🩺ABOUT DR. BREWER
Dr. Brewer started as an Emergency Doctor. After seeing too many preventable heart attacks, he went to Johns Hopkins to learn Preventive Medicine. While there, he went on to run the post-graduate training program (residency) in Preventive Medicine. From there, he made a career of practicing and managing preventive medicine and primary care clinics. His later role in this area was Chief Medical Officer for Premise, which has close to 1,000 primary care/prevention clinics. He was also the Chief Medical Officer for MDLIVE, the second largest telemedicine company. More recently, he founded PrevMed, a heart attack, and stroke prevention clinic.
At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. Dr. Brewer provides services via telemedicine. We find a lot of undiagnosed prediabetes or insulin resistance. Very gracious of you, Dr. Berry, but this raises more questions than provides answers. If I may, Dr. Brewer... your theory is that the statins, due to some anti-inflammatory mechanism, has "shrunk" your plaques. Is it not just as likely that your diet improved your overall health and wellbeing, creating an environment that reduced systemic inflammation and supported atheroschlerotic healing? Is the endothelium healed? Is the plaque, no longer inflammed, now trapped beneath the endothelium? What about calcification? Are statins the best anti-inflammatory... uspecially given their now-well-known side effects? Statins were designed and approved to lower cholesterol, which they do regardless of dose and regardless of whatever other effects they have. They unavoidably lower CoEnzyme-Q10, for instance. Are you supplementing CoEnzyme-Q10? Wouldn't it be safer to tailor a "diet + supplements + exercise" protocol that is anti-inflammatory - AND - helps your body heal the atherosclerotic lesions? Can this be done without the avoidable risk of taking a pharmaceutical?

Well, that's a start - you've got a much longer story to tell now. How why on the inflammation reduction. General principles and specific suggestions for nutrition - what's the problem with carbs in balance (if longer digestion = low Glycemic # overall?) What about EDTA and alternatives (looking that up next.) What about "claudication" of microvessels - technically a parallel CV issue but not = plaque? This last is also very important to issues of peripheral blood flow, vision, potentially risk of embolism/stroke. And thank you!

I thought it was a fair comment. He stressed the importance of weight loss in those who need it,the necessity to reduce inflammation, and the use of low dose statins. People can argue about the pros and cons of statins but even skeptics have to acknowledge that statins do reduce inflammation (to some extent) and hence the benefit of statins to in those with soft plaque. He is not talking about the use of statins in primary prevention. Lifestyle changes are the way to go and that includes regular exercise, a whole food, plant based diet ( on the whole), getting good sleep, stopping smoking, being content and minimising stress in your life. Anyway, that’s my take on the issue having done wide reading. It is of course complicated but as I have coronary heart disease and enjoy life, I have a desire to stay alive and keep the quality of life I currently enjoy.

Good food for thought. Gary Null also believes in reversing and/or curing any disease with diet or natural supplements. There is much about the body we do not know. It is a marvelous machine with very dynamic repair/regeneration potential. We have just scratched the surface of its capabilities. While I do not think all pharmaceuticals are bad, they should be uses more conservatively. Statins scare me because they damaged my father's pancreas. Maybe red yeast rice, consumed in moderation is a good alternative?

Thanks. Yes. I got admitted to med school while still in high school. I had to complete a college bachelor's degree in 2 years to remain in the program. I really wanted to take more time than I did. There were family & regional issues driving me to go quicker.

The prevention doctor I was dealing with here in British Columbia didn't know that choline is required for lipid metabolism. I tried the low dose statin and even at 5mg I get severe muscle ache and joint pain. I was up to 2.75 grams of choline(plus dietairy) in an attempt to tolerate the drugs but no joy. Plus I am MTHFR negative. My inflammation was from MTHFR, SSRI and SNRI medications. I am pretty worried about trying any more meds.

The elevated LDL and triglyceride levels are the result of chronic hyperinsulinemia, which is one of the numerous root causes of vascular endothelial inflammation, damage, subsequent clotting, breakdown of lipid rich, clot sequestered red blood cells, plaque formation, and eventual calcification of the medial arterial wall. Elevated LDL, triglyceride levels, and arterial damage are caused by chronic hyperinsulinemia and inflammation.

How do you know that you reduced plaque? Did I miss something? What did you measure, and how much did it change? And, if I followed correctly, you did this by taking a statin, reducing carb intake, and dropping 10 lbs? Over what time period?

