I added a vitamin c serum and have been more diligent about wearing sunscreen to the melasma on my face, and great things started to happen. The melasma is no longer darkening. It is now fading. I can't thank you enough for all of the new products to try for oily skin. Have a great day.

SE Asian ethnic skin. Fitzpatrick type 4. Diagnosed with melasma by a board-certified dermatologist 3 months ago despite going to 4 separate med spas in the past (PAs/nurses/esthetician’s). It’s so important to receive the appropriate diagnosis by a dermatologist: I have seen more strides in my treatment protocol for the last three months than I have in the last 4-5 years of having this condition. I wish I had seen a derm a long time ago. Luckily, my case was mild and it was due to the fact that I had a strict routine aimed at addressing hyperpigmentation in general. Once I became aware it was specifically melasma, we knew to go slow, gentle but more frequent in regards to in-office treatments. We’d rather do 4 sessions of gentle Clear & Brilliant or Picoway laser treatments versus 1-2 strong Fraxel laser treatments etc. You’re in it for the long-haul. I’m also prescribed hydroquinone and oral tranexamic acid tablets and recommended to use a tranexamic acid serum like Lytera 2.0.

Me too. SE asian & malasma. The picosure laser didn't remove malasma but helped with tighten the skin. I'm using rentin A & hydroquinone for 2 yeats years now. Im almost to wear ai don't have to wear makeup.

I'm have dark skin and was told by many dermatologist to just manage the malasma, there is no cure. Had fraxel lazer treatment by an excellent dermatologist in South Africa, with very impressive results even after first treatment.... Had 3 treatments over a period of about 3yrs. I now feel comfortable being without makeup

Don’t treat your entire/whole face with retin a/retinol avoid very close to the eyes- can cause chronic dry eyes, don’t apply on the side/ grooves of your alar/ nostrils can cause/aggrevate Telangiectasia, don’t apply very close around the mouth - can crack skin and cause inflammation - hyperpigmentation, sometimes you should also avoid chin if you are sensitive in that area - telangiectasia…..

We don't have much options as per your suggestions but it helps to look out for these ingredients. Youre right you cant use peels and lasers when in accutane. I had the same issue. My melasma lightened whwn i was taking accutane but when i stop, it slowly came back. Anyway, for how long did you take accutane and did the oiliness came back?

Hi Dr. Sugai! It's so nice to find your video about melasma! I'm of mixed ethnicities, including asian & latino ethnicities. I was born and raised in Hawaii, worshiping the sun! I've been noticing hyperpigmentation for a few years now, closer to the sides of my face, but the melasma on my cheeks is new and very troublesome to me. I'm in Southern California now, and my Dermatologist has me on the ZO line for my hyperpigmentation/melasma. I'm using the brightening pigment control cream with hydroquinone during the day, and the 1% retinol skin brightener at night. After more than 6 months, my skin is much smoother, but I'm really not seeing much of a difference in my hyperpigmentation/melasma, which is my main concern. I'm wondering if this is not the most effective regimen for me as an asian woman. I've invested more than I ever have in this skin care so far, so I feel the need to finish the bottles, but I do want to find products that will actually work well on me for the long run. I'm 1/2 way through your video, and I noticed you have not included ZO in your recommendations...ugh! From asian to asian, I would REALLY love your input on what I've been doing so far. I've already ordered the DRMTLGY Universal Sunscreen 46 SPF, per your recommendation. So happy to have that! I've been dying to find the right dewey tinted moisturizer with SPF, but I've been going in circles until now. So excited to try it!! Do you also happen to have any recommendations for a dermatologist in San Diego that specializes in aging asian skin?

You should watch this video (the one above that you just commented on 6:45) or any video that explains how to use hydroquinone correctly. You're not supposed to use it for more than 2 - 3 months at a time. Most hydroquinone products will have written on their packaging to use for 60 days and then take a 30 day rest period. Definitely don't use it for 6 months at a time. Extended hyrdroquinone use can cause permanent discoloration ("banana bodies").

Important ingredient question: what do you think about DMAE for skin firming (especially neck) in skincare? I’m 43 and do have hormonal melasma. I didn’t get it during pregnancy, but a few years after. I also have endometriosis and wonder if it’s related. I do all of the things you recommend and have been talked out of laser treatments. Great advice! Unfortunately , it’s a daily maintenance…no quick or permanent fixes “magic bullets” as you say.

