There was a study in a small German town , they took 1000 people and found out 14% have the antibodies . That study may give insight on the spread of the virus and immunity

This is merely the intermediate unofficial preview. In Germany there is some controversy about that study and looks like we'll have to wait for the official results. They use antibody tests, the company claims it has specificity of 99%, however, no institution has verified it yet. In this time of crisis the company regulates itself, basically. My question there is: What can be said about the antibody tests? To what extend might they react to immunity against other coronaviruses that cause common colds?

I think that's down to this thing being around longer than most people think. There was something nasty going around here in the UK as early as November and December. The UK government delaying the lockdown SHOULD mean that we'll get herd immunity before anyone else. They've sacrificed lives in the beginning to protect MORE lives in the not-too-distant future. They've denied that they've had a herd immunity policy but it's pretty clear to anyone with any sense that they did indeed have this policy in place at the start and are continuing with it. As the good doctor has said the only way to beat a pandemic like this is with herd immunity, no matter which way you achieve it (getting people sick deliberately, delaying the sickness and/or via a vaccine).

theknob1 but what about your families safety? Friends? We as people who care about others in our lives should feel responsible to protect them. Infection doesn’t work like violence; it easily spreads unseen in asymptomatics then can be caught by someone you love who reacts differently. Would you feel responsible for contributing to the spread that may kill your family or friends? It’s better for the community in a whole NOT to spread it so that the vulnerable have less of a chance of being exposed. The vulnerable rely on us most of the time anyways. Please keep your responsibility to protect the lives of fellow citizens. I care about you so I’m staying home.

Thx for the information and answers, I can almost keep up with you, without medication sometimes it's hard to keep up with your discussions ,I usually have to go back and listen a few more times when I have to know something better, nyc should keep things washed out in the drains and sewers better I used to go there often, in the winter in nyc the steam that comes up through the manhole covers in the road is moist probably it's dry on the ceilings through the sewers leaves skin greasy, ny state there's a lot of nats and more ticks the outdoor cats are loaded with ticks , is there anything that can done to stop the ticks other than burn the grasses? Any way to infect a population of ticks without damaging other life

The logic of prophylaxis is correct as instead of delaying the spread over people delay the effect of virus to get immunity.

I have two questions: 1. Do we have assurance that once recovered from Covid-19 we develop immunity? 2. Wouldn’t social distancing measures be the best option two allow people to go outside again granting that the spread will be controled or at least slower?

There might be clues found among those who have had a light version of the disease and those that had a severe case. Did it have anything to do with the initial viral load they received? If so, might herd immunity be increased by having volunteers permitting themselves to receive small doses of the live virus?

What is the length of time before a second antibody count be taken to find out if the count is increasing or decreasing? With the antibodies not present in the bloodstream 'in numbers' previous to becoming infected wouldn't the antibody count from the 2nd count have to be lower than the 1st count have to be less to claim immunity. With all the tests they're given for coronavirus are they going to have a 2nd or 3rd test?

Excellent video Dr. Been. Another aspect of developing herd immunity could be to build a pool of convalescent plasma donors or a bank. I understand the scalability is limited. But if we can scale up the bank during lockdown, herd immunity can become a showdown between the R0 of the convalescent plasma delivery vs. R0 of the virus itself, and the R0 of the former is likely to win out - especially in the US, where there is the largest number of people who are covid positive, and consequently the largest number of people who will be immune to the disease.

There is another factor, very important. The lockdown has reduced the # of active cases enough so we could then identify and isolate emerging cases with extensive tracing.

