Just checking how the recovery has gone since this video as I just had left calcaneus fracture but they had to do a osteotomy on my fib to remove a piece of cartilage and bone, I also fractured both Talus dome on both feet I'm 10 weeks and just walking bare feet now, let me know how you went, do you still have pain, would be good to hear. Kind regards

Hi. The patient on this video had a very good outcome - but it is always a combination of injury pattern, treatment, physiotherapy, cooperation of the patient and last but not least also some luck to get a satisfactory result. If you are able to walk after this "short" time after theses fractures I think it is a very good outcome - hope it continues to progress as satisfactorily - best regards

I had a similar injury. Both calcaneus heels surgery. I am approaching 12 weeks now next week. Since one is supposed to go to Partial bearing from non bearing weight then full weight bearing. How will it be possible for me to be partially weight bearing when I start practicing walking because it’s both feet that had surgery? Are there devices for someone in my circumstances?
@rodemund
@rodemund Ай бұрын
In fact, partially weightbearing both fractures on both sides is not really possible. But a bony healing should have taken place and full load is possible. In any case - even if things go well - the first few months will be painful and swelling is to be expected. Hope you will get a good outcome - regards

Comminuted minimally displaced fracture of the calcaneum extending from the tuberosity posteriorly to the subtalar articulation. Intra-articular minimally displaced fracture through the base of the little toe metatarsal. In accordance with the 5th metatarsal fracture pathway this fracture lies within the green zone. Been 3 weeks now - i only had 5th metatasrl break in croatia on holiday and now the above. Stil no surgery not sure when fracture clinic to see me but the pain is bad and now on very strong meds - but wondering how long is recovery and of course cannot even stand up and balance yet plus the A&E doc says it is complilated x ray you have but not sure how many breaks i have? Multiple i preusme? do you think srgery in this case of the above?NThansk

I think i broke my heel. I tried to run then suddenly my foot fell into a little hole. I tried standing up but I just can't. My heel is swelling and aching rn and I can literally feel the pain. I tried to put ice on it but it still aches and whenever it try to stand i can't because I can't build any strength on my foot. We can't go out because of the pandemic so my mom and I tried home remedies such as ice and a little bit of stretch.

Hi. If you now have no possibillty to go to an hostpital for a x-ray it should be best to keep your leg up, cool it several times a day, try to get crutches not to do weight bearing and put on some bandage. In most of the cases with this kind of accident you should a sprain with the lateral ligaments torn. Most of the pain is round the lateral ankle. For a calcaneal fractur pain when compressing the heel on both sides is typical. But try to show it a doctor as soon as possible. good look Christian

Hi sir, i broke my left calcaneus on 14/mar did multiple x rays to track progress and even ct scan initially. It says displaced intraarticular fracture with type 4. Two doctors suggested to go with conservative approach and i am with permanent cast right now and no weight. I am very worried if i should do surgery or go ahead with conservative thing. I just want to walk normally again.
@rodemund
@rodemund 2 ай бұрын
Sorry for your injury. I can't say any concrete advices without knowing the fracture situation but as you wrote it is a worse intraarticular calcaneal fracture. The accident is now 3 weeks ago, for open reduction still possible to operate but it becomes more and more difficult to get a good reduction result. So it probably will be the best to go on conservatively and wait (and hope) for a bony healing. A destruction of the subtalar joint and dislocation of fragments is to be expected. If you have luck, there will be a stable situation with a more or less stiffness subtalar and decreasing pain while walking. If there is ongoing pain the solution could be a secondary surgery to stabilize the subtalar joint (arthrodesis). But without knowing the exact situation it is of course not possible to make a serious judgement. Wish you all the best

Hello doctor. I have a tongue type fracture without involvement of the subtalar joint. I had minimal invasive surgery , also I am 32 years old. I have the cast for 5 weeks and I will have it for 2 more weeks. After removing the cast when will I be able to walk without crutches? Thank you very much.
@rodemund
@rodemund 9 ай бұрын
Hi. Severity of fractures and surgical success are very different in fracture cases and consequently clinical course and outcome depends on many individual factors. But normally it will last about 4 - 6 weeks (after cast removement) with increasing load before one can walk longer distances without crutches and some months with inconvenience after more load. But nevertheless, training is the best way to get a good outcome - even when you have some swelling and pain in the evening. By the side, after 5 weeks and good osteosynthesis perhaps you will need no more cast, just no weightbearing - ask your doctor. regards Christian

@rodemund
@rodemund 2 жыл бұрын
Hi Sorry for you. There are two different things. The problem with the nerves and the problems with the Joints. First there would be a difference which nerve it affects. There is one for the skin on the lateral dorsal region of the foot - not that critical. But there is one for the sole of the foot and one at the ankle - not very pleasant. Additionally, there can be impingement between the calcaneus and the ankle bone. Nerve problems are very difficult to treat - first of all, an precise diagnosis would have to be made through a neurologist. The second point is usually the joint under the ankle bone. If it is not completely anatomically healed but still mobile, it will always cause pain. The first step would be a CT scan and then, if indicated, a fusion between the two bones. This must to heal the pain completely but can bring a significant improvement. You could post me some X-rays best regards

@rodemund
@rodemund 11 ай бұрын
Hi. In general, there will be pain and swelling for at least 3 to 6 months after removing the cast, even for the patients with good outcome, so it must not be a bad sign. Tongue-type fractures normally had the better outcome. But without more information about the result of the surgery I cannot say much. Training and load are recommended in any case. If severe problems persist, CT check in 6 months.

