Achilles Tendon Tear || Ultrasound | MSK || Case 301
Clinical Features:
A 53-year-old male patient came with
- H/o sports injury
- Left ankle pain & swelling
- Difficulty in walking
Ultrasound Features:
- Left tendo-Achilles shows a complete tear with a gap between two edges measuring less than 3 cm with a contour change of the tendon.
- The two separated ends show thickening & hypoechoic echopattern favoring tendinosis.
- Surrounding mild collection & subcutaneous tissue edema are seen.
Remember:
The Kuwada classification of Achilles tendon tear:
Type I:
- Partial ruptures up to 50%
- Typically treated with conservative management
Type II:
- Complete rupture with a tendinous gap up to 3 cm
- Typically treated with end-end anastomosis
Type III:
- Complete rupture with tendinous gap 3 to 6 cm
- Often requires tendon/synthetic graft
Type IV:
- Complete rupture with a defect of more than 6 cm (Neglected cases)
- Often requires tendon/synthetic graft & gastrocnemius recession