@@DrBeenMedicalLectures There is a potentially faulty assumption there. You cannot definitively identify and isolate all the cases. We know that some people do not appear to ever show significant symptoms. If true, you have no hope of isolating everyone who has it. As it is right now, by in large the only people getting tested are people who are showing symptoms. People with the virus but with no (or mild) symptoms are not getting tested and therefore not included in any of our current data. I see this as a huge blind spot. We have no idea what the actual penetrance of this virus is in the population, and we won't know until there is wide spread antibody testing. If I could only get one piece of data, it would be that one because it has the potential to change everything we are doing. It is quite possible we are much further along the curve than we realize. This would also have a huge impact on the actual mortality and morbidity numbers, causing them to be much lower than first thought. Your data point that 14% of people require medical assistance may be actually be lower once ALL the cases are known. I guess we wont know until the dust settles, but it bothers me that this uncertainty in the data is completely ignored. We lose credibility when we make proclamations with certainty and then have to admit we were wrong later. We need to be more clear to the public about what we don't know and what assumptions we are making in the calculations and models. Another issue I am seeing is that many doctors are realizing that some people were being put on ventilators that perhaps shouldn't have been. Management practices are actively changing as we speak, with more conservative treatments like CPAP and hyperbaric O2 being used. Is it possible that the majority of the "young healthy" people who died fall into this perhaps iatrogenic group? And do we really understand the pathophysiology of this illness? We all understand old frail and sick people are vulnerable, but do we have a good explanation for how young people can range from completely asymptomatic to dying? This just feels like we don't fully understand what is going on. I have seen doctors' discussions on YT talking about seeing pulse oximeter readings that are truly frightening and indicate intubation, however the pt is in no acute distress and is talking and carrying on a casual conversation. They describe the clinical picture as being like high-altitude sickness rather than ARDS. Again, this tells me there is something going on here we don't fully understand, which always means we need to take a step back and question our assumptions.

@@ericwilliams5930 There have been a couple of serology studies one being from Stanford which studied 3500 people from Santa Clara which was a hotspot for the virus in California. Study here www.medrxiv.org/content/10.1101/2020.04.14.20062463v1. I saw an interview with the Doctor who headed the test and he thought the study showed the mortality rate more in line with the flu maybe a little greater but nothing near what Dr. Fauci was throwing out that the mortality rate was 10 times that of the flu. Due to the latency of the disease it does make it more difficult to figure out the exact rate. But I absolutely agree that you can not make a determination without knowing how many people are infected or were infected. It really is guess work if you do not have the complete data. These models drive you crazy. The California test also only showed 3% of the population of Santa Clara had developed antibodies. That's a long long way from any type of herd immunity. Interestingly the doctor also said that that figure makes it impossible to do any real contact tracing. Another study from the Netherlands -which I have not found the actual study- took 10,000 random samples of donated blood and also found that 3% of the samples had covid9 antibodies present. Netherlands has 17 million people, that adds another 510,000 cases to the current number of infections which at this moment is only between 32 and 33,000 people. This would drastically bring the mortality rate of the virus down. I have only seen a couple of news articles on the study, and unfortunately, as with the Stanford study the only thing anyone seems to concentrate on is how many people have developed the antibodies and not how that number affects among other things the mortality rate of this disease. The doc who did the Stanford study has also completed or is near completing two other antibody studies. That will be made public in the next couple of weeks. Interestingly he said the cost of the studies were in the $200,000 range which is nothing compared to the money that is being thrown around at this moment. The United States can easily spend a couple of billion dollars on targeted serology testing throughout the country that would start to answer some of these vital questions and give us some real data that allows us to determine if shutting down the whole country is the right course and if the costs are worth it keeping in mind that there are also health hazards that come with shutting down an economy and not just the enormous economic impact on hundreds millions of people through out the world. But at least at long last some real data is being provided. When this all clears I am of the opinion that the folks who make all these wildly divergent models among others will have a lot of explaining to do.

You are a glorious voice of reason amidst all this madness. We proudly thank you. Just as I have been curious among all the political emphasis and hospital use of ventilators. In my personal opinion the lack of mention for CPAP and BiPap machines is a bit nerve racking and alot to be concerned about. I understand they are more likely to work for milder cases so long as they maintain consciousness but is it not in the long run more dangerous to have these patients use the ventilators long-term(days or weeks)?

Thank you for your kind note. I think it's case to case that the ventilators may have unwanted side effects. I think doctors are also fearful of doing something else than the normal protocols. I think CPAP and BiPAP should be used as well.

