The best position to sleep after a total knee replacement is the one that allows you the best night's sleep.
Sleeping after a total knee replacement can be the most frustrating part of recovery. The conventional advice is to sleep flat on your back with the knee in full extension. The problem is many of my clients have not been able to sleep in this position for years. Spinal stenosis, sleep apnea, and other medical conditions make sleeping in this position impossible.
Large bodies of medical research have suggested sleep is a vital component of recovery. This research suggests that one night of interrupted sleep may increase stress hormones, increase blood sugar levels, increase pain sensitivity and more.
If you are working on improving knee extension during the day and you are not experiencing significant loss of knee extension when measured by yourself or your physical therapist then it is reasonable to consider sleeping in any position that affords you the best quality sleep.

Just had my second knee done, first one had a pillow under the joint, took forever to get it straight. Second knee slept on my back, knee straight, much quicker healing imho. Going on 5 weeks hardly need a cane anymore.

My surgeon told us NEVER to sleep on our back with a pillow under the knee. (There were a group of about 6 of us in the knee rehab place and we worked out that he said the exact same thing to us all!) He told us to sleep on our backs. Some people found that really hard, as normally they'd sleep on their side. . I slept on my back with my heel hanging off the end of the bed as a compromise. It was 3 weeks before i could just sleep on my back with the whole foot also on the bed. I have been having extension issues big time, sure, but there is no way i would have got a wink of sleep had i not slid down the bed a bit, (in 1st 3 weeks)and let my foot hang off the end. Thanks for sharing your story. I hope your extension is reliably equal on both legs now.

I'm 19 days out of the knee replacement and I tell you if it wasn't for watching this video and my pain meds for the other problems in my body I would not be able to sleep. I'm getting better but the past few nights have been horrible. Thank you for the video.

4 months tNR and still can’t sleep more than 2 hours at most. My knee wakes me up with Hot throbbing pain all night. It’s terrible. I have 105 ROM but pain never goes away.

Thank you so much for sharing this. I'm 2 weeks post-op and wake up every 1.5 to 2 hours because I'm so uncomfortable. I'll try using more pillows tonight. I am using an ActiveCare calf compression machine, and that adds to my discomfort, what with the wires and noise and squeezing. Stitches come out tomorrow, that should help with comfort. Onward!

3 months post surgery. Just keeping things real. Was not able to sleep in my bed til week 10! Slept in a recliner with a heating pad behind my lower back. It'll get better!

It is so hard to heal when you can't sleep and it is so hard to sleep when you can't find a comfortable position.

My mum had a knee replacement a month ago and I recommended sleeping with a pillow under knee as it is comforting also because a good sleep aids recovery. However physios and many others have told her this causes the knee to be bent during sleep and can also cause an ongoing limp. As painful as it is the idea is to get that knee as straight as possible soon after op.

I was rtolod by my therapist, not to bend the knee when I sleep, so I was always sleeping with the leg extended. Now I am bending it. She had mentioned it might do something to the leg if I did bend it during sleep, as in not being able to extend it while awake.

Thank you for this. I am looking forward to my first knee replacement in 6 weeks. I'm having a terrible time sleeping now because my knee is so out of alignment that my hip is affected horribly. I can't get away from the hip pain no matter what I try. I'm hoping that is resolved pretty soon after surgery. I'm so arthritic I can barely manage to turn over in bed as it is. I hope I can put this information to good use afterwards. Thank you again ❤️

@Scribblescrabbly that knee is doing well. I've had to also have a right hip, and just got my second knee replacement 4 days ago. In between I've had many other smaller surgeries, totalling 10 in a year and a half. Hopefully I'm done now lol. What a kindness to ask. God bless you my friend. ❤️🙏❤️

Week 2 post op and can’t sleep, I’ve tried one pillow but I’ll definitely try 2 tonight. My physiotherapist said not to sleep with a pillow under my knee? I think that might help me sleep though, does that effect being able to straighten the knee? I’m finding your videos very informative thank you for your advice 😀

I'm 5 weeks out of a TKR. Range of motion is 115 but still using a cane. Sleeping has been a nightmare. I average 3-4 hours of broken sleep per night.

I had to sleep on my back for about 10 days before I could sleep in my side. I'm 1 month and 3 weeks out from surgery and now I rarely need a pillow between my legs and it feels awesome to be able to sleep solid through the night.

My attempt to sleep on my side after Knee surgery did not happen until my 8 month period after surgery...( A pillow between the legs did not help)..Laying on my back was my savior during 7 plus months...Now that I can lay on my side...I don't require a pillow and I am resting much better.

