Although we commonly think of carpal tunnel syndrome - or any other condition that involves pain, tingling or numbness in your hands or fingers - it is essential to consider that the problem can actually be originating from upstream.
Consider the bottom of a river that is not flowing well with the region in a drought. Yes, there is the water problem downstream ... but what if the reason for the drought it that there's been a new dam constructed or some other type of obstruction that can actually be found upstream?
It has been my experience that when dealing with carpal tunnel syndrome or other problems involving pain, numbness or tingling in your hands that compression at your wrist accounts for about 10% of the problem ... but that the majority of the problem can actually be found upstream:
15% as the nerve travels through your elbow and shoulder
25% as the nerve exits your lower neck (aka cervical spine)
50% as the spinal cord itself is stretched.
Especially if you experience hand problems on BOTH SIDES, unless you are dealing with something like rheumatoid arthritis, odds are that you are actually dealing with a COMMON ORIGIN that is not affecting just one nerve, but is actually affecting your spine when BOTH OF THOSE NERVES ARE STILL FOUND IN THE MIDLINE.
It is a phenomenon that is known as "Double Crush Syndrome," which in my experience occurs far more commonly than carpal tunnel syndrome alone.
In other words, any time you experience numbness, pain or tingling in your hands, don't just focus on where they symptoms are: LOOK TO YOUR NECK FOR THE CAUSE.
If you suspect you are dealing with true carpal tunnel syndrome, the gold standard test is called a Nerve Conduction Velocity (NCV) tests, which is where needles and put directly into your nerves through your skin in order to measure their electrical activity. With true carpal tunnel syndrome, the NCV tests are normal until you get all the way to your hand, and then your scores suddenly drop. However, because of the location of the nerves as they travel through your neck, shoulder and arm, it is nearly impossible to get an accurate reading.
The bypass then is to take digital articular x-rays (DAX) or cone-beam CT (CBCT) scans of your neck to look for signs of wear and tear or obstructions of your joints that can be the actual cause of your underlying nerve problems.
Here is also where a unique approach to healthcare known as the Blair technique and upper cervical chiropractic can help so that you may not need to go down the neck or hand surgery route, but where you can heal from "carpal tunnel syndrome" naturally.

I had a PFO heart surgery and woke up with severe nerve pain bilaterally in my arms and hands. I was subsequently diagnosed with carpal tunnel in both hands. I questioned why my lower arms were also in pain and was told “we don’t know why but it’s not related!” I refused the carpal tunnel surgery because I refused to believe that the issue was not related and I didn’t have the issue prior to my heart surgery! Plus it was on both sides! I can’t get decent medical care where I live and I’ve been on painkillers including norco and Lyrica for three and a half years! I’m in constant pain and my symptoms are BETTER at night! This has ruined my quality of life and I’m desperate for help…..I’m diagnosing myself with double crush syndrome/cervical radiculopathy! I’m telling my doctor this very soon but she won’t be able to help…….🥺🥺🥺

Update** I went to my specialist appointment and one of the 1st thing the doctor asked me was if I had any previous injuries to my neck or back, or if mt neck or back has been giving me any issues lately. So Im very confident that Im in good hands. Thanks Doc!!

Got a nerve test a few days ago I was borderline for carpal tunnel. Now I’m awaiting my appointment to go over the results with my orthopedist. I definitely have this awkward throbbing feeling in my forearm

If in the forearm as well, it could be an upstream problem w the neck, shoulder or elbow contributing as well. Thats what the NCV test will show.

Yes... I have been diagnosed with CT but I have been intuitively feeling the problematic points in different spots of the median nerve trajectory, until up to my neck-shoulder area. And this explains also why it all flared up badly again since I am visiting my mom and I don't have my orthopedic pillow.. I just wonder what I can do about it..

Thank you for this! My doctor was thinking I have carpal tunnel coz I get horrible pain and cramping in my finger and top of hand. But after watching this I wonder if it's due to or in relation to the bone spurs I have in my spine from falling of roofs and trees as a kid.