Good afternoon dr. I had a question towards my wife. She’s been sick for almost 4 weeks now, she’s been to the ER twice now and they said that all her blood work and vitals and x-rays are good. They sent her home and said that at this point she can’t get worse it’s just part of the recovery, she only has the discomfort and some pain in her back and chest which triggers her anxiety. What do you think? Is she on the road to recovery or can she get worse?

i am an ordinary citizen from india. Kindly clarify my understanding. I hereby confirm you that i will not take this medicines with out consulting my doctor except Vitamin C, vitamin D and Lecal-MZ (calcium, magnesium, zinc and vitamin D3 combination) which i am taking for the last 6 months for improving Immunity. Re the prophylaxis approach, i think it can be further improved by voluntary isolation of people with underlying medical conditions so that herd immunity will be geared toward those who have better chances of recovery

Gov Newsom of California announced that both antigen and antibody tests will be ramping up soon. I hope this is the case, because I do not understand why it's so hard to get a test. Is it a matter of being able to pay for it? Thank you very much, Doctor.

Sir plz answer... I am frm India, here Gov. categorised Districts in Red, Orange and green zone. Red r hotspot, Orange r relatively lesser cases and green r those having zero cases frm last 21 days. Can it safe to say green zone districts r free from Covid, is it possible to make Whole India as green zone until zero cases by lockdown. Thnx

Hi Dr Mobeen. I saw some studies recently looking at how SARS-CoV-2 could be invading T cells via the CD147 protein, similar to MERS and SARS-CoV, which would contribute to its high infectivity and lymphocytopenia. Any thoughts on this, reliability of the study, implications, treatments?

Not a doctor or scientist here... just trying to understand... Am I correct your calculations based on the R0 values discussed (2 = 50%, 3 = 67%) and the outcomes posited assume 100% of the population has an equal potential for death from C19? If yes, then how does your considerations/death projections change if the population variable adjusts to only the percentage of population that is in the highest risk categories (i.e., over 65 y/o and w/relevant co-morbitities)? Am I correct in thinking the threat probability outcomes would be far less (i.e., less harmful amid a mitigation-focused release from stay-at-home protocols)?

I'm fascinated by the 'compassionate use' case of using un-trialed therapies and prophylaxis. Couldn't doctors and ICUs be collecting and methodically sharing data about which approaches have led to improved outcomes? Are they doing this?

Dr Been has suggested an acidic environment is best for the patient. Elsewhere, it has been suggested that sodium bicarbonate worsens the condition of patients experiencing diseases comprising symptoms similar to Covid19, particularly AMS (acute mountain sickness), HAPE (high altitude pulmonary edema) and HACE (like HAPE but cerebral manifestation). It’s an interesting topic, since many people tout seemingly innocuous dietary supplementation, would abstaining from ingesting either help or hurt? Vinegar has to be detoxified by your body after ingestion. And compromised organs are a hallmark of this disease.

I’m not a mountain climber but I found the altitude sickness comparisons interesting. You might find what you’re looking for by veering into that domain. As I understand it, the hypoxic body will work itself into acidosis in order to hyperventilate to get more oxygen. Wouldn’t it follow that trying to alkalize the body would be adding an additional burden?

I think each country has circumstances that are unique. In Australia we are able to keep diseases that are endemic to our nearest neighbour (PNG) out, because we are an island nation with very large distances between towns and states. Would it not be better to have another 8 weeks of lockdown and then keep the disease out of the country by quarantining all arrivals?

Paul, this virus has almost certainly been making the rounds of the US and UK since Nov19 or earlier. If they had it there we had it here. I'm not sure where you are in AU but I'm down bottom of Victoria and this month was pretty much our last chance to produce Vit d from the sun (as UVB gets reflected by atmosphere at out latitude from May to Sep). The lockdown has not only destroyed our economy, making multiple businesses fold, increase depression suicide and domestic violence but has negatively affected our health and made us more vulnerable to serious illness due to stress and lower than normal vit d levels. Supplements simply can not do everything the sun can. Tasmania is going to be screwed. If you're from the top half of the country then you are worrying unnecessarily as the weather pattern 5-11°C that this virus thrives in will simply not hit you. Look to 'climate reanalyzer' monthly temperature weather patterns from 2018 & 2019. Or 'earth dot nullschool' to check out current weather. Recommended to select 1000 hPa to get better overview. I'm in an area that will be in that weather pattern from Jun-Sep20 and we want out. The big decisions are being made by Chicken Littles in Sydney and Canberra that will still be able to get UVB after April and are not even in the weather pattern. I just re-read your msg. You just meant stop entries. The govt will not open the airports for sometime - we're still having to beg for our basic liberties back.

