You should probably include that the Challenger exploded due to the failure of two O- Rings (large rubber sealing rings) in between two lower segments of the right solid rocket booster. This then burned a hole in the main tank. The solid rocket booster then broke free from only one of its mountings, causing abnormal thrust vectoring. The SRB collided with the inter tank structure, causing the full rocket to pitch unusually. This subjected the spacecraft to aerodynamic forces the shuttle was not designed for. The entire shuttle and external fuel tank disintegrated. The crew compartment however, was still intact, and the crew was most likely alive. The sad thing is, the crew survived the explosion and subsequent breakup, but died when the cockpit hit the water. Very sad.

@scammy I did mention the failure. I remember hearing about the crew surviving the initial explosion, but nobody knew that at the time. All that really remained of the Challenger was a fireball in the sky. It was a disaster that should never have happened. Engineers knew about the O-rings, because they had problems with it in the past, yet they still approved the shuttle for flight.

People still blame the Challenger disaster to this day(i suppose even tho im not American. i am filipino tbh)

Another tiny thing that ruined everything: Back when the Soviet Union was still around during the Space Race between them and the US, they were attempting to put a man on the Moon before the Americans. They designed the N1/L3, a rocket that was built to compete with the United States' Apollo program. Apparently, they launched a few days before the Saturn V rocket. However, a single loose bolt caused a massive failure. It got sucked into the fuel pump, bounced around like a damn ping-pong ball, and set the fuel off, killing 91 people and being called the largest manmade explosion aside from the nuclear bomb. Had someone remembered to tighten that single bolt, there would have been a Soviet flag on the Moon and not an American flag.
