For the past couple weeks, I've had a lot of patients come in with the diagnosis of "alcohol overdose".
Managing each of these patients was different based on their past medical history, whether or not they were a chronic alcoholic, and/or if there was any conflicting drug use.
So, for this weeks video, I want to talk about the clinical manifestations of alcohol overdose and possible treatment options in the emergency department.
We'll be discussing:
- The rate at which alcohol cleared from your system
- Vital signs and metabolic abnormalities seen in acute alcohol overdose
- Wernicke’s and Hepatic encephalopathy
- How to test for asterixis and what this means
- Gastric lavage and Charcoal - are these still offered?

I enjoyed this as these patients are verrrry common in the ER I practice in for my PA residency program as well! One thing I find difficult with managing and staffing these acutely intoxicated patients is some of my attendings do NOT get an ETOH level (serum or breath) on them especially if they are chronic users because in situations where they are generally well appearing and just need some time to MTF (metabolize to freedom) they don't want a high level ETOH on the chart to cause issues delay in dispo or legal i.e. level being too high even though they function well at that level or say something happens to them after discharge (ped versus car, etc) despite normal ambulation/neuro exam etc. I know sometimes people appear intoxicated too where they may be encephalopathic from another metabolic derangement or from wernickes/hepatic etc. Could you discuss more about what you do in these situations or is this why you just get the ETOH level every time regardless?

Thanks for the Video clip! Sorry for the intrusion, I would appreciate your thoughts. Have you considered - Patlarny Bloods Victory Principle (just google it)? It is a good exclusive guide for preventing toxic blood and heart desease minus the normal expense. Ive heard some amazing things about it and my m8 got amazing success with it.

So I have a question, can alcohol intoxication settle in to the body the next morning after a night of drinking?? Cool video by the way

Excellent how you explained the mechanisms behind the pathologies related to alcohol overdose and ddx of 2 very similar conditions!! Definitely waiting for the alcohol withdrawal video! Keep it up!

Great video, helps with the understanding of why different labs, meds are ordered when you see alcohol intoxicated patients admitted to your unit.

This was a really helpful video, these patients are very common. I didn’t know that gastric lavage and AC aren’t really used because they aren’t helpful by the time they are in the hospital. It was also really helpful clarifying asterixis, I think most of us know what that is but would have no clue how to elicit it, additionally I didn’t realize that an opioid OD would also show that.

I love to watch medgeeks video.. N this topic are not common as majority of races in our country don't take alcohol however we still have minority who sometimes was admitted for these reason, and I learnt a lot, thanks for your efforts to help us to be better physicians
