This video shows how to safely doff (or remove) the Personal Protective Equipment (PPE) for Aerosol Generating Procedures (AGPs), specific to COVID-19. This guidance outlines infection control for healthcare providers assessing possible cases of COVID-19.

So many staff without PPE. Public Health England are a disgrace for not preparing properly for Covid-19. PPE is still not getting through to many. Theyve had since January to prepare. NHS managers are now threatening staff that speak out. Health staff are dying and PHE are trying to cover up their own incompetence. We criticise China for censuring its staff. The NHS is even worse than China for this. Criminal.

This video makes it sound nice and safe, but in the real world we are asked to disinfect and reuse disposable PPE. Of course we self-contaminate, if instead of binning contaminated disposable PPE we are asked to clean them and put them aside for future use! There is NO SAFE WAY of doffing, when you must disinfect and put aside disposable PPE for further use.

I disagree with this order. Its more logical to take off the gloves first, only if you wear double gloves. Then remove the gown, then the second gloves, then use the alcohol solution. In my opinion its wrong to open the door without using an alcohol solution, the door knob is contaminated in this way.

LULABY1975 she opened the door handle but then she washed with alcohol gel when she came out the room? Even though you used alcohol jel before touching the door handle, you are also contaminated so need to clean hand again after coming out the room!

Contaminated gloves always come off first as they are the main risk for cross contamination. The use of hand sanitizer between every step is recommended and wash hands at the end of the procedure.

I am shocked to see the potential contamination of hands during step 2 not immediately being reduced by alcohol rubbing hands, after which the lady walks to the door and contaminates the handle...this needs to be amended please!

After washing hands, better close the taps using paper towels, hence not having any direct skin contact with 'contaminated' taps.

We use double gloves. Thats better, so we dont tuoch the contaminated surfaces after the step 1.

I work as a newly trained phlebotomist, we have been told by our management that we can carry out venepuncture from confirmed and suspected cases of Covid 19 wearing only plastic aprons, face masks and gloves. We have had no proper training in PPE at all, only a five minute discussion with one of the nurses on the ward. After dealing with these patients we then go on to take blood from patients around the hospital and in the outpatients department. From watching this video I realise we are risking our own health and that of others. Having seen what’s happening in Italy and realising that we are only a few weeks away from this situation I’m worried that the our Trust isn’t taking this situation as seriously as it should be. Thank you for this video, I will pass the information on to my team I will be taking it up again with the management to ensure everyone’s safety.

David Kotwinski almost got it right! 1. remove the visor as this is contaminated with the gloves on. Hold it on the face shield part and do not touch your hair or skin. 2. untie or rip the ties - as many surgeons do - and then take the disposable gown off OVER your gloves, so only contaminated parts of the gown get in contact with the contaminated parts of the glove. 3. remove your gloves and make sure the dirty side of hand 1 touches the outside glove of hand 2 and removes it and the hand 2 slip under the inside of the glove on hand 1 and pulls this off and you hold onto the inside of your glove. Then use gel or soap and water. Ideally there are electronic contacts in wall and floor for water and door control..... Not in the NHS though, so you will get contaminated when you clean yourself and leave the room.

I'm a student nurse, currently on placement on a medical ward. We have a number of patients who have possible coronavirus. Staff working that end of the ward get changed into disposable scrubs and are told to wear a surgical mask, short gloves and white pinny, our arms and face/eyes are not covered. We have been told to take pinny an gloves off in the room and keep the same mask on for 4 hours otherwise they will run out, domestic staff were told they didnt need a mask because they do not deal with the patients directly. When I worked that end of the ward I was told we were not allowed to go in to the other bays with none infected patients. Yesterday the staff working that end of the ward were in an out of all the rooms and there was even people wearing the disposable scrubs wondering around the hospital an sitting in the canteen after being in close contact with patients who might have the coronavirus. It seems nobody is on the same page and nobody has a clue what they are doing which is a worrying thought. All hospitals should be following the same procedures and guidelines and all staff should be trained and treated the same regardless of role.

Suzanne Loveridge I think it will take new nurses less time than nurses who have been there for many many years as they have been in the cycle of washing to wrist only for longer. Whereas it is relatively new information for new nurses so less of a habit to break. The amount of nurses I see regularly that do not wash their hands appropriately is appalling and no surprise hospital acquired infections are still so high.

Could a patient bed area be used maybe tented off? Are the contaminated gowns and visors reused after cleaning or just disposed of?

