ADHD is currently understood to be a disorder of inattention, impulsivity, and usually hyperactivity that arises in childhood or early adolescence and is highly persistent over time in most cases. However, since the first medical papers have been published on ADHD starting in 1798, emotion has always been included in the conceptualization of the disorder up through the 1970s. But beginning with DSM-II and progressing to the present, emotional dysregulation has been excluded from the clinical conceptualization of the disorder and the diagnostic criteria and relegated to an associated problem or the result of comorbid disorders. This presentation reviews the evidence from the history, neuropsychology, neuroanatomy, and observational research that shows that emotional impulsiveness and deficient emotional self-regulation are an integral part of ADHD. Returning emotion to its rightful place as a core feature of the disorder also serves to better explain the development of comorbid disorders, such as oppositional defiant disorder, and well as various life course impairments. Dr. Barkley will discuss how to determine which aspects of emotional adjustment problems in ADHD cases are the result of the disorder and which are likely to be the consequence of comorbidity. He will also address the implications of including emotion in ADHD for their management.
In Part 3, I discuss the clinical implications of putting emotional dysregulation back into our view of ADHD for the diagnosis and management of the condition.

First and foremost, THANK YOU THANK YOU THANK YOU. your videos have been a great source of knowledge for me as. I appreciate all you have done and continue to do. I know unrelated to this particular video, but do you have any research or expertise on girls with adhd that seem to get diagnosed later than boys. Some not til they are tweens and already going through teenage angst, with strong opinions of their own. You mentioned in many videos that adhd is not a function of knowing what to do,but doing what you know. Which makes complete sense to me as i see my daughter struggle with this exact thing. Are their tools you use that work in getting the teen with adhd to actually DO the steps. I get answers like mom i KNOW i need to set an alarm or put a sticky note, but that in itself gives me anxiety. Any video on teenage girls with adhd would be so helpful. thank you

Dear dr Barkley, I recall hearing you say that antidepressants are a bad idea if you are adhd. But I cannot find the video. Are you able to help? Could you please explain why? What if you are adhd but have a severe anxiety disorder for which antidepressants usually helps. My daughter has severe social anxiety (selective mutism) and we are discovering is adhd. We are worried her ssri might not help her adhd…many thanks

Hello Dr Barkley. Thank you for sharing your work here on KZfaq. You mention medication to address emotional dysregulation. Could you point me in the right direction as far as medication is concerned? I’m interested to know which medications help with emotional dysregulation. I watched your videos on ADHD meds, just wasn’t clear which one specifically addressed emotions. I know that stimulants help with focus and attention. Would these also help with emotional regulation?

@priyabriggs659 - He stated that both the stimulants and non-stimulant ADHD medications can help with the emotional regulation. The stimulants, if I understood him correctly, work from the 'bottom up' where the emotions originate in the lymbic(sp?) system. The non-stimulations help with, again - my understanding here, the regulation from the 'top down' where the original emotion is regulated by the more thought oriented bits of the brain.
