Join me for a day in the life of a doctor! I'm a 3rd year internal medicine resident on a respirology rotation (specialty of the lungs). You will get to meet the whole medical team and see how we work together throughout the day. We will be seeing new patients in the emergency department, following up with patients on the ward and doing a procedure to drain fluid from the pleural space, called a thoracentesis.

I'm 33. I started smoking when I was 14. In 2009 at the age of 22, I found out I was pregnant with my son and quit that day...cold turkey! Best day of my life for so many reasons. So, February 2020 I will be 11 years cigarette free!

Completely agree. I work in an ER and I have found many of the younger residents and doctors are far more like Siobhan than many of the older docs. They are a breath of fresh air and the team works so much better.

@Inferiorita nera Sukkks BWC Thank you! I didn't want to say it but you are absolutely right. My hospital is understaffed with many residents performing under necessary expectations. It increases my workload when I'm in the ER.

@Inferiorita nera Sukkks BWC There is no doubt the older docs have better clinical skills, but that is the value of experience. I was referring more towards the general attitude with other staff, including openness to suggestion and a collaborative mindset.

Me too! I have Sjogren's and I'm so excited. She's a great doctor and would be wonderful anywhere but it feels like she's joining the "family" of people with autoimmune conditions.

YES! People should really try to stop smoking ! I remember my days working on respiratory and the high number of COPD patient's I saw, the number of ABG's i had to do etc, it is such a difficult disease to live with.

Jennifer Espino if you saw patients with COPD and end stage respiratory failure due to smoking you would quickly stop, absolutely horrible way to die

I know someone who loves and is so proud that she smokes. Says it's her "thing" and will never give it up. I'll never understand that.

Shelby Zealand my ex bestfriend says that she would never quit it because she wants to die early. She’s only 15 and smokes 2 packs a day. Edit: to avoid any confusion, in lebanon you will never be chastised for underage smoking/drinking.

Yo, what?! Of any reason I've heard, that is the dumbest. Smoking at 15 to die quickly? Who ever invented that? xD La Reina, if you ever meet that ex-friend again, tell her to stop being silly. And to quit smoking, as it's just a waste of time in general. I don't know how much a pack of smokes costs in Lebanon, but if she smokes two a day, she can give that money to poor people who really need it or she can just send it to me. Oh, and Shelby Zealand, tell her to make quitting smoking her "thing".

As long as she respects non-smokers around her, I don't know what the problem is. Of course, it's likely that she will become ill at some point and impose herself on the medical fraternity, and will become more and more ill if she continues to smoke and likely die as a result. But its a metaphysical thing, isn't it? It's your life for what you want to do with it, and as long you don't abuse the love and goodwill of others, then go ahead.

Sure it does. It's the week between summer and winter. 😉 Actually I'm on the West coast of Canada, we don't have winter. Just a more rainy season with the odd snowflake thrown in for variety.

You're likely joking, but Vancouver/West Coast has a normal length autumn and a wussy winter. Snow is news-worthy here since we only see it for like 10 days/year and it melts in 24 hours lol.

I wish healthcare professionals would remember that not all COPD is self inflicted. My Mother was given a COPD diagnosis after her lungs were damaged by infection yet was openly told that her treatment is inferior to that given to someone with something like asthma by the specialist nurse. She’s never smoked yet repeatedly gets asked if she’s quit. It’s so frustrating. Thanks for the video, I hope preparations for your exam are going well

I agree with you my adoptive father has COPD and never smoked! I think some of it is cused environmental issues as-well.

Yes!! I was hoping this would be mentioned. It’s kind of like diabetes - people hear that word and they think “self inflicted”. I love it whenever a medical professional takes a moment to dispel misconceptions like that. (Siobhan does it often, which is one of things I love about her channel.)

Mark Weatherall I was just about to mention A-1! When I saw this episode (only half way through it now), I was wondering if A-1 will be mentioned. I don’t drink and never smoked, and always got told off by medical staff...

The reason alpha-1-antitrypsin (AAT) deficiency isn't mentioned is because it is the cause in only 1% of people with COPD. It's a genetic condition that's rare. The AAT protein is low thereby not protecting the patient's lungs. The majority of people with COPD are smokers and it's why medical professionals do tend to harp on eliminating smoking. I'm sorry some of your family members were treated poorly. Although the above is true, it's no excuse. Doctor's need to read someone's full history rather than making assumptions based off of the majority cause.

right i believe 10% of COPD is due to external factors like second hand, work related (firefighters, textile factory), A-1 too

I appreciate the respiratory doctors because they do so much to help patients breathe and feel better. As an asthma sufferer who takes a controller med and has a rescue inhaler, I can't help but be appreciative for the pulmonary docs who not only diagnosed me but helped me in my journey to breathe easier. I’m currently a medical lab scientist student so this was definitely an interesting point of view. I’m used to getting respiratory cultures but it’s nice to see how important those tests are to ensure that the patient gets the best care.

