Did you go for distance in the refractive power of your monofocal lenses? I understand most people opt for this choice. If yes, do your reading glasses take care of intermediate distance (i.e., computer, and other arm's length activities) or are your monofocal lenses good enough for intermediate without glasses?

Stephen, hello there! I'm not a professional but here is my understanding! This "UFO-like" effect is quite understandable and even expected with a multi-focal lens. The multi-focal lens takes the light coming, and converts it into several (usually three) images projected on the retina. Each image of the three is focused on a distance : on near, intermediate and far. That's why there is always an overlap of there images at any given time, as your retina has three different images, each with different focus. Would this effect last? Personally as a 31 y/o cataract patient, I don't know, but from a testimony on KZfaq (I'll copy the link here next) of a guy with 5-year use of multi-focal lenses, he said that those extra ghost copies fade away more with time, maybe as the brain learns to ignore them and focus on the wanted image with the desired distance in focus. He also mentioned that it took him up to 8 months to reach there (If I'm not mistaken). How's the condition of your vision now in both eyes?

@@anassyria5176 Hello and thank you for your reply. The Vivity lenses supposedly focus over a continuum, and not at three distinct focal points like the previous generation. I am mightily embarrassed to say that I was so excited by the improvement in my vision that I was hasty in commenting on Dr. Devgan's excellent article here. I had not yet seen my optometrist and it turns out that the problem is a result of some minor astigmatism. So now I wear glasses for night driving and watching tv. What, that's it? Yup. My surgeon is not very communicative (there has been no post-surgery follow-up), but the optometrist told me that the astigmatism may be why my closeup vision is so good. I guess that's the tradeoff versus having astigmatic correction in the IOL. Once again, thanks to Dr. Devgan for providing these excellent articles that allow people in the general public to better understand their situation!

@@stephenrosenfeld833 Thank you too for the valuable information and experience! My bad I confused the continuum of the Vivity with other lenses. From what you say I understand that there's no noticable overlap/ghosting effect that affects the edges or contrast of fine details (Maybe except for the halos around lights at night).

@@ratherbfishing455 Yeah, good question! Here it is ten months later and still no followup. It was left in the hands of my optometrist. I should create a fake id so I can keep my anonymity in the smallish city I live in. Based on YOUR id, I'll relate that I once gave a bad Google review to a fishing rod company and they afterwards refused to repair the broken tip on my custom fly rod, even after I removed the review like they asked. Like you, I'd rather be fishing than engaging in comment battles. One good thing to note though is that I can see well enough up close with my Vivity lenses that I can tie on a small salmon fly without needing glasses. That was a concern before the operation!

thank you -- in the video I showed the young crystalline lens with the yellow graph. Accommodation allows the curve to shift at will to provide the widest range.

Did you get two different lenses? I like this idea

I suggest the same lens in both eyes. If you are happy with sharp and crisp vision in the first eye, why would you compromise that in the other eye? If anything aim maybe -0.5 in non dominant eye. Or just wear readers when needed. Quality quality quality

@@utube26able i got an Eyehance in the other eye as well and am very happy. Almost a year ago! However, i feel there is some loss of sharpness over time.

Hi Steve. How does the Eyehance at .5 handle car instruments? I'm considering swapping out my Vivity for Eyehance set at .5 for a number of reasons, but need to see my car instruments whilst driving.

@@stevenswann2835 Hi Steve, I have no issues at all with seeing car instruments. Only issue is with reading or using my IPad and so I use my reading glasses there. Very satisfied with the Eyehance lens.

I agree with dr UD, and you can also polish anterior capsule, if you want.

I use the full spectrum of IOLs. The Eyhance is a good lens but it fails to provide sufficient intermediate or near vision. For patients who want the absolute best distance vision, the J&J Tecnis ZCB00, the Alcon Clareon CNA0T0, or the B&L EnVista are likely better options. Plus, in my humble opinion, J&J has done a poor job of pricing the IOL: more than double the price for such a small incremental change.

​@@rhett7716 I went with Eyhance lenses, which for some reason (maybe Medicare?) did not cost extra, but did require an additional "AURA" refraction measurement to be done ($300 total per eye) during surgery after the old lens came out. But before he selected the final lens to implant. I asked my surgeon to do the non-dominant eye mini-monovision at minus .75. He reluctantly agreed after I showed him a screen shot of the mini-imonovision chart Dr. Devgan used in his video. He said he couldn't be that precise, so I would end up with one eye somewhere between minus .50 and 1.00. He didn't want to go beyond 1.00. I'm now three weeks post surgery and my vision is still changing (getting better nearer) but I have around 20/25 distance vision with both eyes and can see not only the speedometer in the car, but I also see clearly the buttons and switches on the steering wheel! My wife, who got regular single vision lens implanted, cannot read even the speedometer without glasses. I can also read my iPhone if I hold it out to about where my watchband sits. And can send texts and emails without glasses. I can't really tell any difference between the Plano eye and the minimonovision eye if I alternate between them. Distance and near vision seem the same. But I get terrific vision at both near and far distance with BOTH eyes open (again, from my watchband distance and beyond). I bought a set of 1.75 Walmart glasses to be able to see what I'm eating, and I'm using a 2.25 pair as readers. Only problem for me, a 78-year-old who had been myopic since seventh grade, is that I really miss the near vision I used to have about 2" to 3" in front of my face. Next week I get my new prescription for glasses, and plan to order a near vision pair for reading (I read two hours a day) and a separate pair for my computer distance. You can get $10 grams and $50 plastic lenses at Walmart. But I'll also get one pair of progressive glasses to wear around the house. Bottom line is between the "slightly extended" vision I got with Eyhance lens and putting the low-end (somewhere around .75) minimonovision Eyhance in one eye, I'm good to drive, go to Walmart, check my iPhone and even send replies on it. But in a restaurant or around home I'll still need glasses to be comfortable. Especially for reading. But you aren't going to believe the colors! The day after surgery I stood in the mustard/ketchup aisle at Safeway and marveled at the colors! And I had to wear sunglasses at home for a week after surgery because the light was so bright!

