On 10/8/19 NS 25A was heading south through Hamilton OH at approximately 1:30 am. There was a dense fog in the area and it is unknown what signals awaited the train as it made its way south and then east on the NS New Castle District. What is known is that just beyond the second signal they came to on the NS (Mosler Holdout) sat the rear end of NS 143.
NS 25A failed to stop and rear ended the 143 damaging both lead locomotives from 25A and the car immediately behind them and the three rear cars of the 143 derailing them. The eastern most car was a wood chip car and was re railed.
The western pair were tankers containing Butane gas and they took the brunt of the hit. Both cars and locomotives were smashed together and could not be separated at the scene. Once the main body of the trains were pulled away from the wreckage they drug the whole pile up back west to a siding track called the Hamilton Yard Track. Here it took quiet a while to separate. This video shows the rest of 25A being taken back North to a siding in Camden, the locomotives being pulled to the siding, and the separation of the cars and locomotives. The crew is ok from both trains with one member of 25A going to the hospital with minor injuries.

I know the signals in this area were just updated to the new "PGR" system but I don't know if it was active yet in this area but if it was it should have prevented something like this from happening. but this area is a chock point for both NS & CSX were both of the CSX two main and the one from the West have to combine with NS main with only 2 mains and I believe NS & CSX should at lest bring back a third main in this area like it was many years ago to cut down on the bottle neck through Hamilton. Some of the old Cincinnati Bell prints I have seen shows there us to be 4 mains going into the old train yard and roundhouse with sidings for the paper plants. yest the old right away for the 2 western mains is now 3rd ST (SR-127) but North 5th St were the sidings for the old paper plant being torn down is still there and I can see with some realignment being able to fit 3 Mains in the same right-away with little investment and down time and improve the safety of the rail interchange. but the location I do see a large investment in is the bridge crossing North across the old canal at the power plant and the river crossing before CSX & NS split again in New Miami if they want to keep it 3 mains all the way.

Very overjoyed that the pressure tank cars held!! Procar is canadian brand name but tank car itself could have been fabricated in East Chicago, Indiana. Texas or one other state - in addition to canadian manufacture. About 1990, Procor should have been designated for the pressure car builds and east chicago for the 111 atmos. tank cars.

Max Shelltrack it was late at night and very foggy out. When 25A took the signal at the 29.0 the engineer was just flat out wrong. The conductor argued that it was a slow approach while the engineer thought it was a slow clear. When they came around the corner to take the Mosler Holdout signal it was of course a stop indication and the end of NS143 was just beyond the signal. There was no way they could stop in time.
