I think the word "autophagy" here is used in a metaphorical way. It refers to the act of bearing your own repressed emotions and how these emotions turmoil inside your heart and start killing/eating you inside out (autophagy means self-eating, it is a medical term). As it progresses, we see that the char in the song gets closer to not recognizing their self anymore; they wish to stop feeling so hopeless and want to end it once and for all yet don't find the strenght to do it. In the meantime, they start hating everyone that gets closer to them because, essentially, they don't know how to express what they are feeling, that is, they feel like a permanent, shallow facade that keeps just breathing rather than living (also a reference to the term, whereas cells breathe, and when they don't, it's because they're dead). They hate the fact they keep overthinking meaningless things ("my intercerebral guide is a bit strange isn't it"), so in the end they find everything and everyone utterly folly and stupid. Nothing else truly matters to them. This is what it feels like to trust no one.
