ABOUT THIS VIDEO: Type 2 diabetes (T2D) is a metabolic disorder characterized by insulin resistance and insufficient insulin release from pancreatic islet β (beta) cells. But is T2D a result of beta cell destruction or a decline in beta cell insulin secretion?

Dr. Brewer started as an Emergency Doctor. After seeing too many preventable heart attacks, he went to Johns Hopkins to learn Preventive Medicine. While there, he went on the run the post-graduate training program (residency) in Preventive Medicine. From there, he made a career of practicing and managing preventive medicine and primary care clinics. His later role in this area was Chief Medical Officer for Premise, which has close to 1,000 primary care/prevention clinics. He was also the Chief Medical Officer for MDLIVE, the second largest telemedicine company. More recently, he founded PrevMed, a heart attack, and stroke prevention clinic.
At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. Insulin resistant is a hypothesis they never provided evidence or proved this . There was no tools or technology available to prove this. Nowhere is there proof of this I couldn't find anything but cartoons explaining the Hypothesis of insulin resistants.

I agree with your last comment about these being preventable. I am a PA and I am frustrated because I know these things can be improved before something happens.

My doc just said the other day I would never get my pancreas function back to pre-prediabetic levels. I said I will keep trying though.

Weight loss and pio tend to actually reverse IR, and other lifestyle changes reverse the results.

Get a new dr. He/she wants to sell meds, he’s the drug pusher for the drug lords (pharma). Do LOW card and intermittent fasting and you will see results.

@@PrevMedHealth Thank you! I agree for sure. I have bones sticking out about everywhere and at easily wearing 32" waist pants with room to spare. Am still around 185-190#. I watch very carefully the low amount of carbs I eat and still a1c floats between 5.4-5.7% . 6'0" tall on a frame that had once been 6'2". I certainly don't plan on giving up, but it is a fight for these last few pounds needed. It is verifiable that I have really abnormally dense bones. My spinal Dex T-score runs 9.75 in places, and the other parts score 3-5 for some unexplained reason. Some of that extra weight really has to be bone, but surely not that much. I guess I have hit that plateau, but a much lower plateau than in the past using low carb eating. I did find out that I am not eating enough Protein as blood levels of globulin are below normal range parameters and creatinine is low as well. I know I haven't been eating enough protein. All that protein seems like a monumental task. I mever had done it on this way of eating. I need to eat beyond the require 1-2 grams of it per kilogram for awhile to gain muscle for awhile and I am concerned that will mess with my glucose levels. It seems my gl levels raise considerable even eating straight meat for whatever reason.

@@mannyradzky493 He doesn't push meds. He knows better than that with me. He knows I avoid them all I can. He is helpful. He most always says he wishes he had more patients trying to help themselves the way I have demonstrated a willingness to do. Patients can be really frustrating that way and you can see he really gets tired of it. About saying what he says about healing my pancreas, I just let it roll off me because I haven't given up yet myself. I don't cling to every word anyone else says as gospel. Life is a learning process for all of and for a dr. is no different. He is correct... but in the wrong context. I am right in my thinking that I can go back to pre-prediabetic conditions, but he is right in the context that I could never go back to eating very many carbs when I got back there. We weren't designed to.

@@mannyradzky493 I fast a lot. Probably not bringing my body out of a fasting state correctly. Every day of the week is 14-16 hours and at least 3 other days a week are 22-2. I don't get hungry at all so I always think well my blood gl levels must be fine, but still early prediabetic as they have been for years, but in the high 5's.

What if the high glucose is due to a stress response over time. Wouldn't the cells change some of their insulin receptors to limit the amount of glucose that they take up. The beta cells don't have any receptors for glucose so they can't limit what they take up. So this may be the reason they become overworked and fatigued.

Pioglitazone. I lot of people are afraid of it. It’s far less dangerous than prediabetes.

This is something I have wanted to understand. So thank you! After almost two years of Metformin and dietary restrictions, my after eating and fasting glucose levels are down. I reduced the Metformin and they are still down. I eat a bit more whole grain carbs and still down. The last doctor I went to said I don't have diabetes or prediabetes but I need to see a shrink because I shouldn't be taking Metformin(it's over the counter where I live) He based his opinion solely on the A1C. So I have been entirely dependent on your information and the information from Jenny Ruhl, and I haven't been over 130 1 hr after in so long I can't remember and fasting is 85 consistently. I feel like a genius. But of course without good information who knows how I would be doing! So from this research I can conclude that my ability to use insulin has improved... and the beta cells are relieved. Does this infer anything about how we use our muscles?

Thanks for sharing. We don’t really know everything metformin does. It stops the liver from making new glucose. And it changes the gut biome. We do know that much.

Ok, so how do we give the Beta cells a “rest” and not ask more of them than they can COMFORTABLY do? I’m thinking: exercise, very very carb restricted (50/day so when they do need to produce insulin they don’t need to produce much), fasting (long periods of NO eating so they can rest), get your TG/HDL below 1.0 to get the insulin resistance as low as you possibly can, and do these 3 things for years and years. What am I missing or what else should be in the mix? Where does inflammation and plaque fit in?

I’d be happy to take your questions. But I don’t think I understand them very well. The pancreas typically continues to work for years, struggling to make more and more insulin as the muscle & liver cell wall receptors get more and more resistant to insulin. Most never give out, but continue to struggle for years. There are no obvious symptoms (other than gain in body fat - which is rarely recognized as insulin resistance). But it results in CV inflammation and plaque. They result in turn in very bad problems, like heart attack, stroke, blindness, dementia, kidney disease, and erectile dysfunction.

@@PrevMedHealth hi doc yes this was my question 🙂🙂🙂...i wanted toknow once becomes insulin resistant and becomes diebetic how long pancreas beta cell still can work ? and 2ndly if we change our lifestyle and control our diet , exercise and other things. If all are altered can they become strong and functioning again in the diebetes reversal process? ..i heard that the more patients on medications or insulin the lesser pancreas works gradually is that true? My relative recently diagnosed with diebetes he is obese initial reading 240/329 then with diet it reduced 150/197 in one week without medication and lifestyle change is it good progress ? should it be continued ?without medicine..

@@santanuroy571 for one reversing diabetes is a myth. Your running out of beta cells which produces the insulin. If you could truly revearse diabetes you would be able to eat simple piece of bread or fruit with out spiking your blood sugar. When you have full blown diabetes your Pancreas is quitting and dying this is unfortunately the truth. If beta cells could regenerate you wouldn't have diabetes in the first place , but beta cells don't regenerate.

I can say it couldn't be too high. I had soft plaque issues nearly blocking my Left Anterior Artery Decending a year ago and had been having a1cs in the 5.7 to 6.0 range for years. It was 14 years ago, I had my highest one of 7.2%. I have never been higher. I have fought inflammatory issues in general for decades though. That can make a difference.

@@kidpoker007 Couldn't say what the current level is. It was measured only one time. My doc said it was in "good" range considering I was prediabetic a1c of 5.7 at that time, and at higher risk due to family history. Only Brother and Father both passed from cardiac events at 49 and 59 respective. Both had T2D. I can't get a doctor to work up the interest because my insurances are what they are. My last Triglyceride over HDL ratio was 1.
