What a fantastic video!! This provides such a realistic look to the restoration, very much like intrinsic occlusal stains on natural teeth. I like this method much more than the external staining that sticks out like a sore thumb and does not last long. Thank you Dr. Stevenson!

Until now I was thinking that, on average, American dentists were somewhat less artistic and pedantic than some of the popular Instagram dentists, seemed like the majority just didn't care that much about the anatomy and technique ( I shadowed in NYC where a dentist worked in 3 operatories simultaneously, yielding subpar standard results). You proved me wrong. As a recent dental graduate from Europe I am deeply impressed with the skills you have and the way you elaborate on the topic material. I am really looking forward to upcoming lessons on your channel. You have one of the best or maybe the Best content for dentists on the internet. It is just frustrating to see on post-op xray that my class II restoration didn’t adapt well gingivally, even if it looks anatomically good and has a nice contact point.. I had to reschedule the patient for a re-do. As a young dentist, I wanted to be accustomed to excellence, that’s actually the beauty of this profession .

Thank you Dr Stevenson. I have really been helped by your videos. When the interdental box is narrow it is difficult to push the composite against the band as the interproximal carver is too big. Have you any tips?

I'd use the injection moulding technique for this situation. Fill the box part way with flowable, then squeeze in paste composite, thereby displacing the flowable - works well for small class II's.

Hi Doctor - yes, I do adjacent class II's one at a time - seems to work well, and ends up taking about the same time, with the added benefit of being able to verify the interproximal seal on the first restoration.

I was using a paste conventional composite - not a bulk fill so layers help with better polymerization and conversion rates, and also, we can give somewhat better esthetics with deeper dentin shades and more frosty enamel shades covering them.

I have calculated the C factor for various approaches and have found for class II's the wall and lobe is the least, actually about 0.4! The key is incremental placement in anatomical shapes...

Yes, I use Modeling Resin (Bisco) - works great and doesn't decrease the compete strength.

Very good techniques and with practice they work well. The use of a microscope plays a major role in his techniques. When I was Chair of Restorative at UCLA, I added the injection molding technique to our curriculum. Perhaps the only criticism I have is that little clinical research - prospective studies - have been done to provide scientific validity...nonetheless, the techniques he teaches are beautiful and certainly compelling. Thank you!

Stevenson Dental Solutions how about polymerization shrinkage? I mean they basically cure the resin in one large increment .. wouldn’t that cause problems ?

Yes, this would likely lead to problems if the mass was large, hence I've suggested that the snow-plow (injection molding) technique is only indicated for very small cavities - like slots. Bulk filling is here to stay, even if though it has been shown to be inferior to incremental filling in clinical trials. SonicFil by Kavo-Kerr has outperformed other bulk fill materials - but I'm not sure if Clark uses this product. Again, his techniques appear beautiful upon completion but lack the scientific assurance of longevity that many of us require before using new techniques.

just wondering doc, before you put composite in, there was a gap between the matrix and the tooth. Did you try to remove that gap first or just go ahead and restore and deal with flash later?

I'm looking for that treatment here in Sunnyvale, CA. But all dentist trying to make more money with implants or crowns! I know that kind of restoration in my country, but now I living here and all dentist trying to make implants or destroy the tooth and put a crown. I looking for a honest and affordable dentist can fight to maintain your tooth in your mouth without degrading your tooth. I would like to know if you can recommend one, would be amazing.

Doctor, please answer my question. I am a fourth-year dental student. When I get the filling after about a week or less, the filling falls out completely, even though the materials used in the filling are good (I use the Tokiyama filling).The bond is also good, and the cavity I make has the same conditions required for an amalgam filling cavity, but I do not know what the main reason is.

Could be contaminated air - oil in the system. This would be the only thing I can think of other than moisture contamination, but I doubt this is the issue.

What about occlusion dr ? When you all that beautiful work and then you have to reduce again becauz occlusion is not perfection or higher

We usually use the pre-operative contact points and adjacent tooth landmarks to avoid hyper occlusion issues. The objective is achievable with a little extra effort. Frequently zero or little adjustment is required when the lobes are built based on anatomical form...
