The whole sequence where his head is retrieved after execution, and everything in the game afterwards, is also just a dream.

I played it on terror billy all the way until he gets 'upgraded' than I switched it to the easiest difficulty. This is the best way to go as it makes sense for BJ. He felt sick and was slowly dying but after he becomes a 'badass' he gets back into his rhythm and destroys everyone with ease living up to the terror billy name. The experience was more enjoyable as well and I finished the story in a more timely manner.
