Love your videos! they've helped me out in Pharmacy school and now in Med. I don't want to sound like a doofus here, but I think you've fallen for the sulfa drug allergy misconception. In this video you mentioned that captopril shouldn't be used for patients with a "sulfa" allergy. Sulfa allergy is an imprecise term for sulfonamide antibiotic allergies and Thiol (Sulfhydryl) drugs like captopril have no cross reactivity with sulfonamide antibiotic allergies (SMX or SFD). You're still my favourite supplemental study source, and I'll keep on getting your courses.

Thanks for the info. But it's interesting to also show the benefits of ACEI and it's relation to the availability of bradykinine in the organs that regulate hypertension. Clinical Benefits demonstrated in large trials.

Hi Medicosis, Didn't we say that ACE inhibitors cause decrease in GFR and one of side effects is renal impairment? So how do they work in scleroderma. I searched but found different answers. Thank you

ACE inhibitors seem to help patients with diabetic nephropathy, scleroderma nephrosis, IgA nephropathy, and Henöch-Schonlein purpura. The exact mechanisms behind that are not entirely clear.

They lower the blood pressure and as a response your kidney will retain water (ADH) and the hypothalamus will make you drink water...Both will increase your effective arterial blood volume (EABV), and therefore blood pressure.

Yes, angiotensin converting enzyme (ACE) inhibits bradykinin production, therefore with ACE inhibitors you have increased bradykinin (and thereby dry cough and angioedema).

Angiotensin converting enzyme (ACE) inhibits bradykinin production as well as degrades bradykinin...It’s explained in the 5th slide of this video, as well as in previous videos in this series.
