Metal fillings can be replaced with direct composite resin fillings, also known as tooth-colored fillings. Direct composite resin fillings provide patients' teeth a natural-looking appearance, and only their dentist will be aware of the fillings. Very good video doc that guides us step by step. However without viewing the follow up video on composite fabrication and cementation, I would not be completely satisfied. So I am eagerly awaiting it doc .

Just wanted to demonstrate an occlusal composite onlay. If the facial cusp has exposed dentin, it is structurally unsound and needs to be addressed. The onlay accomplishes this.

Doc what would happen if you did not remove the undercut enamel on the buccal cusp and just filled the area with composite

I don’t know. The tooth is structurally compromised - I typically prefer to minimize the risk of potential fractures by removing the undermined areas during the restoration. Thank you for the question.

thanks for the video, great as always. I'm really interested in how you create those artificial caries, would you mind telling your secret ?

Cayenne pepper and turmeric, mixed with flowable. The spices don't allow for complete hardening of the mix, hence the caries - like texture...now my work station smells like exotic cuisine!

Doctor Stevenson what's the prognosis for such restoration for endo treated teeth ? how many years would it last weather a premolar or a molar , and how do you compare it to an amalgam overlay for endo treated molars ? thanks..

There isn't a lot of clear and strong evidence for this approach long term. It would be best to consider this or an amalgam onlay as an interim restoration, perhaps for a few years before resources were available to proceed with an indirect restoration with coronal support.

Wht to remove cusps? Will they break otherwise? Is Inlay resroration accetible in this case?

The planes are simply the prep inclines to follow the tooth inclines. Each posterior tooth, upper and lower has two, plus the functional cusp bevel.

Generally if they are less than 1 mm, I usually remove them. Of course occlusal issues and tooth structural compromises and esthetics must also be considered...

it's an excellent good prep. + totally educational. but this is against conservatism❌ because you removed A LOT of tooth structure just to make onlay and you can easily apply composite in this small cavity!

7:55 would you recommend leaving the undermined enamel? The facial cusp was fractured of completely and left exposed dentin the the lingual cusp was severely undermined. I think that a direct composite onlay just about the longest lasting conservative approach I can think of. Thank you for bring your insights to my attention - I'm happy to learn from you.
