Our healthcare system is such that everyone at some point in time has undergone a stressful time, not only because of the illness but the extreme financial stress that comes with treatment procedures. High hospital costs, expensive medicines and bad insurance schemes have caused people a lot of trouble, many times to an extent that people have lost all their savings or homes and gone under debt. Let's discuss how this whole system is so broken and inefficient, what all wrongdoings are happenings, the things which could improve on or at least be aware of.

Sir my brother is 29 years old MD Paedeatrician(ayush). He just opened his hospital with 12 lac(loan) investment. 7 beds+ furniture + and medical equipments. My parents always tell my brother that admit patient if it is necessary . Because we are also middle class like you mentioned (There is only one difference between poor and middle class people that is medical bill 🔥). Rent is 20000 + Nurse 5000+ cleaning 5000. Yestarday 1 premature baby came with reference from munna bhai (village doctor) he suggested to admit the patient in NICU. My brother checked baby and told his parents there is no need to admit the baby in NICU. Then he prescribed nestle pre nan (600 rs) and 100 rs medicine for mother . After sometime owner (hospital space ) came who is also a general practioner so my brother told him what happened then owner said if you want to become successful doctor then you have to listen to munna bhai(village doctor) because next time they will not refer the patient to you . Doctor also said if you want to pay the loan+rent +maintenance you have to do some unethical practises . Yestarday my brother didn't even sleep because of this incident

My mother 80 years suddenly got heart attack and stroke, it was not possible for us to wait for the ambulance to come, this happened in January this year. A neighbour Dr. Clinic mentioned MBBS consultant etc. We brought him home. We told him patient is serious but he came with just oxymeter. No stethoscope, no BP instrument. No emergency medicine kit. My mother was dying and almost unconsious, we given him our home stethoscope and bp instrument to see the BP. He tied the cough so tight that no one can bear, made us scared. And he said BP 180, and oxygen is 59, you arrange ECG and he started to go. We lost my other within few minutes. I live in a metro city. If we have this kind of health facilities in metro cities how we see ourself vishvaguru or superpower ?

Elective surgery is not that surgery jiske bina kam chal sakata hai, actually it is a surgery which can be preplanned well and done to treat the disease which will definitely have adverse health effect if surgery is not done... For example... Removing any cancer is elective surgery , while surgery in case of trauma is emergency surgery.... The difference is wheather adverse effect is happening on same day or within next few days... Plz don't miss guide people.... Governments every ministry is broken n not working correctly.....focusing on doctors only is not the solution....

@@DeepakTiwari-ls8bdpeople will always blame doctors whatsoever. This is the only professional where I have seen people working so selflessly, and are always overburdened, managed by the best of best. If you want Improvement in services, 1. Target the hospital not the doctors. Almost all corporate hospitals are managed by businessmen and MBAs, and targets are given to doctors. 2. In government hospitals anyone can hit doctors, doctor can try but how can he overrule death, people will die today or tomorrow. 3. Stop private medical colleges, people pay hefty amount to get mbbs and pg seats (in crores, not lakhs), without any merit (or neglegible even if merit standards are provided), these are the ones who then want to earn back their money, and have no skill whatsoever. 4. Reservation, reservation, reservation. Ever seen US giving reservation to blacks. Mind you, blacks were slaves, which is worse than the torture sc/st have undertaken, though is it good to help a caste, but you cannot be blind while following rules, right?? Why did 14 of my 15 SC MBBS batchmates have more money than me, having taken coaching and living out of city in hostels for 2-3 years, whereas I couldn't afford any without my parents burdened in loans and me having only one chance, cause I do not have more money to spend. ( I am from general category, though govt. Has in 2019 passed EWS law, which I can also now avail) So you see, out of 15, only 2-3 are genuine sc/st quota deserving, rest are already upper middle class. Congress has destroyed the country, some things are very very difficult to change, and only time and leaders like Modi ji can help us.

