Do you need an MRI for an MCL tear to determine how severe it is?

For many bone and joint injuries, orthopedic surgeons use x-rays to make the diagnosis. Some injuries can’t be seen on x-rays. When do you need an MRI to find out what the injury is and how severe it might be? In this Ask Dr. Geier video, I explain when an MRI for an MCL tear could be used and how doctors can tell the grade of tear without it.

Jem asks:
I am suffering from an MCL injury 2 weeks ago. My doctor hasn’t asked for an MRI. Is there any symptom that may indicate the grade of my injury?
The medial collateral ligament, or MCL, is a thick ligament on the medial side of the knee - the side closest to the midline of the body. You can suffer a torn MCL from a blow to the outside of your knee. Generally MCL injuries don’t need surgery. Often rest from sports and wearing a brace can help the MCL heal.
The MCL does not show up on x-rays, but an MRI can show an MCL injury. Does that mean you always need an MRI for an MCL tear? How can doctors tell you have suffered a torn MCL, and tell the grade of injury, by examining your knee?
If you have suffered a knee injury that you think is a torn MCL, watch the video to learn how orthopedic surgeons can often figure out if you have an MCL injury and how severe it is without an MRI for an MCL tear.

I injured my MCL snowboarding early this year Jan 3rd. I took a week and iced and elevated my knee as much as I could. After the initial week it still didn’t feel great. As a good piece of mind I wanted to get an MRI. Got my results back today and looks like I have a moderate partial tear with no other signs of complications thank god. If you have a knee injury, you don’t know or can’t tell the severity just on how it feels. wether its associated with mcl, LCL, Pcl, Acl, or meniscus. Your doctor can do a physical assessment, but that’s not enough assurance in my opinion. Get an MRI so you can rule out any other factors. Prevention is key to health and wellness and an MRI will pin point exactly what is happening with your injury. You and your doctor may think it’s just an MCL injury but who knows it could also be a ACL injury which only and MRI can know for sure and if not treated accordingly in the beginning and addressed could be a life long sustaining inconvenience for you down the road. I’m an active individual and I don’t want any lingering complications when I get older. Get the damn MRI. Money vs health and wellness I choose my well being. God be with you all and good luck.

I injured my medial left knee turning to the right on a dirt bike on a berm. The back tire usually spins and the bike whips around. This time I had too much purchase and the bike shot off to my left. My lower leg was stretched laterally. The pain was intense for bit, but when it subsided I was able to walk around without any weight bearing pain. I then I hopped back on my bike. As I rode and tried to squeeze the bike the pain was intense. I went home. Ice, rest and the pain decreased. When I put on a brace the pain goes away. Without it knee pain returns. It's been nearly 3 months. I'm finally making a Drs appointment. My knee gets stiff if I sit too long and hurts on the medial side. Palpating I can find pain on the upper portion of the medial epicondyle. Is that the adductor Tubercle? I think the deep part of the MCL attaches to the medial meniscus. Any suggestions?

I am a goalie and i had a player fall on my knee and bend it awkwardly inwards and i heard several "pops" a few day before Christmas. the inside of my knee was not in pain but it did feel almost disconnected from my lower leg. because of how close to Christmas it was, my doctor was out of the office so i was unable to get an MRI requisition. I have what i consider a really good chiropractor so i went to see him to get his opinion. His opinion was that i had a grade 2 MCL tear with a partial meniscus tear and went on to start working on it starting with electric current Acupuncture. I went on to see him twice a week for the first three weeks and slowly I was able to bear weight and start walking walking again. After every session i would always come out of it very sore the next day, but the second day after would be the best I've felt since the injury occurred. its now been 5 weeks and today i went snowboarding for the first time (with a hinged knee brace for support) and I didn't feel any pain, although the next day the injured leg was the sorest I've ever felt it since the muscles haven't been used all that much. i still cant fully straiten my leg but my Chiro says that it should go back to normal shortly. I found acupuncture to be very helpful in making me feel better, this is just my experience, and i hope you all feel better and heal quickly!

Why is it that even though I had a big pop followed by swelling on the left side of my knee, it feels like the knee is about to collapse on the right side sometimes when I walk?

I was Mexico Beach and water about my hips but was not looking and suddenly hit by strong wave and I hit my inner knee to the ground and up to now It's not improving and finally I got to see an orthopedic doctor. I hope they don't send to surgery. I'm worried it's not healing. When walking it hurts and there are times I felt if I move back and go sideways my will give up. Please advise.

So I recently twisted my knee. Begged for an mri. Given a brace and told I just sprained it. I was refused an mri. Doc stated in my notes in my file that it was a sprain with no soft tissue damage. I was sent to a completely diff ortho that sent me for an mri. They found a tibial plateau cyst, a complex mcl tear snd a meniscus tear. How can the first doc say I had no soft tissue damage? I'm very frustrated that he left me like that. Had I not pursued with another doc within a week from that I would have been left not knowing about the extent of the damage.