Thank you for this video! I have dealt with melasma for years. I tried to go without hydroquinone for awhile, but Cyspera only went so far, AND it was so expensive! Oral tranexamic acid works REALLY well, but my US dermatologist will not prescribe it to me. He says it’s too risky. I see a derm in Mexico too, but the tranexamic acid supply in Mex was nonexistent for awhile, so I’m off of it now. Topical tranexamic doesn’t work as well, but I use it faithfully morning and night (Naturium serum) after vitamin C (Dr. Brenner from Amazon). I don’t see tons of results, but I’m keeping the routine. I’m back on hydroquinone mixture 6% for now from my US derm, and it REALLY works well-until I cycle off it-which will be in another month. Doctor swears if I’m faithful to Heliocare 3x a day and sunblock, my melasma won’t come back… we’ll see. My melasma is best when I get a lot of sleep and have less stress (so that’s during the summer for me because I’m a teacher). If only I could live a stress free life and sleep in everyday! 🤣 Thanks again!

Oral tranexamic acid works amazing for me but keep in mind I’m not on birth control (contraindication), do not have a family history of blood clothes, and my dermatologist is aware that I eat healthy and exercise 4-5x/week. If you live a healthy lifestyle and see the dermatologist regularly (which I do for a series of laser treatments), you’re more likely to get a derm that feels comfortable prescribing it to you.

Can u try beauty series oil. Mix 50 ml of it with 200ml of grape seed oil or sweet almond oil or rose hip oil. it did work for someone I know.

I am using Vitamin C serum 15% Cos De Baha at morning now and hydroquinone 2% + tretinoid 0.125 % at night. Hope really work it in my pigmentation face...

Aloha, Dr. Sugai, where did you move to? Nice to see content from people from Hawaii! I grew up in HI but moved to NYC for 14 years and moved back recently… the sun is harsh here so I protect my skin like an Asian aunty. I get compliments all the time on how fine my skin is for “living in Hawaii.”

I have a lot of post inflammation blemishes from acne and few freckles ...I'm using a lot of products right now ... retinol, vitamin c serum, Hyaluronic acid, arbutin, kojic acid ...I'm hoping they minimise after at least 3 months. I'm also using salycilic acid cleanser and cream to deal with acne.

I have read that vit C makes skin very photosensitive, so isn't the vit C that you are using is the couse for your hyperpigmentation. Do you use sunscreen religiously?

Hi Dr. Daniel, thank you so much for the great information. I’m about to start my Agency Retin A formula accompanied with a hydroquinone with alternating times of taking breaks in between for some hyperpigmentation. I use dermatology’s tinted sunscreen religiously but didn’t know that it’s best to used a tinted one so thank you for a teachable moment for me. I follow another derm on KZfaq who is just awesome as I have learned from her through the period of years so I feel as though you are in that same category with your skills and making it so simple to understand so I’m becoming a new subscriber; looking forward to learning more.

I used to have melasma for years, I visited a dermatologist in normen Oklahoma. She prescribed me hydroquinone 8%, retinoic acid 0.05% and hydrocortisone 1% mixed in tube. I being using it for last 3 weeks my melasma is completely going. Thank you Jesus

Hi, so nice to hear it this subject bc I'm 37, had my son and through the last of my pregnancy I could really tell my face was severely discolored in spots. I didn't really understand the severity of it until probably 6 months into it and after I have my son, it kinda settle down a little bit. I ended up going and having a few micro needling facials done and the lady stated that chemical peels in between a couple microneedling sessions will really do a good turnover effect and resurfacing effect how do you feel about that? It's just awful I hate it and I'm way too pretty to have these spots. If you can help me out please do. Love hearing your messages and watching it wasn't too bad either! Have a great day!

I just started tretinoin gel 0.01 approximately 1 month ago. I haven't noticed any changes yet from that, but I know it takes time. I suffer from malasma around my temples and upper forehead from years of sun damage. I'm in my mid-thirties, and trying to minimize the damage I caused in my twenties. I will be having a chemical peel in January. I also use the azalic acid you mentioned. Niacinamide has also been my go to. Thank you for the great review and suggestions!!!

Great video! My 27 year old sister’s facial skin has darkened, a dermatologist diagnosed it as MACULAR AMYLOIDS…and said that there is no treatment for it. My sister has been extremely distressed for last 3 years, in a depressive state. Please share any info that can help. Please please please… please help… Thank you

Hi Doc! Very informative video! I've been fighting melasma SINCE I started medication for hypothyroidism. When you spoke about tyrosinase inhibitors, my lightbulb came on!! Don't know if it has anything to do with it, but something I have suspected. The name of my medication is Tirosint! Tirosint...tyrosinase ???? Definitely going to discuss with my derm! Thank you!

