Do either of you happen to be diagnosed with BP one disorder? I've recently been diagnosed with that and I am super anxious about taking the medication I have the 10 mg I'm supposed to do my first week at only 5 mg in my second week increase to the full 10 mg dose I was on lithium once but that was not very that was awful that one did not work for me at all however I keep seeing all these it makes me so drowsy side effects and I've took it three days now and I've not been made drowsy at all I'm kind of the opposite it's made me a little jittery I don't know I'm just wondering if It's even a normal prescription for DP-1 disorder because so far I've just heard a lot of people talking about schizophrenia borderline personality and major depressive so I'm just trying to figure out if it's something that is regularly prescribed for BP one or if I need to ask about trying something else?

I requested tapering off of Abilify a few months ago because of fears about the long term effects such as Tardive Dyskinesia and am having very noticeable withdrawal symptoms I think. It was originally prescribed to help me with anger issues but ever since I took my last dosage about a week ago, I have felt exhausted and fatigued as well as depressed. This is the opposite of what I was worried about given the whole reason why I started on it years ago was for anger management, not depression. Depression has never been a struggle for me before until I got completely off of Abilify.

This medication is modulating the dopamine pathways which can have a positive effect on mood. Many times this medication has been proven to work as an adjunctive treatment for depressive disorders not fully treated with other medications. I would talk with your doctor and find out what options there are including restarting at a lower dose and tapering more slowly. One thing I can say is the half life of this medication is 96 hours and it will take 20 days to fully clear from the body so after one week there would still be a lot of medication in the body. Hope this helps

I was on 2 mg Abilify as an add on to my antidepressant. I loved it! I honestly had never felt so good. I was waking up early (never been a morning person) and was motivated to get things done. It also totally zapped all my anxiety. Unfortunately the only side effect I had was weight gain. I gained over 10 pounds in about 6 months. I go to the gym 3-4 times a week so this was a deal breaker for me. I went off it and the weight melted off quickly. I still haven’t found a med that worked like abilify did. I heard Latuda worked similarly to abilify with less chance of weight gain. However it’s super expensive and my crappy insurance wont cover it unfortunately. Sometimes I think about just going back on the abilify and dealing with the weight gain.

My husband, age 47, was diagnosed 10/2018 schizo-affective bipolar was on pill form of Abilify, but wasn't consistent & after another psychosis has now been on Abilify Maintena (400) injectable for 2 years now. Given the fact that we were having some marital issues before this psychosis the side effects weren't an issue for him because all he really cared about was that his psychosis and hallucinations were gone. I hadn't put 2 & 2 together about the side effects until more recently, which I will do my best to explain here & would greatly appreciate feedback and/or suggestions. In conversation about our marriage, he will tell me that he wants to make it work, but his actions don't ever match his words. However, I wasn't attributing him being this way to the medication, at first. He hasn't always been the best about keeping his word, but this was getting ridiculous. Then a series of things happened kind of close together when it was beyond actuons not matching his words- I was more or less asking "Who are you?" Then I found some older text messages between us and I swear I fell in love with him all over again (sap, I know, lol). This was the icing on the cake of events I thought to myself, "woah, his empathy & compassion are completely absent. Being down memory lane made me realize just HOW DIFFERENT he had become. That's why we're not connecting & he is unable to keep his word. I was under the impression that he was just being lazy about working on the relationship. Every level of intimacy is gone (mentally, emotionally, physically). When I tried to have the discussion about potentially tapering & trying something else because I am not only worried about these effect in the longer term for him individually- but I have essentially been without intimacy, and my husband for years now & know I don't want to live this way forever.

This is not medical advice, and you need to talk to your doctor before making any changes to medical treatments. I think what we need to always keep in mind is what the disease process itself does to a person over time with or without medication. Almost all severe mental illness affects cognitive function. Some people do experience emotional blunting from medication but complete changes in personality would not be a common occurrence. I do not know his case, and I really cannot comment on it directly, but I think it’s difficult to tease out which part is causing what, is it a rare side effect of medication or just the disease process itself over time at 47 years old I would assume it’s been going on for some time. Sorry I can’t provide a more robust answer, I simply don’t know enough about the situation. Hope this helps, if you haven’t subscribed to the channel please do and spread the word about the community we are building here.

just another Q, so if you're not a schizophrenic and say don't have an excess of dopamine at all (like a super depressed person example) you don't have to worry about Abilify further decreasing levels of dopamine? It should only ever increase dopamine in someone who lacks it whether lacking in the mesolimbic, mesocortical pathway etc.? great vid btw, subscribed :D
@cornoc
@cornoc 6 ай бұрын
No, it’ll reduce the activity of dopamine in brain regions where dopamine neurotransmission is high, and increase activity in dopamine-sensitive neurons in regions where dopamine activity is low, regardless of whether those activity levels are normal or pathological. It’s not a miracle cure, and it can easily boost or dampen dopamine activity inappropriately depending on your neurophysiology and the part of the brain it infiltrates.