@@anassyria5176 completed second eye. August was right eye and October left eye and still healing. Just had one month followup after left eye. Was told that left eye might need YAG procedure but will hold off and see how things are once my next followup is done in three months.

Did you get your surgery done I’m 41 and have a cataract on my left eye never needed glasses it’s been so hard trying to decide I’m on the fence between a Toric mono or the vivity. what did you end up getting and how is your vision now?

If your refraction is off then the quality of vision from any lens will be off without glasses. If your problem is you aren’t plano then glasses or contact would help. If it’s contrast or quality of vision issues then consider exchanging the iols.

But doesn’t it depend on how bad your eyesight is to begin with? If you have presbyopia before surgery?

did you use Rayner. To get best and get smallest print vison?? don't you have you have to do about atleast 1.50 bet eyes. If do -.50 or so how much more vision do you get. I never had mono vision contact lens so no way of trying out . -1 more or less gets you maybe 18? inches to see near??? I would want under -1 difference. What experience did you have and what did they set eyes at?? Thanks

@@rachanait3889 الحمد لله. استطيع الرؤية الآن بشكل افضل. المشاكل التي كانت عندي انه كانت هناك أجزاء باقية في محفظة العين من العدسة الأصلية واكتشفت بعد فترة وصلت الى ستة أسابيع واضطررت للخضوغ لعملية اخرى لتنظيف المحفظة. سبب هذا التهابا في القرنية وانتفاخا. تلقيت بعدها قطرات مختلفة لمدة شهرين ثم تعافت عيني تدريجيا. وأصبحت قادرا على الرؤية بها بشكل جيد. لكنها لم تصل للمستوى الذي توقعته والتي تروج له الإعلانات

@@yassertamer7416 نفس شي قال لي طبيب عن تنظيف العدسة لم التجا اليها بعد لان صار لي فقط ستة اسابيع اما بالنسبة لقراءة عن قرب لا استطيع بعد ضبابية ولكن حسب ماقرات ان متل هده عدسات تستطيع رؤيا بها عن قرب لدالك زرعتها الحمد لله على سلامتك رغم ان طبيب اكد ان الليزر لن يسبب المضاعفات ومن خلال كلامك كانت مضاعفات

الطبيب هو اللدي اكد ان تحتاج الى تنظيف ودالك بعد اربعين يوم ولكن لن يعملها حتى تمر تلاتة اشهر بسبب ضبابية رؤيا ولا استطيع قراءة

There are the Crystalens, Trulign Toric and the new upcoming technology (wish it would come soon) TrueVision liquid lens. In principle the accommodating lenses are way superior and closer to to the natural eye mechanism

I would like to know the same thing. I believe that a monofocal lens plus glasses is the best for me also, but would like to hear what the Dr says. My current Rx is +5.0 -1.50@90 +2.50 which is strong hyperopia. Like you, I don't mind wearing glasses after the cataract surgery since I am skeptical that any IOL could give me optimal vision without glasses at all distances with my Rx, and I have worn glasses since I was about 5 years old. I tam thinking that the J&J TECNIS Toric II IOL would be best for me since it has a much higher abbe value (freedom from chromatic aberration) than AcrySof lenses from Alcon, although I heard that Alcon has a new IOL material called Clareon just made available in the USA.

Nobody answers this. I happen to know many opthalmologists. They all choose monofocal lenses for their own eyes. And wear glass3s for near vision.

@@katkat1194 No Sir. Eyhance lens is a monofocal lens and you may need glasses while reading. I am fitted with Eyhance IOL in both eyes still I need glasses for reading purpose.

J&J acrylic is way better than Alcon's...the quality of Acrysof acrylic is poor

Is it possible that your cataracts had yellowed, and that removing them revealed what your brain sees as a 'bluer' world?

@@MGrant-dx2tj Things appeared more vivid in that eye before cataract surgery, but now it's very extreme and the glare is horrible. I read that surgeons should discuss what lens to use in case the patient is an artist or interior designer. I met another patient who had surgery at the same hospital with the same complaint.

@@ratherbfishing455 I agree that surgeons should be aware of the patient's career and lifestyle before recommending a lens. I switched surgeons for this very reason. I hope that you're able to find a resolution for your situation.

@@MGrant-dx2tj Thank you! I think that this teaching hospital is overwhelmed with patients, and a resident doctor saw me without the supervising surgeon being in the room. You're lucky if you get a few minutes to discuss the surgery ahead of time. They need a checklist and time to read a patient's file, as one time a doctor told me to stop the Muro drops. He did not know that I had swollen corneas.