If your statin conclusion is correct, then why do we not see the population arterial disease complications curve change the upward trend or at least flatten as statin use is prevalent in the US? As I watched you today, your body language did not seem so confident about your statin usage. As you know, this is a very complicated process, also, you yourself could have experienced your own Placebo type effect as you were taking your CIMT’s in support of your conclusion. A scientific and independent double blind process must be used as you are so well aware. You also know that low dose statins come with risks for a portion of the population, is your personal experience with statins overall a gain for the population?

Do you prescribe statins if Coronary Artery Calcium score is high? I am 65, 35 lbs overweight with high cholesterol for years, CAC of 567. Lost 20 lbs lately but 35 more to go. Exercise regularly 5-7 days per week includes weights, HIIT, basketball, biking. Mostly Keto diet for last 6 months. My Dr prescribed statin but I’m not taking.

Based on your suggestion I decided to start on 10mg Crestor daily. I eat fairly well - low fat, low carb, BMI is good although I don't exercise at all. I was kind of taking an extra 5 mg due to the fact that my activity level is pretty low. Suggestion?

Get the good fats back in your diet. Low fat is old school thinking and not healthy.

First of all, two points. a) You need exercise to achieve good health and b) There's nothing healthy about "low fat". In fact, eating "low fat" greatly raises the odds that what fats you are now eating are predominately pro-inflammatory omega-6 "unsaturated" fats with few if any healthy mono-unsaturated and saturated fats. Latest science suggests you avoid the Crestor, exercise by walking a mile every other day, eliminate (to the greatest extent possible) all omega-6 fats (processed seed oils) and add organic mono-unsaturated fats like avocado oil and virgin olive oil and saturated fats like virgin coconut oil. There is also reliable science indicating that sufficient vitamin D3 and K2 can mobilize calcium out of plaque and into bone where it is needed. PubMed is your friend.

The one thing that I see is that many doctors only talk about one supplement and don't care about anything else. Like they attack K2-MK7, by itself it doesn't do much, but when you do it right and add Nattokinase, D3, Nitric Oxide and Co-Q10 cleans out my calcified aorta and all my plaque.

Give Rem chol a try, it's a newish thing to draw the arterial plaque out. I've been on it nearly a month and results quite amazing. I am unable to take statins not good side effects. But doc suggested Cavadex- rem chol. I'll be continuing on this. At least read up on it. All the VERY BEST.🙏🙏🙏

Let it travel up😌 I used my early morning about 5:00am read in bed for about 45 then in evening. Need to let it soak in. If you know what I mean. I found it very good, and also went on the forks over knives while food plant based. Only on one med now at night. No cholesterol, blood pressure, or Aspirin. Feeling great. All the best.🙏🙏🙏

Started on this journey in May. Despite eating 100 g of net carbs or less/day, keeping total calorie intake at less than 1800, and exercising 4-5 times/week, I've not lost any weight. Strength has improved and muscles are more evident but no measurable wt loss. I track all food using FatSecret and track all exercises as well. So I have a detailed record of what I've done if it works. Next CIMT in November.

Cut your carbs to under 20 net carbs per day. Also eliminate highly processed seed oils (soy, corn, canola, safflower, etc.) from your diet.

@@brucenichols540 I experimented with IF prior to starting my current approach. I did find it worked better for me for weight loss, but also because I wasn't eating 1800 cal/day. Just ate when I was hungry and till I was full. It's hard for me to get in 170 g of protein/ day eating in an 8 hr window. Hard to even make it in 12. Still experimenting.

I don't know your weight and height, but I think you're not eating enough calories for your level of activity. I recommend you to watch videos from MindPump. Their advice is really good and always experience and science based. Good luck !