Another point: I wrote Paracetamol 500 (composition name i.e. generic name) in my prescription. So the patient goes to a medicine shop, the shopkeepers has no fu**ing qualification. He will give the most expensive generic medicines whose actual price is 5rs per packet but MRP is written Rs. 500. A shopkeeper will always try to sell the most benefitable one. So what should I as a doctor do? 1. Write composition of the drug? 2. Write branded medicines? 3. Write branded cheap medicines? The best thing is to write branded cheap medicines. That's what we mostly do.

Since a lot of medicos will come to this video - For anyone deciding to pursue medicine in India - DON'T. If you can't imagine yourself doing ANYTHING ELSE - jump in. It you're from non ews General category, good luck cracking it because with 68% reservation, you'll need to be extraordinary to do it. Both UG & PG. If you're already in MBBS, work your ass off, give USMLE/PLAB and leave. Run fast & as far as possible because no other nation is as thankless as this one. The value of doctors is absolute nill in this country. You are expected to go to college for THIRTEEN YEARS and then do social service. You will not be sleeping, eating on most days, will work 36-48 hours at stretch and will be beaten /blamed for almost everything. For those who are already doctors - let's mourn our lives. 🥂 Nitish has made bare minimum valid points in this video. There are 1% doctors who do this, yes but the rest are blamed for it because they charge money for their time, opinion & expertise - which is looked down upon in India. People want free treatments. The generalization is harsh. It feels terrible when you've worked for years to just be blamed for what the pharma industry and MBA executives do at the end but that's how non medicos see us. Instead of holding the government accountable for not providing affordable Healthcare, you want doctors to do social service at their personal cost. I go to work prepared to be beaten up and no one wants to talk about it but you're quick to call us all corrupt to promote the insurance company.

I know there are problems in our healthcare system! BUT TRUST ME! I am living in Canada and after coming here I observed and experience possible the worst ever healthcare system and I started respecting India for that matter! here for taking an appointment is a huge thing once you get the appointment there's a long waiting list for even X rays and Sonography! there are 6 months of waiting for single X rays only! Its free and that's why the waiting list never ends! In India you have both options like private and public hospital and people definitely can get the treatment ASAP on emergency basis! when my province crossed 200 hospitalization only they declared emergency during Covid! I am not saying Indian health care is perfect but it's way better than what we have here! I have seen Indians booking roundtrip to India only for their dental issues as even with the insurance that will cost you close to 1,00,000 INR in Canada and other countries! This is not a doctor's fault. Most doctors are very underpaid and even with the commission money, they don't earn a lot. But the main thing is not that. It's that they have a pressure on them. It's the entire Health care system that's corrupt, and doctors have to put into that to survive. You don't expect those people who spend their time and hardwork and energy to save your life with everything they got, those same people never want to hurt you.