Hello, my 23 year old daughter was initially prescribed an SSRI for what her psychiatrist diagnosed as depression. As her depression worsened the dose was upped 3 times. When sitting her 3rd year exams for medicine she went into a full blown manic episode where she thought she could speak 20 languages etc. After 6 months in a psych hospital her meds were 1000mgs of lithium with 300mgs of seroquel. 2 years on she is now on 600mgs of lithium and 50mgs of seroquel. The problem now is her anxiety is very high with low mood. We reduced the seroquel because she was unable to study or concentrate on her course work so had to give up her dreams of becoming a doctor. She is now hoping to return to uni to study forensic science which is why we decided to reduce her seroquel and lithium. Blood lithium steady at 0.6. she has now been prescribed abilify 0.5mgs to start then onto 10mgs. Please could you let me know what you think of this drug regime. Her manic phase lasted 3 weeks, 2 years ago with no other manic episode. Her main problem is anxiety. I would really appreciate any feedback. Sorry for long post.

Hers my problem I can't find any research where the animals serotonin or dopamine levels were measured, I can't find many people who had any tests for these or mri either, so my wustions are 1. If you play with dopamine and serotonin and they don't actually have a chemical imbalance what are the dangers!? 2. The side effects of abilify I've seem people complain of are Burning skin Skin on fire Blurred vision Confusion Agitation Tremors Movement disorders Akathesia Tardive dyskinesia Now can you call this a cure when these sound like symtoms of a poison in the body and many say have Drs been ruining their health and causing new disease and disability! People can't function basically after these drugs ssri's, antipsychotics it seems we're based in very many cruel and flaws animal experiments they pharma industry sold doctors chemical cures and down played the symtoms of the drugs, they are the things they call side effects! One side effect is it ruins people's lives! Too many kids are now being targeted when doctors miss basic things like vitamin deciciencies, hormones, food additives, allergies, autoimmune disease, poisons like heavy metals lead in water, arsenic, heart attacks how do in know!? I had all of those things er Drs misseed and was sectioned accused of psychosis the only people deluded and psychotic were the staff to be honest! I had my eyes opened to the abuse the coercion the bad decions that cost lives the many peopen harmed held illegally accused unfairly misdiagnosed and abused, Autistic people vulnerable victims of domestic violence and crime blamed by doctors and failed by them due to their own poor decisions and Lack of insight inability to follow regulations or even treat people with respect and decenc, unsure sad a lot of abuse and misinformation, a lot of people harmed by excessive use of drugs, khciatrists in denial about all new symtoms of disease crated by these drugs and all the misinformation from doctors from pharma! And healthcare! When medicine makes people sick it is not medicine! A heart attack missed being irritated with staff for not listing symtoms isn't psychosis, I don't know sjt happens to common sense but please start lokkng at evidence of people harmed by these very flawed and toxic chemicals called drugs! Look at the animal experiments, read the research before etou go harming people make sure you've done all the right blood tests and don't accuse poelenkf things that are not true! Gaslighting is what abusers do to their victims so when doctors and nurses do it to victims of crime or those who are sick it's a double edged sword another knife to the heart, another life destroyed! Apiprazole is an extremely dangerous drug. "they do not come without side effects," is how doctors like to say it. The drug trials for aripiprazole were sub par, and many of the people accounted for during the entire studies of the trials had dropped out prematurely, so the data from the trials are extremely inaccurate. Aripiprazole turns off the impulse control box in the brain. Studies show that antipsychotic drugs reduce brain size, among several other extreme dangers to the human body.

Hi, I'm on 2 mg of Abilify in the mornings, an ostensibly low dose, and I'm experiencing side effects like extreme fatigue, muscle weakness, joint pain. Is this typical? I've been on the drug for over a month. I'm also taking 200 mg of Lamictal at night, which I have been for over a year. Something else that may be relevant is I just started taking a small dose of CBD at night. Could that be interacting with either the Abilify, Lamictal or both?
@ShrinksInSneakers
@ShrinksInSneakers 2 жыл бұрын
could be related to CBD which is a CYP2D6 inhibitor which may increase abilify plasma levels. In general combining abilify and lamictal is a reasonable option for bipolar disorder. Hope this helps, if you haven't subscribed to the channel please do and spread the word about what we are doing here.