@@barbarapecka4935 Thanks for your comment, doing keto (and especially a low carb diet of less than 100 grams per day), and supplements help a lot, now one thing that I don't see listed, and please notice, this is not medical advice, in the channel you will see that there are key components where you can do something about, exercise is a must do, ideally doing high-intensity interval training, with resistance training and some aerobic, if there are some extrapounds involved losing weight is going to be a consequence of the combination of low-carb and HIIT, sleep is critical, many people neglect sleep, stress is another factor to address, medications have a role, such as statins or even vascepa as another option, and the final thing is track progress, Calcium score might not the best way to do that, just because as you stabilize your plaque you will get more calcification, ergo a higher CACscore, a CIMT is the best way to determine if you have soft unstable plaque or calcified stable plaque, then comes reversal, which we discussed is hard for any person to do but possible for someone with FH, hope this helps

So there is no way to remove cholesterol from arteries? CIMT doesn't measure cholesterol, it just measures how much plaque you have in general, including inflammation, but not cholesterol itself

He has a video, shows how to make a drink with garlic, lemon or apple cider vinegar, turmeric. Take vit k2 mk7 to remove the calcium from blood vessels and send it to bones. Dr Berg also has a video on how to remove plaque. Search Dr Berg plus the symptoms, condition you want to heal

Yes, it is plaque that bursts or breaks, but it is apparently the soft plaque. Apparently the calcified plaque is stable. It is calcified as part of the body's healing process to protect itself.

Statins work, and that's confirmed in the studies stacks deep now. Combined with life style changes, you can get reversal of soft plaques, and or, improved plaque stability. While I'm a big believer of inflammation is a key driver of CVD and other diseases, we know you don't need inflammation to initiate CVD, doc needs to read more recent papers. Elevated LDL is sufficient to initiate CVD.

What I was doing was taking inflammation out of that plaque." So, is plaque reversal a myth? Is Dr. Brewer selling hope? I'm still looking for the end of the rainbow.

You were insulin resistant and eating carbs in that state is no bueno. Seed oils are a big part of that problem because they cause local insulin resistance. And inflammation. Fuel on the fire.

IF, plus vitamin K2-MK7, eliminate all processed, so called, food. No statins required. PS: I hate to ay it but my wife who eats all processed foods at age 66 has better Triglycerides to HDL ratio than I! Her Ratio was 0.86 eating processed food, but was meticulously with counting calories and zero in-between, She even routinely eats Peanut M&M's or Milk Duds Friday, Saturday and Sunday night, meticulously measured out!!! She eats, Beef Ravioli Chef boy r dee, and various frozen dinners.I asked her to do 18-6 IF and within 6 weeks her Triglycerides to HDL ratio dropped even lower to a ratio of 0.76! I dropped mine from 2.1 down to 0.84 with IF and keto in 8 weeks. Backed up by lipids panels. So, as much as I hate to say it, you can eat processed food, but you must have extreme calorie control and regimented three meals per day.

Her mother might have eaten processed foods and sugar while pregnant with her . There is a correlation of what your mother ate while carrying you in womb . It is why we are predisposed to different foods .

Not necessarily, if the plaque gets calcified you might actually see an increase in the Calcium Score, but some people do see a reduction

Don't worry about your CAC score going up (usually does as you get healthier) as long as your progression is marked by calcification and stabilization. This is called the "CAC Paradox." My CAC score went from an abysmal 600 (with lots of dangerous soft plaque) to over 2,500 (with no soft plaque) after achieving superior metabolic health. A perfect example of the “CAC paradox.”

@@DanielTrevorOnline how can you differentiate the types of plaque on the CAC? Hubby has score 663, now 1 1/2 yrs later still 663. Which I understand is a good thing that it has not progressed. However I have no idea how to know if soft or calcified plaque.

@@juliem2538 This quote is from Cleveland Clinic (rated #1 in the world for cardiovascular care): “Calcified plaque indicates that heart disease has been present long enough for an inflamed lesion to heal and calcify. The CAC (Coronary Artery Calcium) score is like the tip of an iceberg, whereas the soft plaque is what’s under the surface. What’s really dangerous is the soft plaque, and estimates reflect an approximately four times as much soft plaque present as hard, or calcified, plaque.” The CIMT test can detect soft plaque. This is all in my book with Dr. Ford.

@@DanielTrevorOnline very much looking forward to your book. My husband eats very low carb (mostly). As a matter of fact he is too thin. Just had surgery to clear a carotid artery. But obviously a lot of damage was done already. He went through a very bad time with auto immune very high inflammation levels and never got a diagnosis about a decade ago. Not doing so well in past couple of months again. Trying to figure out the right meds, supplements, and dietary specifics to help him. He is your age. You take 300mg Niacin a day (not slow release) right? He just started on Crestor 3 x week 5 mg. He gets all the side effects pain, memory loss… has tried them all in the past