Cor-na death calculation India: 135 Crore Population. Number of deaths recorded till date on record 5 lakh - Consider it 15 lakhs. 15 lakh divided by 730 days = 2054 deaths per day in whole country. 2054 deaths divided by 36 (28 states and 8 UT) = 57 deaths per state per day. 57 deaths per state per day, isn't it a normal death rate???? This is how dangerous Cor-na is. Shutting down the private medical services or non-acceptance by doctors without reports, if report is positive only authorized medical centers are supposed to admit the patient (Huge shortage of beds, doctors, medicines, oxygen, suffocating arrangements, because only finger count centers). Oh boy! This is going to be a long post, but if anyone is interested in knowing the full/balanced story, please continue reading. The video states some relevant and true facts, but a lot of the facts are also irrelevant/false/misguided. I do agree that few doctors malpractice, but in my career, the percentage of doctors working purely out of a money mindset have been very less. If the pricing of the drug is the concern, the pharmaceutical companies should be looked into, and not the doctors per se. That being said, let us look at some of the points discussed in the video 1. The efficacy and action of Innovator and generic medicines may be same on paper, but in reality, the efficacy varies, and sometimes by quite an amount. 2. On paper life imprisonment and 2years jail term may be valid, but considering the legal loopholes in our county's system, we all know they can be bypassed. (Maggi is a good example to demonstrate the same- the controversy with lead being found in Maggi is well known. But my question is, since the initial batches of Maggi were FSSAI approved, how did FSSAI allow a product with high lead content in the first place?) 3. If pharmaceutical companies stop their research and development for finding a new drug, we won't be getting many of the drugs that are now an important part of management of any disease, because the Govt. funding for R&D is very minimal. That being said, if prices are to be regulated, the Govt. should bring in guidelines and policies for the same 4. The point my brother Nitish said about whenever the patent expires, companies make few changes in the drug and introduce a new drug is not completely true. The newer versions of the original drug are often more effective, have fewer side effects, and are better tolerated. And that too requires R&D. Some of the newer versions of the original drugs have even replaced the original drug because of its efficacy and better side effects profile. 5. The efficacy of a drug does not only depend on the drug molecule. How the drug is packaged, how well it dissolves, how rapidly it dissolves, how smaller particle it breaks down into(Pharmacodynamics) affect the availability of the drug in the body. Also, the effectiveness of a drug also depends on the patient's profile, his/her body structure, enzyme composition, fluid levels, etc (Pharmacokinetics). So, the same paracetamol from different brands may have different efficacy. And also, the paracetamol from the same brand may work differently in different patients 6. My brother Nitish also mentioned about doctors not prescribing generic drugs for hospital pharmacies. I guess our brother has never visited a govt hosp pharmacy, where half of the drugs are not in stock most of the time. Most doctors in govt hospitals prescribe generic drugs, but most often they are not available in the hosp pharmacy 7. The points that Nitish mentioned that non essential drugs are basically essential drugs with few modifications to sell them is completely false. Essential drugs and non essential drugs are two separate categories of drugs 8. I do agree that some labs do sink testing and their reports are not reliable. Same goes with medicine brands. The same medicine(eg. Paracetamol) from different brands are not reliable. So, when a doctor prescribes a certain brand, or refers the patient to a certain lab, it's not always because we want to make money, it's also because we want to give the patient the best brand that is available or receive a reliable report so that the patient can be managed well. 9. Also, the fact about elective surgery is completely false. Elective surgery does not mean patient does not need surgery. It just means we plan the surgery at a fixed date so that all the pre anaesthetic check ups, routine blood investigations, etc can be done beforehand and the risks can be minimised. The other kind of surgery is emergency surgery, which is done to save the life of the patient in emergency situations even though we do not have any pre-surgery check up. 11. Regarding discharge certificates, whenever a patient gets released from the hospital, he is bound to get discharge certificate. Lack of education is the key factor for a lot of malpractice that goes on. But whose responsibility is it to educate the masses? 