Great video! Have you ever used Mini Melanage Peel for Melasma? I’ve used it in the past for my melasma and had a great response. I believe it uses tretinoin and hydroquinone. I am an avid sunscreen user (mineral with tint).

Nowadays, in Thailand, the fractional PICO lase is very popular. I mean, people show off their bleeding face from PICO in social media. I don't think it's proper for our skin type. 🤨

My mom is a 50 year old Asian with Melasma, & the Derm recommended her not to get laser/ chemical peel or any kind of procedure done for her Melasma due to her being Asian and Asian skin doesn’t ever react well so she Says…. Other than products, What do you recommend for Asian skin Dr?

I struggle with melasma on my cheeks and it is really a pain in the butt..!!! 🤷‍♀️ I am constantly using treatments for both day & night. I'm currently using Tranexamic acid from The Inky List at pm, and Azelaic Emulsion from Naturium at am. I use spf from Dermatology, both chemical and physical. The prods are excellents..! Melasma sucks but sadly, it's something, those who have it, like me, have to learn to deal with it to keep it controled.

There’s alotta talk in this video & others that I’ve watched about melanin prevention… & I’m a light skinned black woman who in no way wants to stop the production of melanin in my skin… I hate being light skinned it’s so unattractive to me … so I’m glad I’ve come across this video & others who speak on which products to use to prevent melanin production… I have some melasma on my face/cheeks that I do want to gt rid of so I gotta keep researching on how to gt if it but that’s the only darkness on my skin that I’m dissatisfied with…

Dr I have a very serious concern regarding tretinoin...i started tretinoin 3 years ago in hopes to help my acne and hyperpigmentation...after using it for the 1 st month i saw i developed melasma like hyperpigmentation in my forehead and contour region (the skin area between the ears and cheeks) ... I never ever in my life had those melasma like thing before ... Its been three years and that shit is constant... I am a 100% sure i got it from tretinoin because when i stopped using it for like 1.5 months i saw significant decrease in that hyperpigmentation but when I restarted it( very very slowly i started with just a tiiiiiny rice grain amount, wore sunscreen religiously) i got those melasma like hyperpigmentation all over again.... I'm so depressed about it.. it's been 3 years n i really cant go around waiting for my skin to "fix"... No progress whatsoever...what should I do? Stop it forever?? It's a glorious product but that really deep brown hyperpigmentation is bugging me

Over a decade ago I started getting more noticeable dark spots on both of my checks, so I had laser procedures done and now I regret because I have now a permanent hyperpigmentation caused by laser burns and melasma also all over my face. My dark spots in my checks are bluish gray color. I have used every high end to cheap products and nothing seemed to help get rid of my spots 😢 my current skincare routine is vitamin c in the morning and moisturizer with SF 50 at night I used the Soosul Korean fabulous five set and incorporated and/or I altrenate tretinoin with 10% Azelaic acid from the ordinary one night then the next night I’ll used the naturium niacinemide then tranexamic acid. I see some lightening of my face but those dark spots on my cheeks doesn’t seemed to fade. Wearing foundation doesn’t cover my dark spots. 😢 What advice you might to recommend as I would love not to wear foundation and have a clear face. Thank you in advance.

I’m a Pacific Islander, and tan very easy. I use sunscreen but still suffer from melasma. Both my cheeks have it pretty bad. Not sure if it started with sun damage or hormonal. Plus my mom and grandma have it so it’s all of the above I guess. A derm doctor prescribed hydroquinone. Yes it helped fade but once I was done with the cream it came right back. And I’m loyal to my sunscreen. I just want a good regimen for skincare, and possibly something to help fade it but without hydroquinone… it’s horrible. ☹️I too lived in Hawaii for 6 year🤷🏻‍♀️

Do Aerolase 4-6 sessions, it’s the only way. We were on the same situation but it’s all good now.

Tri-luma worked for me. I had dark patch on both cheek and in a month it lightened 90%. My dermatologist recommended it.

You loose your femininity when you get melasma, I don’t even use makeup, there’s no makeup that covers my upper lip melasma , no matter the price or brand of the foundation i still look like I have a mustache