If it's the top/backside of your hand, that is not traditional carpal tunnel syndrome. That is a different nerve pathway. True CTS affects the palm side thumb, index and middle fingers only. Even then it can well be double crush, and if you know you have damage in your neck, I definitely wouldn't rule that out as a contributing cause. ... I really should do a followup video to this one. I once had a client who had CTS surgery because of tingling on their 4th and 5th fingers, which didn't work at all. Their neurologist should REALLY have known better bc that is a completely different nerve that does NOT go through the carpal tunnel at all. So no wonder! @@DrJeffreyHannahDC it is a debilitating pain on the top of my hand from the tip of my pointer finger down to the side of my thumb area. I am an artist, I can no longer draw, write, knit etc. I'm only 34. I also clench my fists in my sleep. It's been very depressing and has affected my quality of life. For now my doctor gave me braces for sleep to keep from clenching and gabapentin. I really hope we figure out what it is. Also I told her I don't get tingling or numbness just severe cramping and burning and inflammation. Thank you for responding!

A simple x-ray is an easy place to start. If there is evidence of osteoarthritis, there is likely some connection. To find out the specific details (depending on the circumstance) a DAX (digitial articular x-ray), CBCT or conventional CT would show if there is vertebral misalignment or disc injury that could be affecting the nerves or spinal cord in the neck.

I had neck persistent neck pain and my doctor prescribed 3 days of prednisone. On the first day, I lost feeling in both hands. I went to a neurologist and he said I had carpal tunnel syndrome and has me in spints and that it will take months to go away. Should I get another opinion?

Last month i woke up out of the blue with both hands numb and tingling. It is happening every morning the past 30 days. And while i drive with hand on steering wheel. All fingers except pinky so my doctor is having me have an EMG test done this week. They think it is carpal tunnel but after watching this video im wondering could it be in my spine?

(Not individualized advice. Always consult your healthcare professional.) If both hands, an MRI of the C spine is a wise part of any investigation including the EMG tests, which don't always show problems unless the nerve conduction is physically compressed.

@@jeremyrebel8931 Basically yes. Think of it like a flowing river. If there is only a downstream trickle, you can trace things back toward the blockage. The limit is if you get to the neck because you can't stick needles into the cervical spine nerves without causing LOTS of collateral damage. That's where an MRI helps fill in the gaps.

Ok guy. I’ve never had problems in my hands. Started this new job withn a month my right index finger half of it feels numb. Dad and dr say it’s carpal tunnel syndrome. Idk. Because it’s still feels like it did from day one. I’m starting to think I’ll never get the feeling in that part of my finger again. While everyone thinks I’m being superficial of a pansy about carpal tunnel syndrome. Ok if that what it is. How much longer till I get the felling in my finger back? Seriously. I don’t know if this is gonna make me like a villain or hero. I could help people or become a villain. This is how I treat my situation. And I’m about to have my first child so I’d like to keep this hand if possible. Or will I have to cut it off and become the wired dad!

Could also be a C6 or C7 nerve if only the index finger. If new work w a different head/neck/working position, it could be causing a flare. Best to have testing to come up w an accurate prognosis. See if you have a local specialist: UCCNearMe.com

@@DrJeffreyHannahDC so this could possibly not be carpal tunnel syndrome? Just a pinch nerve? Or how does this work? Because part of my finger is still numb ish. From the day it started to now. I’m ready to feel my fingers again. I’ve tried most everything I’ve seen online that I can do at home and nothing. I’m happy to hear from someone but I’d like just a little more info if possible. Thank you for there c6 and c 7 info…if it’s carpal tunnel you think it would come and go not stay for ever… a pinched nerve makes more sense to me

@@charlesterrellii7592 May not be. On your description, it probably isn't a pinched nerve though. That is actually VERY rare and dangerous. You'd be having a LOT more symptoms plus loss of hand movements if that was the case. Of course, I can't say that 100% sight unseen, so one way or another I'd recommend proper testing to find the exact cause (and solution).

Good question. It can, yes. This is what's known as a "double crush" where its the combination of both parts that can cause the symptoms in the wrist. It takes nothing away that true CTS DOES involve the wrist and may need surgery. Before that though, its important to tell the difference if its an upstream or downstream effect.

If it is the whole hand, that sounds like something different. It can still have a common connection with the neck, but the possibilities including Thoracic Outlet Syndrome, which happens when the muscles in the neck clamp down on the arteries to the hands and decrease blood flow.

Sorry, I don't fully understand your question. Plus of course I can never diagnose anyone sight unseen. That said, in terms of generalities and if I understand your question correctly, there are certain genetic conditions where two vertebrae in the neck are fused into one bone. This condition by itself typically does not cause carpal tunnel issues. However, it can be a "weak link in the chain" if there are other things going on can make it more likely to experience symptoms associated with that same area. I hope that helps (some).