Hello doctor, I had calcaneal fracture after 35 days my cast was removed and told not to keep heel down for 10 more days. Now I have stated to walk little bit with the help of Walker. But I feel the pain. from when can I put my complete weight on the leg? Walking will not make my heel worse?
@rodemund
@rodemund 2 жыл бұрын
It is always better to do weight-bearing and physiotherapy with increasing load after bony healing of the fracture. It is normal that you will have pain the next months - up to 6-9 months. You should go on training. In this time and we hope the cartilage will also become stable and will fill some incongruencies in the subtalar joint. People who train too little almost always have a bad result. But I cannot say anything for your individual situation. If their is to much damage in the joint or the axes or height are not restored there could be persiting pain. Then the only solution is an arthrodesis.

@rodemund
@rodemund 2 жыл бұрын
It is difficult to give an answer because I do not know the fracture and the result of the operation. Basically, however, it can be said that even with very good surgical success, the time until freedom from symptoms can be very long - many months, sometimes even 2-3 years. It is therefore too early to make a good prognosis. Important is a regular controlled training and physiotherapy whereby the foot can (and should) hurt and swell again and again, especially in the evening. I would definitely work towards playing footbal again. Possibly a second operation with fusion of calcaneus and ankle bone can help - this is not to be aimed because it, of course , decreases the mobility of the foot, but it can show good results - but you should really give yourself a lot of time until this operation (more than 12 to 18 months). good luck and I hope for a good result

I have a tongue type heel bone fracture in my left heel it was operated one month ago now my cast has been removed and asked to bear weight partially but it's painful is that normal?
@rodemund
@rodemund 2 жыл бұрын
Yes - it is normal. After 4 to 6 weeks you can expect a bony healing and a stable calcaneus. But the repair of the cartilage and the leveling of existing small steps and gaps in the lower ankle joint will take time. Additional you will have scarfs in the subcutaneous tissue and muscles and there is always a reaction in the neighbouring bones. All in all , with individual differences, you will have more or less pain and swelling in the foot for the next 6 months. But it is important to go on training and weightbearing. If the foot is painful and swollen in the evening, but with elevation and cooling problem-free again the next morning, then this is a good concept. If it still hurts a lot in the morning, then reduce the intensity of the exercises a little. Patience is important, it takes its time. good luck

In general, it must be said that calcaneal fractures can often lead to permanent problems, especially if the lower ankle joint is affected, as usually is the case, Even small inconsistencies in the joint cause persistent discomfort. You should get a CT scan. If there are severe changes in the lower ankle joint, the only option is usually an arthrodesis to relieve the pain. @rodemund
@rodemund 2 жыл бұрын
of course it always depends on the individual fracture situation. First of all, the lower ankle joint should be assessed by a CT scan. In the case of severe post-traumatic arthrosis and persistent pain on uneven terrain, arthrodesis is certainly the best way to achieve improvement. However, take your time. The symptoms often improve after 1 to 2 years and the success of the fusion operation does not depend on the time of the operation.

Hello, sorry that it is not better yet. Normally it is a problem in the lower ankle joint (subtalar joint) due to incongruence and destruction of the joint surfaces. You should have a CT scan to evaluate this. The solution would be an Arthrodesis of this joints. It is not perfect because you loose the movement there, but most of the time, if the anatomical reconstruction is not perfect, you will have a significant limitation in this joint. If the arthrodesis succeeds and the joint is stable, it should be much better in terms of pain and load-bearing capacity.

Fracture types, treatment options and techniques as well as the clinical course of these fractures are very different from case to case and there are simply a lot of factors that influence the outcome. Take your time. In general, persistent pain is to be expected after 12 weeks. I usually perform a CT scan after this time to see if the bone has healed, full weight bearing is possible, control axes, height, osteosynthesis….. and examine the joint situation for a prognosis and further treatment guidelines. The injured cartilage in the lower ankle joint needs much more time to heal and to compensate some remaining irregularities of the joint surface - often more than a year. Frequently there is also bone substance loss in the surrounding bones (need heal again) and also scars in the foot muscles, ligaments and joint capsules. Intensive physiotherapy is advisable - even if there is some complaint - controlled exercises are usually better than too much comfort. If there is no improvement (after 1-2 years), arthrodesis can help. Wish you all the best
