Cirrhosis Assessment for Nursing Students

This video provides an overview of the assessment and underlying pathophysiology of the cirrhosis patient.
I am Gail L Lupica with over 20 years of experience teaching nursing at the pre-licensure level. I earned a Bachelor Science in Nursing at SUNY Binghamton in 1986, a Master's in Nursing Education in 1992 from New York University (NYU) and a PhD in Adult and Post-Secondary Education in 2011. I have many years of experience in NCLEX-RN test preparation and remediation.

Gail I LOVE the simple straightforward but not too basic teaching and instruction on every single concept!!! Gail, you mentioned 3 reasons for bleeding in liver patients. Splenomegaly, decreased production of proteins, and malabsorption of vitamin K. In our lecture, we were taught that the lack of clotting factors was a key reason for bleeding in liver patients. Are clotting factors the same as the proteins?

Hi Winona, Not necessarily. The indirect (or direct) bilirubin level could be very elevated when the liver is temporarily inflamed.

Great videos! I'm a little confused. My textbook says in hemolytic jaundice the bilirubin is unconjugated and it is NOT excreted in the urine because it is not water soluble but in biliary obstruction, the bilirubin is conjugated and water soluble so it can be excreted in the urine. In your video you said the unconjugated bilirubin is excreted in the urine, do you mind explaining?

Hi Ashley! I don't even remember referring to excretion of it. Everything you said is correct! Unconjugated bilirubin is elevated in hemolytic jaundice and conjugated bili is elevated in obstructive jaundice and knowing that is useful when looking at their labs when they first come in. If your professor is asking you about excretion of it, then the fact that unconjugated bili is fat soluble and therefore not excreted in urine, makes perfect sense.

Ashley Jonelle Tierneyi know I’m late to the party here lol. I’m learning this section in Patho right now. According to what I have learned, there are 3 categories of jaundice: prehaptic, intrahepatic, and posthepatic. In prehepatic, you wouldn’t see dark urine as the bilirubin isn’t conjugated yet. In intrahepatic, both levels of unconjugated AND conjugated bilirubin is elevated so you will have dark urine. With posthepatic, you also have elevated conjugated bilirubin but also obstruction in bile flow,so you will see dark urine and pale stools.
