Thank you for this video. I have been coping with intense, practically 24/7 pain in both of my knees and was diagnosed via MRI with Chondromalacia patella (though mine was traumatic, caused by an accident and not athleticism) for almost 3 whole years now. I went through almost all of these stages except for surgery. While my PT group were really great and well-intentioned, I’ve gone for many months and only gotten worse. They’ve had me focused on my thigh muscles and doing these awful deep squats, wall slides, leg press, and quad stretches. I haven’t gotten better and actually have only gotten worse. I’m only 25 years old, and had pretty much decided I’ll just have to live in this pain forever before seeing your video. I’ll try out your glute exercises and hope for the best. It feels like none of my doctors have taken this seriously. I’ve even lost 27 pounds trying desperately to relieve some of this pain to no avail. Wish me luck.

I was just diagnosed with this and MRI shows missing cartilage in the weight-bearing portion of my kneecap. No one ever suggested physical therapy or resting! ( I also am a floral designer and stand for 8 hours a day 35 hours a week. I've been doing this job for the past 30 years ) They just immediately are talking about cortisone injections and surgery! I am 50 pretty active, and not overweight. I am so glad I found you and that I've been doing my own research instead of just following their recommendations blindly! It's so frustrating! Also the surgery that they wanted to do was micro fracturing. When I looked that up I saw how pathetic the results were! Anyway, I feel very relieved and hopeful after watching some of your videos. Thankyou for this. Since I've been doing the baby squats and other glute excersises I'm getting only occasional knee pain. Previously my weight sessions were a real trial, as the lunges, squats etc were a pain fest I made myself endure. Having watched several of your videos I realise I'm so quad dominant (and have knee arthritis) and I'm a cyclist and a runner too.2 of my friends have recently scratched from ultra cycling races with swollen knee pain, so I know hours on the bike is not great unless you correct the imbalance

I fell and hit my knee cap many years ago. At age 56 my X-ray of my right knee in 2020 showed high grade chindromalacia and chronic patellar Retinaculum tear. Very painful at night when I try to sleep and very stiff. I plan next year seeing a knee specialist in another state with better technology because in Miami , Fl I am not getting any help. I am not going to live with this any longer. I am wasting what Years I have left. I want to be pain free or at lease half pain free. I used up all my treatment injections and PRP ing already. Only seem to be getting worse . I love your informative videos . I’ll keep learning from you. TY

Thanks a lot for the useful information! I'm 66 year old now and am almost sure that I'm suffering from chondromalacia patella for many years now. In that time there have been made X-rays, a MRI, a CT scan and a scintigraphy. Also have done many kinds of exercises, but all the things I have done only made my ;problem worse. I think you are spot on about how to address this problem. I hope that your approach and exercises will help to relief my pain and that I will get mobile again and get a part of my life back.

I’m 69 female active I have OA a slight tear in my meniscus kneecap pops and tracks to the outside. I have been doing physiotherapist somewhat contrary to your suggestions VMO and all. Thank you for your suggestions!! ❤

It's insane how you explained how this condition typically starts because it started for me in highschool after football practice. Walking for the field to the bus and it was like the power to my knee went out. Then came back on. I've always blamed it on Coach having us take a knee to talk to us.

This is exactly my problem. My knee injury had made my kneecap not align in the groove the right way. So now it has rubbed and worn off some of my knee cartilage. My doctor had been sending me to physical therapy and told me I could go for my walks (I would walk a mile every day). In doing this it caused the wear on my kneecap. I didn't know this at the time. I kept telling him my kneecap was not in the right place. You could see by looking at it that it didn't match my other kneecap. He kept telling me it was where it should be. I wish I had been able to go to a good doctor because this one I feel has caused me a lot of needless damage. This information is really helpful in understanding my situation. Thankyou :)

Hello doctor. Thank you for your educational videos on this topic. Does it help to walk on treadmill with high inclination. I tend to feel a lot of contraction in the glutes and the quads don’t tighten.

MRI showed cartillage defect in my knees. Therapy isnt helping at all. I will be getting PRP shots. However, if they dont work, surgery is the only route. Can you live pain free with torn cartilage? If not, then surgery is the only option.

@@sagarisbusy hey there! Yes my pain has gone significantly. I have been taking physiotherapy sessions locally in my city. And also started to lose weight. Medicine will only help you for a sometime. But for long term relief you need to be persistent with your lifestyle

Do you have any exercises to strenghten knee after surgery, I was told I have chrondopathy (it rattles in my knee when I bend it) after arthroscopic surgery of my patella last year (because it dislocated twice) but it’s been more than a year since that surgery, I can walk but I still can’t run and feel secure when I walk…

Thanks a lot for this video.. I was doing quad strengthening exercises and knee pain was not getting better. I see why now.. I was diagnosed by doctor as having chondromalacia patella but I am curious if knee stiffness a symptom of it. I ask this as I am not sure if I should start the glute exercises with knee stiffness or wait until it gets better.. thoughts?