I’m 5 weeks out from partial medial right knee replacement. This may not be a popular statement but it it what it is. I take zero pain meds during the day and ice frequently as I’m doing a lot to bend it. At night, I use pillows as he said and vary them but the only way I’m getting solid sleep is norco. 10mg/325. I don’t like taking high doses of Tylenol or advil as it’s hard on your liver. My plan is to take it two more weeks and then switch to 400 or 600 of advil. There I said it. Yes it can be addictive but I had a friend who died of kidney failure as he took too much excedrin every day for years. I JUST WANT SLEEP!!!

I am 12 days post surgery. I have been sleeping in my recliner. I use a pillow under my calf and it keeps my knee fairly strait. This also allows me to make adjustments during the night. I have been a 0 on my extension for a couple of days. My flexion is tough though around 90 to 95 five as the day goes on.

I’m recovering from knee surgery….having a pillow in between leg doesn’t work for a while we have an incision inside the inner side of the knee which for a long time is impossible to sleep I KNOW at least for me now no problem but it took a while. FYI

Timely question please Tony; I am a stomach/side sleeper and in one of your videos on sleeping you said that getting a good stretch of sleep is essential to recovery, so if it negatively affects the knee's extension that is okay; it CAN be worked with. Getting proper sleep (not like pre TKR, but a few good solid hours) is essential. SO, my question is, how soon after TKR can we side/stomach sleep, in the way you instruct?

Getting for total knee replacement on right side. This is a concern for me to be able to sleeep. Been looking at wedge pillow made for one leg to keep leg elevated. Not sure what to do at this point.

Had my knee replaced 3 weeks ago and it's been a frickin nitemare. Getting comfortable has been almost impossible. I wish I could sleep on my back but I never could. I noticed you put the pillow right under your knee when you were lying on your back. Isn't that what we're NOT suppose to do. You're the expert.. just thought I would double check.

Melanie aka Starlitblond honestly there is no right answer. Getting sleep is the top priority. Many will say don’t sleep with a pillow under your knees because it will limit your extension, but if it helps you sleep and you work on extension during the day it will have little negative effect. What is your preferred sleeping position before your knee replacement?

My surgeon told me never leave the knee bent for any length of time. That includes sleeping at night. I was told To sleep on my back with the leg outstretched and the knee as straight as possible. And that’s how I slept. Did I sleep good? No it was horrible, as I am a side sleeper. The way I managed to get some decent sleep was I made sure I took my regimen of pain pills before bed, my surgeon prescribed a 12 hour morphine tablet and oxycodone every four hours for break out pain. It worked like a charm. But there’s no denying those first couple weeks are a real challenge. I’m happy to report I am almost a year out from my surgery and my knee works wonderfully. One of the best things I ever did for myself!

I am waiting for a total knee replacement in both knees (not at the same time). I have heard about sleeping on my back and keeping the leg straight. I can have a pillow going lengthwise, just not under the knee

Wedge pillows work well too. It really all depends on your knee extension and swelling after surgery. People who are gaining extension normally do not need to be so strict about sleeping on their back with their knee straight.

I’m two weeks out from my second TKR in the last six weeks, invest $30-40 in a memory foam wedge sleep on your back, it will help with swelling big time, well worth it

My surgeon said to sleep with immobilizer for 2 weeks. But so painful, I can’t stay asleep. Then I let my heel fall off the edge of the bed and now my hip hurts. How straight does my knee need to be? When is enough enough?

I had 2 knee replacement in right and 2 left.I was never able to sleep on either side, is it a bad thing to be a back sleeper? I know patients heal and recover differently, but for me who has fibromyalgia, RA, Htn, poor sleep habits due to pain. I just want my life back, but expressed my issues to my doctors, however I try PT, and use my lymphdema machine as they recommend, But its getting nowhere fast. Very frustrated!

My orthopedic surgeon says don't sleep with knee bent. Sleep on your back with knee straight, even with pillow under the foot to allow the knee to drop down.

The nurse told me not to have a pillow higher than my calf and not under my knee. Was that just in the hospital, can I sleep with a pillow under my knee now that I'm home?

I use my dog's leash to wrap around my toes and then I just swing my leg while I'm swinging myself onto the bed😊

My physio said to keep knee str at nite to lie on bk and not to use pillows under knee or in between and then knee locks as it bent and will n hard to str out again..like who do u believe

I was told by my PT not to sleep with a pillow under the knee because they want it straight... I tried the pillow last night and hit a solid 5 hours! I don't want to do any damage that could cause a set back . Not getting sleep though is just exhausting. Thank you for this video

Sleep is the priority. As long as you work on range of motion during the day, getting a decent night sleep will do more to help you get to your goals than being miserable and awake all night.

I'm already 8 wks post-op , but until now I still can't have a comfortable position in bed. I already tried so many ways but each position gives ne so much discomfort that makes my sleep deprived. Please advise. Thank you.

Now past 2.5 weeks past surgery. Yes things don't seem to improve. But they are. Knee goes thru what i refer to a metamorphosis and pains are in different places. Sleeping and good meds seems like only thing working.