As a firefighter we double glove to hazmat/radiological incidents. The woman in the video touches the gown as she draws arms out of sleeves, then visor, then door handle.

Removing the gown in this way contaminates hands (sleeves are turned inside out with 'outside' touching uncovered hands) - suggest either double gloving or an extra alcohol gel step BEFORE using those hands on the face to remove mask/visor!

Important while removing glove to take from the outside of first glove then from the inside with second glove while holding first glove in a ball with dirty second glove so it ends up inside that glove!!!

At 1:43 the advice is to undo neck tie then waist. At 2:07 Cecilia did it the other way around, potentially contaminating the back of her neck having touched the waist tie. It's so easy to get it wrong. As for the buddy, one hopes that one is always available to take your glasses off and put them back on when you're ready

There weren't any neck "ties". She undid the velcro binding the neck and then undid the waist tie.

Doesn’t seem logical for us to remove gloves before removing gown. She obviously came into contact with the outside of the sleeves as she pulled them over her bare hands. I have to do my first shift tomorrow in an emergency dental clinic where we will be performing aerosol generating procedures and I would expect the sleeves of my gown to be highly contaminated. Double gloving and removing the inner pair along with the gown would seem the best option. Any opinions out there?

Most people bend their head down when they take their face shield off and touch their scrubs with the face shield. A buddy will prevent that but why not remove it before the gown? Also, your mask/N95 should not get contaminated if you are wearing a shield..Something to think about if you are running out of N95s

The nurse contaminated her hands with the outer part of the gown when doffing the gown(its hard not to, using these steps. The contaminated hands were used to doff the mask(she might have touched her head ). This video needs to be review.

I think the NHS should invest in sensor hand gel/rub/soap and faucet. Is the hand gel/rub good for one person use only?

You touch it with contaminated hands then the gel disinfects them so it is logical that the bottle doesn't need to be 'clean'.

Surely the gloves should come off the last. With gloves on, you pull and untie the gown rope and the top. holding one gown arm from outside to take off, the glove on that hand will come off the gown, then clear hand can hold inside of the gown to get it off another arm.

Once gown removed hands should be washed again every time you remove a piece of PPE wash your hands. or double glove, lastly hair cover

Very last moment hands are washed by using the potentially contaminated hands to open the tap, which is then closed with the elbows thereby contaminating them. Should taps not be opened/closed by elbows (akin to when surgical scrubbing) or by the buddy?

This is dreadful. She touches the outside of her apron with her bare L hand at 2min15 then drags the outside of her sleeves over both hands as she turns the sleeves inside out and removes them. Next she takes off her visor with her contaminated hands, touching her hair and exits using the door handle before cleaning her hands. It is dangerous for this to be used for instruction

Disinfected her hands THEN touched contaminated gown straps, then touched her head...wrong. THEN washed her hands at the end and turned the water off with the back of her hands then dried. Again wrong. Dry your hands then turn the faucet off with the dirty towels, not your hands.

What PPE??? I work for pennine care in mental health and we have no PPE and have been told we have to enter a covid19 present ward to help out when our ward is clear of it??? Same old story STAFF DONT MATTER!!!

Always wear double gloves please. Doffing the only single pair of gloves liable to give you exposure.

This video does not apply to real-life situations in hospitals nor care homes, where there is no 'doffing area' between the room doors and the corridor, with washbasins in. The order of doffing is questionable, as it results in contamination of hands from outside of gown, and then touching the doors, and then contamination of head while removing face-mask; and the buddy that hald the glasses, if following same procedure, did so with contaminated hands. When hands have been washed, they should be held up/out front, not down by sides (basic surgical training from 30 years ago), and taps should be activated by elbows alone, or paper towels, or feet/sensors. I think a much better sequence and video could have been shown. Ask IPC how to do it.

Meanwhile, back in the real world of Care Homes with 2 staff to feed and wash 12+ residents who are all high dependency. I have no idea why my agency is insisting its staff watch this video - talk about setting people up to fail. All this will do is give the moaners a good excuse to moan even more and the rest of us a good laugh. If care home staff farted around like this even more of our clients would expire of dehydration than they are now. I think a reality check is in order for the numpty who thinks it's a good idea for care home staff to watch this - obviously conditions are vastly different in the NHS - no wonder waiting lists are getting longer by the day and despite all this most people will get infected in hospital.
