Prior to formal revisions, Accutane or isotretinoin was contraindicated if undergoing lasers, microneedling, deep peels & energy devices. This was based upon case reports of skin scars secondary to argon lasers & mechanical wire dermabrasion in the 80s & 90s

Skin Science: Accutane #oratane #roaccutane - #isotretinoin increases skin fragility, delays wound healing, & has effects on #collagen synthesis. Keloid formation has been reported following certain procedures, & hence the blanket cover on all procedures over the past 4 decades

Revisions: JAMA 2017 revisions & other evidence based publications revised recommendations with evidence based medicine in mind. Obviously recommendations are worded as ‘no increased risk’ not no risk. Excluded procedures include wire or mechanical dermabrasion. This is different from #microdermabrasion. Fully ablative lasers are also not advised

Caveat: I, along with most other #dermatologist are still mindful of the side effects of #accutane & may modify the dose & timings based upon individual side effects & relevant history such as a personal or family history of #keloid scars, location of scars, density of lasers-energy based devices, considerations with staph carriage & many more. Be guided by your dermatologist as to suitable procedures & timings

I wish more dermatologist were like this. Informative, kind, and especially caring. It should be a rule that dermatologist try to encourage people’s diets, and physical activity before they just sign over medication.

I have done micro needling while on isotretinoin [very low dose] and I had absolutely no issues at all, I tolerated it super super well, even better than without the isotretinoin, but I have to say I am someone who has not even very dry skin from the Accutane.

I guess the tolerance is one thing the other is lower production of collagen in your body due to accutane. So the treatments are less effective while on this medication.

@@greatestever1750 No, it does not, it clearly states in the drug description that it slows collagen production to 6 months after taking it. I had the drug, I have seen it written there. I had doctor tell me that I can’t get minor surgery because of that. Your body makes less collagen since it is harder for your body to properly close incisions. Accutane does not work the same as topical retinol.

Omg, thank you doctor for this info! I was remembering the time when I finished my accutane after done CO2 laser, but I cant remember how long it has been since I stopped one. This week I just done my 6th CO2 laser and I'm thinking to get back on accutane for some reason, with no laser for sure :)

Hi Dr Davin Lim..thanks for the awesome video! Looking for resources for people taking Accutane about the way it might affect the hair and hair life cycle. Is it safe to do threading, sugaring, use hair dye, and will my hair fall out on a moderate titrated dose? Are there steps like doing a test patch, to mitigate risks, and/or reducing treatments? What can people generally expect in terms of sensitivity, pain, healing while taking the medication versus before they started? Currently am 1 month in and it’s going well, and not sure whether to keep my monthly appointments for my grooming regimen. Am in a career where high standards of grooming are essential, and curious what are some methods patients use here? Thanks 🙏

Such an informative video! What is your opinion on various bodies (eg Cancer Research UK and US EPA) saying there NO need to wear sunscreen when UV index is below 2 (e.g., most of the winter in the UK). This goes against what I've heard most derms say to wear it every day rain or shine (maybe because clients use acids?) so would be curious to hear your take.

I was prescribed 10mg A-cnotren which is basically accutane, it said on the package that dermobrazia procedures are not allowed, but my cosmetologist told me to do one session on my body , im not too sure about it, and since my dose is small Id also like to ask if laser procedures would be ok (not on face)

I took only 20 mg but even low dose was bad for ME. I had mild-moderate acne. I am very grateful I became aware early of the side effects in my second round of accutane which was 1 year after the first round. In the first round I took accutane for 5 months on low dose for mild/ moderate acne. It drys your body organs and has caused me mild Fatty Liver Disease(which I caught on early), irregular periods, a fibroadenoma which I have mistaken for malignancy and caused huge pain and anxiety for months. Alternative: Get an estrogen test and your doctor will prescribe supplements to balance your hormone levels, which in turn will clear up your face even better than accutane. I WISH I had known all of this earlier😭

Don’t encourage people to not take accutane. If you were unaware of the risks at the time, then that’s your fault. Some people really benefit from this drug. Anyway, hope you get better.

@@timothydugan5922 You're kind of right. For people with severe acne accutane is their only choice at the end. I literally saved myself, and I was very furious at the time of writing this comment. Plus, all that I mentioned was for People with mild-moderate acne. thanks for your response!

@@ranakamel7772 yeah I agree with that as well. mild to moderate acne should be combatted with something like trentinoin, but accutane is a very strong drug that shouldn’t be prescribed loosely. take care

That’s your dermatologists mistake for not testing ur hormone levels before prescribing you accutane. You r not a candidate for accutane if your hormones are screwed. I had a couple of different tests before starting accutane and my dermatologist told me if my hormones turn out to be the problem she will refer me to an endocrinologist instead. I also dont have severe acne but mild/moderate that didnt respond to other treatments.

Hi, Dr.Lim! I’m on a low dose isotretinoin 30mg per week and it’s been 4 weeks now. Do you think my dermatologist gave me the right dose (I’m 33yrs old, weight 68kg, all blood tests are normal, got many acne on my face) and will I expect skin purging even if I’m on a low dose? OMG Dr. Lim, this video can in the right time cause I just finished accutane in Sept, and even though I have clear skin my face is very much darker then the rest of my body (I am black). I have been thinking about using glycolic acid 10% lotion and help restore my original pigment. Tazorac 0.1% cream is slightly working to help lighten but the major discoloration that I have is very concerning

Hi there, I’m on my 3rd course of accutane (2015, 2017 and started the 3rd in October 2020). I’m black too and YES that’s one of the side effects. It took me 8 months to a year to get my skin tone back for each course. You could use a vitamin C serum and sunscreen (UVB &UVA) consistently. Do not do a thing to your face else you’ll irritate it.

@@abenamainoo1757 I actually didn't use anything....I had to wait it out to see my tone back. Right now that I'm on the medication..I look like I have a charcoal mask. I suggested a Vitamin C serum coz I once used one as a moisturizer and it brightened my skin in 3 weeks. I'll definitely use it when done with my medication. I own a skin rejuvenation clinic in Brisbane. I know of young women that are on roaccutane for such mild acne (which upsets me as I know it's not needed) prescribed by a dermatologist. They are told by their dermatologist that in no circumtances to have any type of skin treatment as it will cause damage, which I also disagree with. They are also told in no circumstances to use most cleansers, serums and creams. 🤷‍♀️

Hello Sir, I recently diagnosed with thyroid and currently I have acne on my face, my doc. Prescribed Accutane 20 mg for a month and after a month again I took a blood test, everything is fine except thyroid level -in a month my TSH went to 11.2 from 7.9, and I am thinking is it because of isotretinoin?? please reply

Hello Dr. Lim Thanks for the very useful video for the people with acnes and skin problems. I’m taking a accutane 10mg 2times per week. It means 1tablet(10mg) per 3days for 1year. My body weight is 50kgs. I think this is really really low dosages. Do I still need to avoid PRP injections? In fact I got PRP injections twice while I’m on accutane with this amount of dosage without knowing that I need to avoid PRP . I didn’t feel a serious scarring at all. I wanna keep going the PRP once a month. Please advise me about this. Thank you !!