My general rule is 5-10 mg/day for depression and higher doses for schizophrenia and bipolar disorder. Most people with schizophrenia or bipolar disorder will need at least 15 mg/day and in some cases up to 30 mg/day. Now if this is the first medication you are taking and have no prior history of medication use, I might start with 5 mg to assess tolerance and limit side effects. The goal is to keep a patient on a medication until they are stable, starting with too high of a dose of titrating too fast can result in people coming off the medication shortly after starting which is not the goal. I like abilify and ziprasidone as first line options for patient who have not taken medication before, even thought the efficacy may not be as high, they have lower side effect potential and are weight neutral medications. Hope this helps

@ShrinksInSneakers
@ShrinksInSneakers 2 жыл бұрын
This is not medical advice, it's for educational purposes only and you should talk with your doctor before making any changes. There is good evidence for aripiprazole in bipolar mania, major depression, and schizophrenia, the evidence is far less convincing for a pure anxiety disorder. There is some evidence to support the use in anxiety disorder but it would not be my first line or even second line option. If there is MDD with anxious distress, I would consider using it because of the evidence for augmentation in depression in the dose range of 5-10 mg/day. So it can work but is not a first line option. Tardive dyskinesia is a rare condition that is less likely to occur in second generation medications and partial agonists like aripiprazole. It can still happen but I have not personally seen a case clinically from this medication. I think it's always important to consider the severity of symptoms and level of dysfunction when deciding the level of side effects you are willing to potentially face. Akathisia is far more common side effect with this medication. I hope this helps and if you haven't subscribed to the channel please do and spread the word about the community we are building here. @ShrinksInSneakers
@ShrinksInSneakers 2 жыл бұрын
With bipolar I disorder unfortunately many doctors have moved away from using mood stabilizers as the primary treatment. There has been a substantial increase in the use of dopamine blocking medication as the primary medication choice in bipolar disorder. This case is a little different because the manic episode resulted in psychotic features. In that case a dopamine blocking medication like Abilify makes sense. These medications will interfere with the dopamine reward pathways and could theoretically resulting in things like a reduction in motivation. It can also cause sedation which will make you feel tired and unmotivated. I like a combination of a mood stabilizer and Abilify in cases where psychosis is present, it also leaves you with the option of tapered off the Abilify once the psychosis resolves and using something like lamotrigine for example (I have a new video up on it check it out). If you haven’t subscribed already, please do, and spread the word about what we are doing here.

@ShrinksInSneakers 3 жыл бұрын
I would defiantly talk with your doctor about this, and I will provide my general thoughts for educational purposes, this is not medical advice. It’s not specifically indicated for panic/anxiety, and I think there are better option for anxiety and panic attacks. Are you sure the anxiety and panic are related to the buprenorphine? If so, it might be better to restart the buprenorphine at a low dose and see if the symptoms go away. Then you would begin to taper off the buprenorphine at a much slower rate over a longer period of time. I could be totally incorrect about how I’m approaching this because I only have your comment here to base my answer off. If this is not the case and the anxiety and panic predate the substance use disorder and buprenorphine treatment, then it is probably not the medication taper that caused the issue. I think a more traditional approach in that case such as CBT alone or in combination with an SSRI medication would be a good place to start for anxiety and panic depending on severity of symptoms.

This is not medical advice, it’s for educational purposes only. Always talk to your doctor prior to making any changes. My answer would depend largely on what the diagnosis is and what symptoms are being targeted. This is almost all the medication classes, the only thing missing is a benzodiazepine. When I this I always question the diagnosis. If for example it’s bipolar I depression the lithium and aripiprazole is a reasonable combination. Some have said bupropion is the antidepressant least likely to induce a manic episode, so people have made it the go to option if an antidepressant is going to be added. I’m plus or minus on the bupropion. I hope this helps, if you haven’t already, please like and subscribe to the channel and spread the word about what we are doing here. Abilify is not a dopamine blocker it's more of a dopamine modulator and it may balance dopamine levels in the prefrontal cortex for adults and children with ADHD. There is some evidence but i would reserve this option for people who remain symptomatic despite adequate treatment. Also stimulants have one of the largest effect sizes in all of psychiatry around 0.8, so they work. Hi, I was just prescribed 5mg aripiprazole by my psychiatrist over the phone. I get the prescription tomorrow. It is likely I won't be able to see my psychiatrist face-to-face until May. I am already taking valproic acid (for bipolar 1) but I've been having some issues with it lately (blood levels dropped due to my ketogenic diet!!) Akathisia is the thing I'm a bit worried about because I had that really bad in the past from something else. If I get akathisia and it is mild enough for me to put up with should I be OK to push through and wait for it to subside, or would that be a sign to stop immediately??