This is so cool! Im studying medicine as well, (Naturopathic medicine) and we’re currently learning about dyspnea/ lung pathologies and diseases in our clinical medicine course. Hi Siobhan, I love your energy. I dropped out off nursing school 20 years ago because my anxiety was extremely high. I would have panic attacks. I’m back in now and loving it. I have a different outlook and love the practical and learning experience. I’ve just had to get marked off for putting the sterile gloves on. We also have dines obs and we have just gone through basic lung functions and abnormalities. When you were talking about your patient with COPD I really could picture her leaning forward, trying hard to get oxygen and I have an idea why. It’s exciting. I go on my first hospital prac in 2 weeks and I’m looking forward to it. 20 years ago I would have been dreading it. I’m really grateful to you and everyone else who make videos and taking up valuable time to provide me with information about medicine and hospitals. Thank you.

I am a registered nurse who just started working in an medical ICU and before this I had zero hospital experience (I worked in a skilled nursing facility after I graduated). I found your videos very helpful in helping me getting familiar with all those procedure I need to help doctors to do as a nurse. Yayy I've never been this early before. I'm fairly new to your channel but I have watched all of your amazing and informative videos!! Thank you for being such a caring doctor, as a cancer patient in the United States, it seems as if most of them lack that. I do have two very nice Dr's I can talk with but my other 3 specialists are not as nice and lack the empathy I feel helps me communicate with them how Im truly feeling. Since I started watching your videos I never even think to complain about any sort of waiting time especially in emergency because now I see what's going on behind the scenes, it's easy to get caught up in your own problems especially when you're hurting but it hasn't seemed to matter I always try to be as gracious as possible to the healthcare workers around me because they're put under a lot of stress and extremely long hours and their streamly knowledgeable and educated citizens just trying to do their job as best they can in a country where we offer free healthcare and therefore have very high volumes

hi Siobhan (I hope that's the right spelling) .. I just wanted to congratulate you on becoming a rheumatologist. I've been here since you were a 1st year medical resident, and all your hard work has definitely paid off. you will make an excellent rheumatologist and anyone that sees you and is treated by you, are getting one of the most compassionate and caring doctors out there. congratulations again, and best of luck on your new career. :)

My niece just finished her residency, she has decided to be a Hospitalist, she really liked it. & my other niece finished her residency in OB/GYN . I am so proud of them both. Good luck to you as well. 😊

Wow, I’m a first year nursing student and always feel like I don’t know much about anatomy and physiology. I always doubt my knowledge and my ability to be a nurse/health care professional. But it was SO refreshing to watch this video and understand mostly everything you were talking about. Hypoxia, COPD, pleura effusion, ox sats, cell count differentials. Makes me feel like when I graduate, I will actually be competent and knowledgeable enough to do my job and do it well 💕.

I loved this video I work for a Pulmonologist. I have been a Medical Assistant for 6 years and I'm going to start taking my prerequisites In January to get into the nursing program to finally be an RN It is very heartbreaking seeing the patients with COPD most of them were smokers for 40 plus years. Lots of lung cancer patients as well. I always get so scared when I take there pulse ox and the machine starts beeping because it's so low and they are gasping for air. I love your videos they are very informative. Keep up the great work!

Omg I've been waiting/hoping you would be doing a respirology rotation. I'm an aspiring Respirstory therapist, and so I loved watching/learning what the physicians role is. You're amazing and so wonderful. I hope I meet doctors like you more.

Thank you Siobhan. This is truly an important video of you about treating patients with Respiralogy Rotation. Some people with cancer get this kind of lung problems. Siobhan, you know, because of my illness motor neurone disease, I'm unable to speak louder, because doctors said my lungs are shrunk because of nervous breakdown in my body

This was was one of the most interesting videos by far for me since I am constantly seeing and taking chest x-rays it's always fascinating to see what the doctors interpret from the x-ray images we take and how they are diagnosed!

I quit smoking this summer because I had a tonsillectomy, so happy that I did not start again! Tried to quit smoking 2 times before but this time I have succeeded and feel so much better! I almost never get colds anymore, food tastes better and I don’t worry about lungcancer or COPD anymore ☺️ Very good video! ❤️

I am a recent follower to your channel and I quite enjoy it as a layman we get to see so many aspects of a residents life thank you for that I had a heart attack a few years ago so I get to be one of the chosen few to be a regular visitor to hospital for tests and such and it’s even better as it is a teaching hospital UNHBC so I also when I am there I have let it be known that if medical students or residents are there I will be their guinea pig I’m an oddball that way. Once even I was visiting a friend in Emerg and the attending was someone I knew and she asked if the students and 1st yr resident could do checkup on me was quite entertaining for me