So sorry for you. I am 4 months from TKR and my knee hurts every day and especially at nigh. Might get 4 hours occasionally but knee still hot and achy and sore. Terrible painful recovery. Good Luck

I’m exactly 2 weeks post surgery and I have a long body pillow which I fold in half so, it’s like having 2 fluffy pillows. If my pain medicine works, I can usually get about 3.5-4 hours of decent sleep! But that rarely happens. Thanks for the tips tho!

Total Therapy Solution - Physical Therapy So, next Wed is my 4th week! I’ve been experiencing a lot of spasms in my surgical knee. They wake me up and I’m yelling and crying! I’ve tried muscle relaxers, ice packs and I’m about to try heat! Of course, no one else has experienced these spasms (none that’s had the knee surgery anyway)! I’m at my wits end and need some kind of relief! Have you had anyone experience this or what advice can you give me? Thank you in advance

Total Therapy Solution - Physical Therapy I’ll try more bananas and water! I was told they were spasms. Are those the same as a charley horse? What it does is, hits in my calf and makes my whole leg contracts. It makes it hurt more because, it contracts on its on even though I couldn’t lift my leg that far if I was trying to do it by myself!

Thank you for addressing sleep after tkr. I am 5 weeks post op and it's my hip/glutes and lower back that keeping me awake. I NEED HELP! my pt can release these muscles in therapy but by the end of the day there is no position I am am comfortable in. Is this normal? I am told it is due to the extreme misalignment i was in prior to surgery and my body is adjusting. Im going okn a week with very little sleep.

I was told to avoid resting the back of my knee on a pillow. In other words nothing under the knee.

Had my left knee replaced this past November 30th. All was well till a week & a half later- I got a blood clot and a staph infection- knee had to be opened and cleaned out & had some parts replaced. Still cannot get comfortable no matter how I lay and it’s really messing up the sleep patterns that I don’t have, not to mentions all the antibiotics I’m on, they make me feel like imma blow lunch and shit every pair of pants I own at the same time..

Pick the position that feels best at the time. There is no bad position. As long as your range is improving you can sleep in what ever position is most comfortable. If you are dealing with limited knee extension you may want to sleep with the knee(s) in extension but that could be on your stomach or back.

Hi there, I need advice pls 🙏.. I have a surgery with plates & screws due to tibia plateau fractures.. I also suffered 2 compression fractures on my spine - L1 & coccyx... I need to move around, up & down when doing basic washing & toileting etc.. I do not hv a flat area leading to toilet orkitchen. All theses r so strain to me resulting in a lot of back pain... How could I sleep ? I hv a leg Brace on to secure the knee- can bend much yet.. Lie down flat to sleep is very painful.. Wat position can I sleep with。?

@@totaltherapysolutions not yet.. When I was in hospital, nurses move me ard with a commode chair when doing toileting. I don't have so much of the back pain but only the butt pain frm tailbone. They hv give me a set of exercise to do after I discharged. Now waiting for outpatient physio vacancy.. When I got home, there r so many transferring, standing up & down, many movement just to doa simple thing... At the end of the day & next morning my back hurt so much that I can't move or get up frm bed.. Could u pls advise how shld I sleep with this prob. Thank you 🙏

This is a great question. I have never had a surgeon recommend this until recently. I would say it depends on what the splint is intended to accomplish and if you are accomplishing that goal. We have had a couple patients locally who for the first time have been told to wear these extension braces. I think it is a great idea, but once they have full extension there is no need to continue wearing the brace. But they were never told when to stop wearing the brace. What are you using?

My ortho doc and pt guy said sleeping on your back with a pillow directly order your knee is not recommended.

Thanks for those ideas! What do you recommend to people having a hard time with full knee extension? Should they keep sleeping with a bent knee or try to endure the pain and keep the knee straighter?

If extension doesn’t improve in the 8-12 hours of daily activity then depriving yourself of sleep isn’t going to help. We know poor sleep increases pain sensitivity. Also causes elevated stress response and blood sugar levels. Sleep is more important than nearly anything else. I think that recommendation may be appropriate for someone who is bed bound, but that person isn’t likely getting a knee replacement. What do you think?

My surgeon, PA and entire physio-therapy staff tell me to sleep with operated knee's foot on thick pillow and thin pillow under calf, facing up. That advice, they say is very important to get the knee to zero degrees (and mqybe a smidge more}. So far it seems to work, but I wake up a lot, so i take an hour or two nap in the afternoon. Im going to start the rolling pin massage from one of these videos right away.. Boy, I'm glad I found this video series.