I’m currently taking 5ml of Bipolar 1 which is more on the depressive state. It’s been very helpful. I finally can work again, have energy, want to go to the gym and can do all the things that felt like moving mountains before. However, I do feel some restlessness. I’ve been on the drug for about a week so is that something that eventually goes away?

@ShrinksInSneakers
@ShrinksInSneakers 2 жыл бұрын
I’ve never seen these side effects in clinical practice, and I would not classify them as common reactions to the medication. Some of this will also depend on what dose you were on. Abilify has a long half life about 75 hours so it would take 15-16 days to be fully removed from the body. At 19 days you should be fully clear of the medication, I would consider other possibilities for the symptoms as well. If you haven’t subscribed already, please do, and spread the word about what we are doing here.

@ShrinksInSneakers
@ShrinksInSneakers Жыл бұрын
This is called hedonistic homeostatic dysregulation and is due to the mechanism of action of this medication. It's a partial agonist and if it's used for a disorder like depression it is likely to increase dopamine transmission resulting in over involvement in pleasurable activities like sex or gambling. This is something to watch out for and you should talk with your doctor if you notice an increase in these behaviors. Hope this helps, if you haven't subscribed to the channel please do and spread the word about what we are doing here.

I would not say it’s a common side effect, I never dismiss anything like this it’s always possible to see rare side effects and everyone’s body is different. An important point is most psychiatric conditions have some cognitive disturbance. Disorders like schizophrenia for sure, but even depression and anxiety can cause issues with memory and concentration. There have been some studies that indicate potential cognitive enhancement with aripiprazole in patients with schizophrenia and bipolar disorder, although I would not necessarily use its cognitive benefits alone.

I would Really like to see and would appreciate a video on anti-sychotic selection. Like all things being equal why do you choose one second generation anti psychotic over another? For example, why would you choose to use abilify over vraylar, or vice versa? How does what symptoms you see influence that? I understand minimizing metabolic risk and EPS risk etc....but beyond that? Like a combination of evidence and anecdote. This is something I struggle with and would really like more insight into. Thanks for all you!

Sure, sounds like a good topic. A lot of medication selection is about side effects. If you have a person for example who is first break and new to medication you do not want to start with medications like zyprexa. You want to use medications with the lowest risk of side effects and help the persons adjust to the process of taking medication. If they have schizophrenia they will need to be on medication for many years its best to reserve these medications until the need arises. There is more to it but side effects is a big reason for using second generation medications. Hope this is start to understanding

Does 5mg has significant more side effects than 2.5mg as ssri augmentation? Does 5mg has additional efficacy compared to 2.5mg as ssri augmentation for ocd or depression?
@ShrinksInSneakers
@ShrinksInSneakers 3 жыл бұрын
With OCD make use the dose of SSRI is optimized, most people will need higher doses and longer duration 6-8 weeks minimum for OCD. That would be the first place to start. when using aripiprazole for depression the dose is between 5-10 mg/day. There isn't much data to support higher doses in augmentation for depression. I do not think there will be much difference in the side effect profile but the 2.5 mg/day is very low and is usually how I would start someone who has experienced side effects in the past, or is apprehensive about taking medication.

@@ShrinksInSneakers Hello. At what dose/length of using does one need to worry about tardive dyskinesia with Abilify? thanks :)
@ShrinksInSneakers
@ShrinksInSneakers 3 жыл бұрын
@@carladalcol183 Tardive is less common with second generation dopamine blocking medications, not to say it cannot happen. Akathisia is more of a problem with abilify. It's a common misconception that tardive increases with time. About half of cases occur within 5 years of starting medication, at a rate of 5% per year roughly. Thus tardive is more likely to occur within the first few years of using the medication. There are only a few proven risk factors they are: age (older patients starting medication), female gender, and schizophrenia diagnosis. Schizophrenia itself causes tardive, it's not just medication. So you can see this is a difficult question and exact answer is difficult to provide. Monitoring with an AIMs scale is a good way to monitor. I would place an educated guess of about 0.5% at 1 year for newer second generation dopamine blockers in patients with schizophrenia. I think it's even less common in patients using these medications for mood disorders. Hope this helps a little
