I've tried lots of skincare products for three years. I had a moment that I have the least acne but after months,it start to grow again. May be due to stress and hormonal imbalance. I take Doxycycline right now. But I think it still not work. I still have inflammatory acnes at cheek and jawline areas with lots of tiny bumps. Hoping that one day I got a clearer skin. Seriously, my face is never clear from acne since I'm 12 year old. I thought it was normal but realising that my friends who face acne during teenagers had better skin when it comes to 20s ages,it make me wonder what happen to me actually. :( Day by day,it become worse and it spreads to ears and neck areas.

Honestly, I had bad hormonal acne as well as acne which I believe to be from some sort of digestion issue for about 7 years. I tried everything- from drugstore products to high end, prescription and over the counter. The BEST thing that I’ve ever done for my skin is stop eating dairy and refined or processed food, and drink a lot of water. I can’t even begin to explain the difference in my skin... I have model skin, or “glass skin” if you’re familiar with that term. I also had some congestion, but using a chemical exfoliant (lactic acid) fixed that up within a few days. Also, I’m currently 33 weeks pregnant and have kept up my diet, and haven’t had ONE breakout. I drink kefir and will have some Greek yogurt to ensure my baby gets the calcium she needs. But seriously people... if you have skin problems, cut out dairy and watch your skin become perfect.

No wonder Proactive never worked. All Benzo would do is break me out worse. I started to incorporate Differn about a couple months ago and I saw an immediate difference.

I never wear make up. I’m 37 and now my chin is consistently flaring up. I have never had bad acne until I turned 35. It’s so frustrating. I have tried many over the counter creams, gels, and washes. I also use a micro dermabrasion face wash once or twice a week. Thank you for this video! I need all the information and advice I can get.

I've noticed a difference in my skin when I started working on my moisture barrier. Even though I have oily skin, it was dehydrated. Incorporating Korean skin care is helping

I've been trying to do that for the past month. I think I am using hydrating products and 7 skins but my skin is still oily as ! It's so hard deciding which products is right for me, I'm gearing towards going to see a para medical skin therapist to give me a routine instead :(

@@diane11m93 I've been using differin gel, cerave face wash, vanicream moisturizing lotion and cerave hydrating tinted sunscreen.. for the past 6 months and my skin is free of acne. Also, I noticed that if i dont wash my face right after a workout i break out. Hope it helps

I cut dairy + processed foods out my diet and noticed a complete turn around in my skin. It’s been 5 days and I don’t have any active acne coming up on my face so I definitely can say based on my experience DAIRY, processed foods, & sugar can cause hormonal acne. Do your research on different foods so you can be aware of what your putting in your body. Moral of the story if you don’t feel like products , etc is the cause of your acne change your DIETT ! Extremely life changing
