Disease of chronic or prolonged hypercortisolism (excess cortisol)
Commonly causes:
Iatrogenic (excess use of corticosteroids)
Hypersecretion of ACTH from a pituitary adenoma (Cushing’s disease)
Adrenal cancer (adenoma or carcinoma)
Adrenal hyperplasia secondary to ectopic ACTH or CRH production (by small cell lung cancer, pancreatic, thymus, bronchial cancers)
Not suppressed (cortisol nl/↑)
Suppressed (cortisol ↓)
Approach to Cushing's syndrome
Clinical presentation of Cushing’s symptoms
24 hour free urine cortisol
AND
Low dose (1mg) o/n dexamethasone suppression test
↑ urine cortisol
AND
Not suppressed (cortisol unchanged)
Suppressed (cortisol ↓), normal result
Not Cushing’s syndrome
Suspect Cushing’s syndrome
Measure plasma ACTH levels
ACTH ↓
ACTH nl or ↑
Suspect adrenal tumor. Confirm with CT/MRI and resect
Suspect extra- adrenal tumor (ACTH-dependent Cushing’s syndrome)
High dose (8mg) dexameth. suppression test
Ectopic ACTH. CT/MRI abdomen (pancreatic cancer), chest (lung ca)
Cushing’s disease; ACTH from pituitary tumor. Confirm with MRI and resect transsphenoidally

wow amazing detail. My wife was recently diagnosed with "drug Induced" cushing. She had been on depression/anxiety meds for well over 30 years. Around 7 years ago she became drug resistant to all depression/anxiety meds. She is currently being treated with Korlym and it seems to be working. It does take 1-3 years to retrain the body to regulate cortisol

What if the patient has both an adrenal tumor & a lung nodule? How do you determine which is causing the high cortisol levels and determine Cushing syndrome? The patient has a 3cm adrenal tumor that presented on a CT scan 12 years ago being 0.5cm at that time & has more than doubled in size since, now 3cm. Recently, the patient presented to the ER dx of sepsis, a random CT scan presented a 7mm lung nodule & noted the increase in the size of the adrenal tumor.

Great video thank you for sharing this information with us all. I am a patient that has a 35mm tumor on one of my adrenal glands and have all the physical attributes of Cushing syndrome, I have never been over weight in my life and have always been active but from going from dangerously high bp levels 215/120 for the last two yrs and putting on all this weight. i am at the end of my tether with it! i have reasontly been tested and have the blood test results of the suppression test. My acth level was low 0.9 and my blood sugars was high and also my Lipids were also high the cortisol level says it was at 24mmol I havent seen the specialist yet because she postponed me and everything is done contactless. I am really concerned that i may have cushing syndrome. The moment i do anything im in so much agony in the abdomin and lower back. I sweat profusly. I have small scratches that just wont heal. Pleased can you give me any idea as to what i can do to help myself or to elevate some of the symptoms so i can get through the next two wks. From a stressed out paient. Thanks for reading this

how does a tumor secrete ACTH? and also how does a negative feedback responds to the excess ACTH produces by the adenoma in pituitary gland?
