Join me for my first call shift as an attending physician! This shift is called a "nocturnist shift" because I stay in the hospital and look after 150 patients admitted to the internal medicine service overnight. It may not look too different from my previous call shifts, but this time I don't have a supervisor to review with... I'm an independent physician!
Learn about subarachnoid hemorrhages and heart attacks, while seeing behind the scenes in the hospital at night.

As a patient you don’t see the behind the scenes work the doctors put in - people think they’re just not coming to see you because they can’t be bothered. I wish everyone could see this video to see there are so many things going on, so many other patients and some are more urgent, if you’re not getting seen immediately that’s a good thing because it means you can wait whereas someone else can’t! I’m amazed by how Siobhan does it all with a smile on her face, I definitely couldn’t stay so upbeat if I was that busy!

And this is your first shift ... what a crazy one. It's not difficult to see why SO many Doctors and Medical Staff are suffering burn-out. Your dedication, intelligence and enthusiasm are amazing. Just don't forget to take care of YOU.

Doctors and nurses are beyond essential. You guys are human beings, and won't ever be perfect. As a cancer survivor, I've learned many things that I feel medical personnel should do differently. The thing that is most healing for patients is to treat them like human beings, whether they're there for something minor like a broken bone or something massively major.

How many patients did you end up seeing? I was out of breath watching you hustle. I would feel so relieved to have a physician like you taking care of me or one of my loved ones.

Being responsible for 150 patients and covering 5 pagers is exessive. Was this happening in the "before" times too or is this due to the shortage of medical staff we're experiencing in Ontario now? This is really scary. You are an exceptional doctor but doing this on all calls can only lead to burnout.

It’s normal for internal medicine doc to cover everyone in the facility. Most docs though only do phone though and not physically in the building. I was an RN for 20 years. Most of my docs trusted me and knew I would not call them overnight unless it was something I could not handle.

@@CTA-- wow! Nurses make no where near what a physician makes! At the hospital I worked at we had on call physicians and docs in the ER. Teaching facilities like this one has residents and an internist at all times. But at my facility, I had my 10 patients to myself overnight, but because I was senior staff (the most experienced) of the grouping, I had to get report on 40 patients just to be aware of “potential” issues that could arise.

There is no shortage of nurses in the US, but the public is being fed that line, what is happening is the hospitals are increasing the nurse/patient ratios and forcing nurses to stay at home (use PTO or take time off without pay). This is why patient care is so bad and patients are dying. Not sure if Canada is in the same boat but thought it's important to inform the public the media is lying about the nursing shortage too.

This was a very refreshing vid. Many MDs out there with similar “a day/ night at…” approaches; nevertheless, the fact that you discussed medical cases and interventions, while maintaining patient privacy was very good. this is a good time to remind everyone to keep an up to date living will. you never know when you’ll be too sick/incapacitated to make your own decisions

And make sure to carefully read the living will created for you by your lawyer. I asked my lawyer to specify that I don't want to be kept alive, and the document he created specified that I Wanted everything to be done to keep me alive.

@@snail7519 in the states graduating from medical school and passing your boards make you a physician. Is it not the same in Canada? You're an MD all through residency meaning she has been a physician in TRAINING for many years now. Still a physician. This is just curiosity on my part.

I work as an overnight CT Tech in the ED, I LOVE watching her videos and getting to see the entire picture of the process, especially when it involves patients needing CT scans 😜 Still so educational and fun to learn about!

I remember the day when I was browsing in you tube day in the life of a doctor and got to see ur day in the life hematology rotation, and that was 3 years back and now when I see your transition from being a resident to finally a specialised doctor what should I feel other than being proud... So nice to see you working at a hospital I'm very familiar with. I had surgery there in April and it doesn't surprise me that you were in charge of so many patients. Sadly, due to the pandemic, we are severely unstaffed at our hospitals here (and probably everywhere to be honest). You did a great job keeping up with things!! I've been watching you for a while, and I'm looking forward to more videos from you as a full-fledged doctor!

You're such a wonderful doctor!!❤️I lost my aunt 4 years ago from SAH😭She experienced headache and ringing in the ear but thought it would go away on its own. Then she was rushed to the A&E. Every second in an emergency matters, but the doctor that spoke to my family was very disrespectful. She told us that the CT machine nearest to the A&E is under maintenance and they would have to rush her to another CT machine but it's at another part of the hospital and she wouldn't make it... My family was begging the doctor to just rush her to the nearest available CT machine... Instead of trying her best to save her patient, she wasted time explaining why my aunt wouldn't make it. It still makes me mad and breaks my heart talking about it💔She was warded in Neuro HDU and on life support for 2 days... I held her hand and prayed with her and tears rolled down her cheeks... She left us forever😭My family was mad at the A&E doctor and wanted to report that doctor. But the neuro head came down and spoke to my family and apologised to us. My aunt was a very nice person so we just let it go because we knew she wouldn't want us reporting the doctor. Sadly no apologies can bring her back to us and we miss her very much everyday😭

I love watching your channel. I've worked at a critical access hospital for 7 years now. Started in ER admissions but moved into a different position in admin office but still do an entire weekend on call in ER a couple times a month. You are correct in that transfers do take a bit of time (our nearest hospital which happens to be a level 2 trauma center is 25 min flight by heli, hour & half by ambulance). Keep up the good work & showing what actually goes on.