What is the exact technical definition of herd inmunity? All the definitions ive read are kinda vague and not very precise. Also, what are the concequences of it? The virus eventually dying unless it mutates strongly?

It's the enough number of people infected in a community that the infection stabilizes. Which means, one person only infects one more person. Technically it is (R0-1)/R0. To your second question. Eventual outgoing is that either the virus adapts to us and becomes harmless or we all become immune by vaccinating or contracting it. It may also end up to resurfaced every so often in various seasons.

For example, there’s evidence that serum levels of pro inflammatory Interleukin-6 is lowered by taking fish oil, or is found to be lower in yoga practitioners. Are you sure? anecdotal data, untested, ignores the effect of long term prophylaxis on the immune system. as well as the fact that what we are doing is hardly prophylaxis , since we are increasing the number of people spending extended time in poorly ventilated, climate controlled indoor environments, while continuing to provide just enough randomly selected essential workers to keep the exposure going, while keeping those tight little boxes of poorly ventilated, climate controlled, environments on wheels running (public transportation) which not only can take them back home to infect their family, but is possibly the most efficient hub, to connect the largest number of people, from the most diverse number of locations, to extended exposure in the tightest confines of such favorable parameters to the virus to spread. (See New York City, Chicago, etc, as well as nursing homes, prisons, cruise ships, naval vessels etc) You get brownie points for agreeing with the professor, but in the real world. you get sick. not to mention , that if we know that the disease doubles every 3 days, we would certainly have 1,073,741,824 cases already worldwide. Also the real world, you cannot shut down society to an effective enough degree, for long enough, without creating upheaval in the food supply, and without devastating the majority of the population who will no longer have the means to make a living. not to mention that it is a logistic impossibility. I also would not hold your breath for a vaccine. there are 6 known corona viruses that can infect humans, some have been around for many decades to date, we have not found a vaccine for even the oldest: the common cold. You can however, practice the time proven strategy of isolating and quarantining the sick, and isolating those who we know have the most chance of fatality from the disease, (controlling 1 to 5 percent of the population is much more plausible than controlling the entire population). while the healthy 95% develop herd immunity.

Thank you for your wonderful videos. I like the prophylaxis idea but I'd not take Hydroxychloroquine! I'm taking 12000 IU of D3 each day along with one 600Mcg K2 capsule I have been taking 8000 mg of Vit C but now I can't get hold of more. I am taking ionic zinc and magnesium each day Also looking to add Quercetin and Astaxanthin to my daily supplement regime Now I need to sort out my sleep! Getting to bed at the same time each night and sleeping for at least 7.5 hrs That is VITAL and I've never managed it my whole life! Need to start exercising too as I'm far too inactive And I need to work very hard to lower my BP which is dangerously high. I think we should all do our own research and take the steps we consider right for us to make ourselves more healthy and robust. I eat organically produced food and don't eat wheat or sugar (other than that in the red wine I drink) and will eventually stop eating the rice cakes and remove all grains from my diet. I also fast for a minimum of 36 hours three times a week which I believe along with my low carbohydrate diet must be inducing autophagy on a regular basis. We know that many people suffer a mild illness from SARS-CoV-2 and that some people don't even know that they contracted it. I believe that if we were not in such a poor state with terrible diets, high levels of obesity and auto-immune conditions etc. this would not be anywhere near the problem it is. I know my weaknesses. I need to sort out my sleep, lose about 10lb, and work harder to get my BP back down to 'normal' levels like I did a few years ago - I really should have kept up what I did back then every day! If we can become healthier in general as a result of this then that will most certainly be a silver lining of this pandemic. I mean, look what Dr Terry Wahls achieved!

Building our immune systems is key. We dont hear enough ways to build our immune systems up, which would enable us to put this bug to rest and as a matter fact any other bug that might infect us. I kind of think of it , like having the chicken pox as kids we all wanted to get exposed to the virus, and basically became immune to the disease(herd immunity) in ones life time. That is the same philosophy that should be applied to covid 19. Of course, if you have a comprised immune system, such as diabetes , obesity, cancer, or some metabolic syndrome, should protect yourself, but at the same time work on building your immune system up, giving one a quality of life. great lecture, thank you:)

Can I ask if a negative test could be a false negative. I was tested 3 day's ago and it came out negative. I do have lung issues but I have been feeling sick I have a low grade fever I'm not so sure if that negative test was false or not.