I still can’t put my hand on the difference between PFPS and chondromalacia patella ? I’m 21 and the dr diagnosed me with chondromalacia just from the Clark’s sign is that normal or should I consider it PFPS

Hello sir I just came across ur video today, and u have just explained whatever am going through right now. I have been diagnosed with stage 4 chondromalacia patella with geode formation and edema around for left knee and same with right knee though haven’t done a MRI yet.. doctor has prescribed vigorous Physio for a month followed by by diagnostic arthroscopy if pain persists. I had been doing actively yoga since a year which could have worsened it to stage 4. kindly provide the right exercise video for my sake🙏🙏🙏🙏

Doctor I called down in my room on my front body and after 10 month my ortho said mri show cartrrage damaged what you advise me to do which exercises you advise and do bone broth, glogosimin and msm will help?

Thank you so much for your help, I have chondromalacia patella stage 2 may be even worse for almost 3 years now, part of which I did those quad mascles exercise and it make me feel worse, so now I am doing gluts ones for awhile but my knee still hurts , still I do beleive doing gluts are proper path to go and I will continue follow your advise making my gluts stronger to get better with chondromalacia patella but I also had knee cup dislocation very painful which is how my chondromalacia starts at the beginning and it keep somewhat sliding and painful while I standing and walking do you think I need to use tapes to keep it in proper position or use brace? I try sleeve bra ice but it compress my knee cup too much and other braces are very uncomfortable too so I am doing taping now around knee cup and put it little bit up releasing some space between knee cup and thigh bone it is decompres it little bit but still hurts , could you please gave me some suggestion what to do about it I do not want to do surgery . Thank you so much again for your hard work to helping others!

@@ManojKumar-ti9lb Hi! I been doing those exercises for 1.5 years and I am really thankful that I found them ! My situation is very difficult, but if not for those exercises I would probable can’t walk or walk very little . Before I found this amazing doctor through the internet I did quad exercises and as he keep saying that they are very wrong for my condition. I do believe that quad exercises can do more harm than good, and ones I start other ones it feels definitely better , but you need to do them every day to the rest of your life and they will help you to maintain your problems. To me those exercises Is God blessing and I do really appreciate and thankful for this amazing doctor who really care if other people .If you have similar problem or problems which doctor mention in him videos you will benefit from them , take care

Hi Swati, Swimming and cycling will be helpful if you can get your glutes to work rather than your quad muscles during the exercise. If you feel you are using your quads more than your glutes while swimming, then you will likely make your knee problem worse. I used to live in a place where I had to use the stairs more than 10 times a day for more than 10 years . I am 24 years old now, and an MRI scan recently showed that I have grade 1 chondromalacia. I used to do bodybuilding and TRX exercises without any pain, but occasionally I would experience severe pain which I ignored. I haven't been able to exercise for a year due to my studies, and a few weeks ago I had severe knee pain and had an MRI. Should I completely avoid using stairs? I weigh 72 kilograms and I am 168 centimeters tall. Has my overweight condition exacerbated my problem? Is cycling harmful for me?

Hi Dr, I hope you are well. I appreciate you are busy. I have Patellar Chondromalacia and there is also an intra-articular loose body, a small structure around four millimeters in size, which may be a detached fragment of the articular cartilage. The articular cartilage injury is graded as four, The ligaments and menisci appear normal, and I have a Patella Alta, which is a slight elevation of the kneecap that may or may not contribute to the altered biomechanics and erosion of the articular cartilage. The report also mentions minimal fluid within the joint space, which is not of much concern. I see online Dr, You are to be the best around when it comes to Chondromalacia so any advice how to best treat it will be much appreciated as I’m fit 44yrs old and I’ve just got a HA injection but don’t think it’s helped. Again, I appreciate you are busy and you might not have time to respond. Thanks Colin Colin Carrick

@@ramakrishna-ri3sx this typically is not a life time problem. I’m 18 and have had knee pain for about 15 months and it has progressed to chondromalacia stage 1. It can be fixed but can also progress through all 4 stages and enter into osteoarthritis😢

@@joshhazel742 no use doctor done PRP and viscous injections still now use. The pain levels are still same I am not putting more stress on legs. I stop workouts. Life is very difficult can't even sit in Indian Tolites using Western. I changed the doctor he suggested me physiotherapy but still no use. Spending nearly 12k rupees per month but still no recovery. 😞😞😞

If you notice that your quads are sore from a workout, should you massage/roll out those quad muscles so that they are not pulling as tightly on the knee cap?