My knee is actually good but it's all the swelling and redness that stops movement more than 95 ° it goes straight and I have never needed crutches. Its my hamstring that is hurting a great deal and am told its because my quadracep is weak

@@rogergrote2546 yep I’ve had lots of physio mainly massaging tight quads and hamstrings, thankfully massively helped. Think all the muscles freak out and tighten causing all sorts of symptoms

farrukh waris great question. In my experience there is no good answer. You try different positions to see what works. As you heal don’t think because 1 position felt good last night it will be good tonight. You are welcome to join our Facebook group. I have a wedge pillow that I use. I am currently at 7 weeks post surgery and honestly wonder if I am ever going to get right again.

Debbie Williams it can definitely be frustrating. But I usually tell my patients it takes 12 weeks just to recover from the surgery, then 13 to 24 weeks to get back to normal. Keep up the good fight. You will win in the end.

Debbie Williams have you tried using a rolling pin for self massage? That has helped a lot of patients. Separately, tonic water has small amounts of quinine which has been suggested to reduce cramping.

I am now 4 days past knee replacement surgery and I am not sleeping, my leg is extremely swollen and my ice machine does not seem like it it doing anything. I am getting frustrated and tired (from not sleeping). When I played football in college we would just pack whatever was hurt in ice. I am about ready to have several hundred pounds of ice delivered and start doing my "old school" ice treatments with about 10+ ice packs all around my knee and thigh to see if that will help. Very frustrated right now...

I just completed two staged surgery to fix the infection of primary knee replacement. Extremely painful and can't find the right position to sleep. Today is the sixth day. Wondering how long it will to get back to somewhat normalcy of life?

Ellen Rosser I am not sure I understand. But I will try to answer. I saw a new patient today following a TKR. She went shopping yesterday and was worried she over did it. I usually say you may be sore and the knee may be more swollen but it is safe to do what you need and want to do. I have had patients out shopping this time of year for several hours. The knee will hurt but it won’t be damaged. As long as you recover in a day or two keep doing what you are doing.

I have to sleep on my back. I am 7weeks post TKR. Still uncomfortable but lil better. I can lay on my side with pillow for bout 30 minutes.

I put a hot water bottle wrapped in a pillow case on top of a pillow under my bad knee , this gives me a few hours of sleep , 4 weeks after op for full knee replacement

Nothing scared me more in my life than when they said I needed an artificial knee. The more I read about the percentage of recovery the worse I felt about it. I am so happy with my stem cells going on seven years now. I know they don't grow cartilage but whatever they do, I am pain free although I don't garden anymore. Who cares at 70 years old? My first thought when I found out about knee replacement was that it sounded like something from Nazi Germany and I wondered how could we be in the Dark Ages still? If I ever needed more done I would save the money for the grafting. But my holistic doctor was right on when he told me ahead of time he could almost guarantee me I was going to be just fine and he was right. Just something to think about before you let them cut your kneel apart.💖 P.S. I was 80% better in 10 hours!

I agree with your rendition of knee replacement surgery, it sounds barbaric and it is, however after all is said and done it is so worth it. Even though we can't kneel down I plan on rigging something up so I can garden, it's my nirvana.

@@nancyjones6428 you should be able to kneel down. Many do. It just may take a year or two before it's most comfortable. Many use knee pads too. That will be my plan. I have both mine done July 14th. 2022

Phil Hewitt if that is comfortable for you and doesn’t cause your knee extension to suffer then use that position. Some clients can’t sleep on the backs because of sinus or other breathing issues.

Ellen Rosser I would say the faster you can get out of the house and doing what you enjoy the better. I have never had a surgeon recommend any limits on activity following knee replacement. I was told by my surgeon that I would be down for 2-21/2 months this is including physical therapy. I am one month down. Started moving around about 4weeks post TKR not too much just visiting.

I think it's based on individuals and know your limits, my wife was anxious to get back to normality asap, but everyone heals differently and she just ventured outside at 8 weeks and almost passed out from the pain..

I love these videos from people that never had a knee replacement… you shouldn’t sleep on your side nor will you bare to do it. The only way is to sleep on your back and have to elevate leg above heart. Pillow below your ankle or calf. Never under your knee. #1 priority is to always have your leg straighten. #2 is to bend it.

cooldog60 I appreciate your perspective. I would never go against any advice you have been given. But it important to understand everyone is different. Not everyone has difficulty gaining extension. If you are able to gain extension and work on it during the day it may be safe to sleep in a comfortable position at night. Always consult your medical team.

@@totaltherapysolutions from what i can gather most people have flexion issues not extension. For me sleeping on my back has been a big milestone. Maybe Tony this video needs redoing explaining the reason why back is best but that no sleep is worst. And also if you have zero extension then a pillow under the knee at night is going to be fine?

My surgeon said to sleep with immobilizer for 2 weeks. But so painful, I can’t stay asleep. Then I let my heel fall off the edge of the bed and now my hip hurts. How straight does my knee need to be? When is enough enough?