Nearly six weeks ago I leapt off the bed with my right leg, foot turned to the right and heard a pop when I hit the floor. (Trying to avoid my dog who likes to sleep in the two foot wide space between my bed and the wall.) i didn’t actually feel any major pain at the time and a few days later struck the end of the bed right on the kneecap! An x’ray showed an old baker cyst but no broken bones and I was made to stay in the hospital where a variety of pain killers were given with no success! I was and still am in excruciating pain! A walk in clinic sent me for an MRI which apparently saw a contusion….and I’m certain the blow to the kneecap caused it! But the pain I’m experiencing isn’t allowing me to walk. I am fine after sleeping and then walking brings the pain back. In researching MCL I’m convinced that the pop I heard days earlier has caused a tear (evidenced by a line of broken blood vessels running down the location of the MCL) and the second injury no doubt caused bleeding under the kneecap. I am dropping in to walk in clinics here in Montreal and been given nothing but heavy duty pain killers…..when I feel that the MRI has missed a MCL tear. The MRI has not been downloaded to my file yet so clinics have not been able to access it. Could the doctor who ordered a steroid injection three weeks ago and gave it to me have missed the MCL in the MRI? (BTW…the steroid shot left me pain free for almost a week) I’m 78 and feeling completely crippled by this situation.

I tore my right MCL (grade 2) and meniscus about 7 weeks ago from a ski fall but I'm still experiencing significant pain. I have improved about 75% in the last 7 weeks, initially I could not bear any weight on that knee, but I still have a hard time straightening my knee without pain or bending it less than a 45 degree angle. I also have pain to the touch. I wonder whether this could be some nerve damage, or scar tissue that I'm feeling. I haven't been ordered an MRI yet but still have hope 0 surgery is needed. Let me know if this recovery time frame is normal as well as the pain sensations. I am pretty athletic, and am hoping I'm back to skiing/climbing in 3 weeks or less.

I just had an MCL injury while skiing the other day. I heard a popping sound when I fell so thought it was a tear or rupture. I am a very active 64 yr old woman and ski, mt. bike, road ride, and hike daily and hope to do so for another 20 years or more. Doctor looked at some MRIs from 6 years ago that I had on file which show previous knee injuries. Through manipulation he assessed it as an MCL sprain but did not order new MRIs. I hope that this young doctor (30s) didn't look at me and decide that as an "old lady" with other knee problems that there was no point in having surgery because I was not some young athlete that might want to continue to participate in a lot of sports activities (because I still want to)! Might ask if I can get the MRI, or maybe wait since I might eventually need a knee replacement and they can do it then?

I have some type of injury to my knee, still trying to figure it out, waiting for my doc appt. Can't bend the knee more than 90 dg., but I can straighten it fully. However, side-to-side movements are painful as is crossing my legs where my foot rests on the other knee - like sitting crosslegged. Knee just doesn't bend. Can walk easily straight ahead with no pain or any other issues, and I can put all my weight on it, and I can even do a deep lunge. Thing is, I don't remember any event in which the injury occurred. I do remember, before the injury, now that I think of it, some episodes where the knee just starts to collapse - but I always catch it. Also there was no "pop" when the injury happened. I am having substantial relief from any pain because I am wearing a compression sleeve. I am very active, run - or used to run - 4-6 miles daily, but of course I am following the "rest" in the RICE acronym. Anyway I have been checking all the ACL, MCL meniscus symptoms but I can't find one that fits.

@@saurabhkhandelwal1507 I think your injured leg has less muscle than your good leg? And did MRI confirm that you have MCL injury? I am not a doctor but I only speak based on my own experience. I have the same issue as you, if I don't exercise for 2 to 3 weeks, the pain will come back once I started doing intense exercise like running

you need a surgery for sure if you want to play sports at a competitive level. other than that, you are fine! just make sure you exercise so that all the muscles around the knee are strong.....

I hurt my knee 3 wks ago. Pain below and inside of knee. Went to a ortho 10 days later, x-ray showed no broken bones, 2 days later had an ultrasound,, neg for a blood clot. Will probably have an MRI this wk. Very painful, sharp, throbbing pain, worse at night, I'm in a brace. Have you been to a ortho? Is it any better?

Just had an MRI, they thought I had a meniscus tear and cyst, turns out I had a bit of plica in the joint. I still have pain though and think there may be an MCL injury, I have a lot of pain near the joint line on the inside ofthenknee which increases when foot is externally rotated and the relaxed. I would love to send my MRIs to someone if they want to look and give a second opinion.

My take away? An MRI comes down to money. I want an MRI and I'm just an old guy. An MRI is the tool for the job. Without it the doctor is just guessing. An educated guess yes, but still just a guess. The patient - ME - doesn't like guessing.

There are nw plenty of low cost MRI places around the county. I got my knee done for 300.00 out of pocket. After I got the report I filled out a bunch of paperwork and because of the diagnosis (complex tear of the medial meniscus) I got even penny back. For the other knee recently I found out my insurance will pay 100% of that MRI. In our area the place is called Advantage Diagnostics. Good luck

hi david, i am a professional fighter... i have a shoulder labrum tear due to posterior dislocation of my shoulder while sparring .. my father also had same posterior dislocation when he was my age.. he never get any 2nd dislocation after that but weakness of right arm happened.. in surgery the doctor basically attachs the labrum via 2-3 stitches.. how to strengthen the joints to avoid any such future injuries...
