From my experience, I’ve sprained my right ankle 5 times and left twice. I’ve iced the first 3 times after the injuries and the healing process took 3+ months. The worst took 6 months. I finally stopped icing because I felt like it wasn’t helping me so I began doing inversion therapy and heating the area. I feel more relief heating rather than icing. This dramatically increases the healing process for me. Ever since then I’ve gotten worse ankle injuries and they’ve healed much quicker when I didn’t ice.

I don't think you are telling the full story here. The aim of acute management of any soft tissue injury to minimise the amount of bleeding and swelling in to the damaged tissues. We know that the more haemorrhage and swelling into an injured area, the longer it takes to restore range of movement, start strengthening exercises and get the injured person going so they take longer to get back to normal walking, daily activities and sports. The idea of icing an acute injury is to constrict the damaged blood vessels that are leaking into the soft tissues and minimise the extent of the initial injury. The idea of resting initially is to prevent vessels that have clotted off from reopening and causing further blessing. Yes we need macrophages to come in and start removing damaged cells but they can do their job much more efficiently when there is less damaged tissue to remove! I advocate icing for 20 minutes every hour for the first few hours then every 2 to 4 hours over the first 48 hours. The reduction in pain through icing also allows for early range of movement exercises which I agree can help lymphatic drainage - so can soft tissue work and massage treatment, avoiding pressure and friction directly over the injured area initially. In summary, first minimise the severity of bleeding and swelling into damaged tissues by using ice intermittently, then let the body's inflammatory response do its work in the days after the injury.

Scott Malin I have sprained both my ankles in 2 separate incidences about 6 months apart approx 3 years ago. Because I was at work on a drilling rig and needed the job I played it down, grit my teeth and pushed thought the emmense pain and swelling to walk normally so not to have an "incident". No rest or ice. To this day I have full range of motion and functionality. I run, sprint, barbell squat and lunge 2 50kg dumbells with no pain....except when I'm not moving. I have chronic pain with a hot or cold sensation and constant ache. It's always in pain until I'm moving my ankles. Any suggestions as to the cause of this pain and any possible treatments?

You maybe right and wrong here. Ice is to get the swelling down. Once it's down then you don't need to ice anymore. Heat makes the blood flow better in the area to heal but still you need to ice it down.

I agree with a lot of what you have to say Scott! However, ice is beneficial in the first 24-48 hours when the goal is to limit swelling and limit the amount of activity to avoid damaging the tissues further. Besides slowing down nerve conduction, ice decreases cell metabolism and narrows blood vessels. The decreased metabolism means the macrophages aren't as active but if they were very active they'd be producing a lot of waste which would contribute to swelling, and swelling leads to muscle inhibition, which prevents using muscles as pumps to help the lymph system do its job. Also, by decreasing the size of the blood vessels, you limit how much extra fluid leaks into the cells, which leads to more swelling. In my experience as a licensed Athletic Trainer, the best way to heal an ankle sprain is a combination of ice for the first 2-3 days, compression wrap, elevation of the limb, and easy active ankle range or motion. More aggressive ankle rehab starting with isometrics can begin 3ish days after the sprain!

Right, I was running yesterday and rolled my left ankle. It felt pretty bad. I came home and took some anti-inflammatory stuff and also followed the RICE approach. I have been injured before and I understood, somewhat, how the body works. I figured if we want to flush out all the waste from an injured area then we need to pump fresh blood through that area and, in my opinion, accelerate the healing process. Of course we need to do low impact exercises, stationary bike etc, and to help/force the body for a quicker recovery.

my ankle has been perfectly fine for the past 8 years since I last sprain it. Iced, wrapped, elevated and exercised my ankle. it took about month or so to heal. I believe the first 3 is probably why it took so long. now I have that feeling again last few days. is that normal to just sprain your ankle after so many years. does the muscle eventually become injured inevitably or did I just not heal correctly?

Very well said. Good thing I never use ice for my sprains. I just put bandage on it and let my body do the recovery. I can walk properly after 48hrs from having a sprain. BUT, it still has a limited motion and angles. But the best word for it is, I can walk properly without people noticing I have an ankle sprain.

hi, just did something to my ankle this saturday while skateboarding. I had high top shoes on and kept them on until I got home, but have been doing ice water buckets a couple times a day and elevating. I tried to limp to stay mobile, but am not able to put pressure on i still. Is this normal? It definitely swelled up immediately and started to bruise on day 2. It's day 3 and its still swollen and I can feel it on the inner ankle when I put pressure on it. thanks in advanced!

I’m a chronic ankle sprainer as well i found consistent massage over the area helps promote healthy blood flow and clear old scar tissue in the afflicted area. I was just thinking about how the body is an expert in healing itself and how humans seem to do things that go against what it is trying to do for itself, such as ... when it is injured, it hurts, so you won't use that area as usual so it can heal ... but we take pain pills, etc., so we can use that part as usual. Same with an area swelling. We do things to take away the swelling, such as icing it, and then it can't do it's job. So I was EXTREMELY pleased to come across this video ... and now I don't feel stupid for not icing my newly sprained ankle. I also use it a little as well. Just made sense to me.

I plan to see a doctor for the ankle I just sprained today to make sure I did not fracture anything but I assume I let it be swollen. Was planing to ice it. Same happened to me few years ago when my foot did not swell. I just waited it off and I found out later I had avulsion fracture. So let the inflammation do the work and let the doctor check it out just in case.What do you think about that approach?I assume the doc will recommend the icing?
@MalinMethod
@MalinMethod 8 жыл бұрын
+bwspot You should definitely find out if you have a fracture first. If not, then rehab should be the very next step, assuming your doctor does not have a problem with that. That way, your ankle will start healing quickly, while also improving strength and stability. This is by fat the best way to proceed with a sprain.