11. I agree with Nitish that is rural areas, there is a vast deficiency of doctors and most of the doctors who practice there are not qualified or are just quacks. But this is something that should be regulated by the govt. Rural areas often do not have hospitals, pharmacies in 20-50km radius. Even if there is a hospital, the infrastructure and availability of medicine is limited. Doctors, nurses try their best in the limited scope of facilities to manage the patient's, but if the patient dies or deteriorates, the doctors and nurses get beaten up. Why should doctors practice in such rural setups just to get beaten up, even after trying with whatever they have, when the govt is not concerned about the healthcare of the people 12. Health insurance are very good options for bringing in the healthcare architecture. I do agree with that. But, let us just think for a minute. In India, if you purchase a vehicle, you have to get it insured, but there is no mandatory health insurance. Why can't the govt bring in more policies and funding for mandatory health insurance for complicated cases which cannot be dealt with in the govt setups. If a poor person purchases a bike, he has to avail insurance even if he earns less, but can he not avail health insurance if proper policies are in place? 13. India spends about 1.5% of GDP (or even less in certain states) on healthcare. It more or less been the same in the past 30years. In a country where around 60% of the population still struggle to make ends meet, is 1.5% of GDP enough? Before blaming the doctors and the healthcare staffs, please go to any govt hospitals near your home and stay with them for 24hrs. I hope the idea that we just sit and earn will go out of the window. There were days where I didn't even get to drink water for 14hrs because of the huge workload and understaffed conditions at all time. 36-48hrs continuous work shifts, lack of a proper washroom/bathing room, lack of proper resting room, huge patient flow, administrative difficulties, lack of equipments and medications(sometimes even the basic things such as cotton, gauge; that too in a tertiary setup govt hosp). And all of these after 4.5years(MBBS) or 7.5 years(MD/MS) of rigorous studying. Despite everything, everyone of us try our best to give back to the patients. 14. It's not the doctors at fault(some doctors are unethical, I agree, but most are not). We don't fix the price for a medicine, we don't fix the price for a surgery(a surgeon recieves around 10-15% of the total surgery cost), we don't fix the bed charges, we don't fix the rates of investigations, we don't supply generic medicines, or equipments to the hospitals. It's always the people at the top, the management guys, CEOs, or the political leaders who do that. But it's always the doctors who get the beating. Everything in this county is doctors fault. If medicines are not available, doctors fault. Oxygen not available, doctor's fault. Cotton, gauge not available, doctors fault. Equipments not available, doctor's fault. Bed not available, doctor's fault. Investigations getting delayed, doctors fault. Getting a date for surgery after many months, doctors fault. If the patient relatives keep the patient at home, take medicines on their own from local Pharmacy, and then bring the patient to the hosp when he/she has deteriorated, and if the patient then dies, it's again...you guessed it right... doctor's fault. So, before blaming anyone, please take note of the whole scenario and the conditions. In a country where the media always paints the doctors as villains, half true information such as in your video only adds fuel to the already distorted mindset of the people about doctors. Every profession has good people and bad people in it, ethical and unethical people, hard working and lazy people, but why is it that only doctors get pointed out everytime. No one blames a lawyer when they lose a case, or an engineer when a bridge collapses, or somebody dies because of a bad road. Bro, I understand money is important for everyone, and that is why brand promotions for KZfaq videos are also important. But if you present half-baked information along with certain false information on your channel just to promote the brand, then what is the difference between you and the other companies which are misleading people. So, please do your research before presenting any information. Sorry for the long post, but with so many things at fault, I couldn't make it shorter. Hope that clears up some of the misconceptions/misguidance/doubts that have been said in the video as well as in general.