In my country of The Netherlands we are preparing for, and already living in, the 1.5 meter society. We currently have a reproduction number of around or below 1. If I understood you correctly, assuming the new measures my government will take will bring the reproduction number back up. If this would bring the r0 back to 1.5, when using your formula this would mean only around 33% of the population will need to become immune. Is this feasable do you think?

What happens after lockdown is lifted is a big question which nobody seems to be able to answer with any degree of confidence! How to deal with the long intervening period between lifting of lockdown (starting May 2020) to mass vaccination (April 2021) is a matter of concern... Dr Mobeen... can you throw some further light on prophylaxis including:- (1) Primary prophylaxis that tries to stop healthy people from getting sick. This could includes Vitamin therapy, etc, etc? (2) Secondary prophylaxis that tries to stop people who are sick from getting worse. This includes treatment of using HCQ, Remdesivir and other Drugs?

An epidemiologist in india proposad a way as well. His method of achieving herd immunity is where we can send young people ages ranging 21-39 let's say. So as to have less load on the medical systems as they have more inbuilt immunity. Pleas discuss this idea as well.

Tough to measure the rate of spread for this because we were drastically increasing our testing capacity, during the early days. More testing will find more cases, but we cannot separate real spreading, from just more detection.

Apparently frontline healthcare workers are doing a good job with ppe. However, I came across an article about a study at Fudan University in Shanghai www.the-scientist.com/news-opinion/what-do-antibody-tests-for-sars-cov-2-tell-us-about-immunity--67425 that found that in a small amount of recovered patients, antibodies were not detected. I was wondering what your thoughts would be on this. Also, as my son is blood type O, I wondered if anti-A antibodies could suppress the virus enough that there could be less virus specific antibodies produced that might now show up on tests. I think we are making a mistake to limit our options We know that R0 is significantly modulated by social factors... so we can change the r0 by behavioral modifications. For example.... what if everyone wears a mask in public and there are temperature checks before entering work, transit, and stores. This would reduce the r0 of infected persons while at the same time allowing a more normal lifestyle

What about controlled herd immunity? People could arrange to voluntarily acquire the virus under medical supervision in controlled numbers so that eventually we would all acquire immunity. This would be an alternative to waiting for the second wave which could be a tsunami. Vaccines are a long way off and may not be as effective as naturally acquired immunity. What do you think? Tim.

Using infection must be done a controlled way (using inoculation in closed social systems) to avoid crashing the healthcare system. I would go for that. Knut is kind of right, but I think we would be better off controlling WHO gets infected. In this case children are the best to start. LOW RISK and they are society's DEPENDENTS.

The R0 is not a fixed number, so without NFI R0 is lets say 3, with Lockdown R0 is below 1, hence if all would wear masks with social distancing 2mtr without lockdown the r0 would be around 1.5 (imo) so you would need a herd immunity of around 33%. and for those who get sick yes self isolate and prophylaxis would be the way forward until we have a proper vaccination (although it still has to be seen if vaccination will work with corona viruses and likewise herd immunity as we dont know how long immunity lasts)

Lock down those at greatest risk, everyone business as usual. Normally we quarantine those who are sick, but we can't do this, why not quarantine those at greatest risk maybe even give them the prophylactics'. Allow the other 85% to create heard immunity, can give them the prophylactics too?

Targeted approach? Isolate those who are most vunerable and arm the rest with prophilaxys to greatly lessen the impact from the virua .Rather than this draconian lockdown policy that we may stuck in a lockdown yoyo measures and if second wave come and theres not enough herd immunity in thepopulation with no readily vaccine we are screwed? I don't think that will work in the USA. We can only advise the high risk group to observe isolation as much as possible. Other than that we have the same idea

Providing the ‘slow immunity method’ to certain sectors such as the medical community or essential workers [while taking into consideration data collected re:blood types etc.before method is administered due to propensity of severity of the virus symptoms] could prove a good 1st step to getting societies back on course to better functioning. With those people potentially protected [time needed to observe results of this therapy] if method proves successful, it could then be an option for other groups - teachers, postal workers, etc. I suggest segments of society and gradually we get back to a normal.