Slight correction here.... ..FDA approves Generic medicines if they are atleast 65-70% effective or 65-70% bioavailability of Branded drugs.... Means there are many generic drugs in the market which are only 70% effective compared to Branded drugs.... I This gap of effectivity may what lead to Longer course of treatment and higher failure rates... ... Regarding the tests..a particular symptom (for example fever) may pertain to more than 30 diseases... Therefore besides Widal test for typhoid we've also to write NS1 or Blood culture to rule out associated malaria or septicimea ... ... Hope you understand it's not always the doctor's fault for writing so many tests... It's just that the medicine in itself is a very complex subject and we've to rule out any possible existing conditions or in the end we would be blamed in case the patient doesn't recovers due to incorrect diagnosis as a result of inadequate testing.. ... A good example of this can be seen in COVID where besides Rapid test...RT- PCR and Chest X ray were also recommended to differentiate it from a pneumonia case and calculate the intensity of spread of infection...

Do not generalise and tarnish the image of doctors! I request you to please visit any government hospital and notice how hard and under what facilities and circumstances the doctors esp junior doctors work, sometimes they work for 72 hrs non stop and also look at what drugs are being prescribed. Major life saving surgeries are being done with perfection at affordable cost. There are very talented doctors out there and they are working selflessly. If the infrastructure , facilities and jobs in govt funded hospitals, policies and GDP share allocated to medical system is not enough then doctors are not to be blamed. Some sentences used in this video are highly objectionable and these provoke goons to assault doctors in the country. I do agree at certain points but those pertain to pharma companies not all the doctors.Pls be mindful of how you are portraying any profession because how so ever intelligent and aware you are aap jab bimaar hote hain toh doctors ke pass hi jaenge aur wo accha kaam hi karte hain!

Like if a person having renal cancer then he may require surgery in next 1-2 year or so. Trauma surgeries, brain haemmorage are emergency surgeries. All elective surgeries & necessary & required.

I am general surgeon....I am in favour of generic medicines...but I also preper standard pharma company for patients... People don't know that people consuming drug are also second quality drug even if it is approved by FDA.. I give a example .. Like mangoes ,if you go in market there export quality and normal quality of mangoes.. both are mangoes but there is different in product size ,shape, taste also prize.. Same happens in drugs..FDA approved drugs export in USA are of superior quality because people are aware that they don't want any chemical to inject in their body before assuring itts safety..but people in India directly take medicine from medical store without prescription..there lots of side effects of medicine to patient that only certified doctor can tell.. but it's mentally that can't be changed..

Let me ask you how many people go to KFC and Starbucks? Does a rickshaw wala, a farmer, not even 1% people in India; even those who go, well know that coffee and chicken could be made at home but since they can afford they choose to and they don't break FD they don't empty there bank 🏦 deposits, don't mortgage there house 🏠to do so. Briefly saying they haves options but a patient visiting hospital doesn't. Being a doctor is a service not a way to make as much profit as one can, similarly as being a politician but both have misunderstood as a money making career. On the other hands doctors writing generic medicines giving right suggestions are there and loved and respected by society...

I work in a hospital where the patients are so devoted to doctors that they don’t accept other medicines which are same in composition and 50 percent less the price of the prescribed medicine. They trust the doctors with all their heart but unscrupulous doctors are blatantly lying at their patient face to face . This immoral treatment should be stopped .

This is why education is so important, once there is a significant insurance coverage in a country; then insurance companies and corporate hospitals collude to fix arbitrary rates for any minor procedure. This is why Americans come to india for cheap healthcare, because the rates in their country for any procedure or consultation have grown around 20 times since the Eisenhower presidency pushed for universal insurance coverage. Can't believe i bought this shit in the first half.

Treatment given is, general lungs infection treatment, regular antibiotics, paracetamol, vitamin supplements. "CREATE PANIC DECLARE PANDEMIC "

As a doctor I can confirm that I don't take any money or foriegn trips from pharma company or pharmacy. But I can tell that options for generic drugs has both plus and minus. Plus is low cost. Minus are no medicines and researches will be done on new disease or organisms and we will be forever dependent on foreign companies who hold the patent rights for these drugs. And secondly these drugs may not contain equal amount of drugs as described on the label. And in government hospital we can get generic drugs but these companies and contracts are given to politicians relatives only.just like any other government contracts

Well said,it's absolutely true about the pharmacy part, but please don't generalize this and some how twist the message and make the doctors the villains here.(as per your title which you have put for click bait🤦) The true culprits are the pharmaceutical companies and for that matter any multinational companies and businesses who mainly focus on their profits. Eg. Bun or stuffing max cost 30 rs but you pay has high as 200+Rs for McDonald's, subway etc A spectacles cost max 500-1000Rs as per raw material but you guys pay 2500rs+ for lenskart🤷 And the list goes on.. This has become a part of this society.. I tolly support generic medicine myself,(medical officer at government tertiary centre) And promote them amidst by fellow practioners too. But people society need to change, (You prescribe generic diclofenac, the patient wants and trusts volini only 🤷🤦)
