Wow! What an honest and informative presentation! Im blessed to be a youthful 53 year old but dabbling a bit with Botox. I do get injected around the eyes and forehead but your approach is conservative and it seems you appreciate woman who should age gracefully. We’re definitely not in world where most see it that way. I got mine done and it looked nice for the first year. I loved it so much. Now 5 years later I hate it 😂 it’s still there and looks like I have a wrinkle of filler and it’s lumpy 😭😭

One thing I don't see that you ever address is bone loss. When you speak of volume loss, it's about correcting it with fat grafting. But volume loss is not just fat loss, it's also bone loss. Actually, fat descends more than it disappears-something you touched on in this video. So, places where there is real volume loss is caused by bone loss. How does it make sense to replace bone with fat? What what do you think of structural volumizing, which is the placement of long lasting filler at the bone level?

My thoughts exactly. And of course a plastic surgeon will suggest something surgical. I think that filler is a good option to replace lost bone structure.

Fillerd with a larger HA particle size and heavier molecular weight is typically used to contour or provide lost volume due to bone loss. They provide more sturdy results. However, all fillers tend to migrate. Something that looked good initially can end up looking very strange after a while. We also know that filler can be problematic to dissolve. Lots of people that got filler for instance in their cheeks or jawline have ended up dissatisfied.

OK THAT WAS GREAT - now can we have one on Botox? Frown lines, 11's, Crow's Feet, Lip Flip (top and bottom), and any other places I haven't mentioned? It used to be that it was just used for the forehead and crows feet, but lately it seems to be used for many other applications and I'd love to hear what you think about that! I have my cosmetic surgeon do Dysport bilaterally on temples for migraines, a lips flip (I’ve had a lip lift), & to further lift eyes after a brow lift to make the eyes symmetrical

I do believe that fillers are bad as they deform the face , and nothing helps with the bloody sagging only a face lift , sadly most people like me would be happy to do a vertical restore at your capable hands but do not have the means to do so. I can only watch your amazing , artistic work on the videos you make and believe that one day I will be able to do so , but already 60 ,time flows like a river never to go back . Gosh, I wouldn't even have considered getting filler in my 30's or even early 40's but I would now I'm post menopausal early 50's so slightly gutted to hear its kinda too late! My sagging I realise I can't do anything about but I'd love to reduce the marionette lines and correct my downturned mouth as it makes me look miserable 😧, I hope this is still possible 🤞

Always good info! When filler first came out, my friend was a nurse practitioner. She offered to do it on me for free…but her own face looked strangely distorted and I got really scared for her. I was also frightened what she could do for me…so glad I said no! Especially now that we know how it doesn’t leave the face!!!

Thank you, Dr Karam! These videos are like having a free expert consult and it's really helped over the years with filtering out those that don't have our best interest at heart. People tend to forget that skin keeps growing. No matter how you modify it, it will keep growing and going down its originally intended route. That's why fillers don't work on a permanent basis. I just hope they invent something new by the time facelift is the only option because I know for a fact I won't be able to afford it, yet I wouldn't be able to live with myself if I start looking like my mother :'D

Or would you do it in separate transfers area by area? -Does the swelling after the procedure cause skin sagging? -Do you recommend to go to surgeons who are experienced with breast lipofilling? Hi Dr Karan, I’m a big fan of yours and really appreciate all these educational videos. I learned so much and I think they make a lot of sense too. I do want to ask if there has been a recent change of ingredients or formula on the rinse cleanser. I tried it before the previous version and fell in love with it. It made my skin so silky. So recently I bought another two bottles of rinse. The package looks different which I didn’t mind. But it appears that when I use it, it made my face slightly pink. After I switched to another gentle cleanser, the pink stopped but when I switch it back, the pink showed up again, so I wonder maybe there has been some change on the ring formula I really miss the old version. I had such great experience with it. Also, the new vitamin C serum seems to dry a lot faster than the previous version. I don’t know if it’s good or bad.

The temple region could benefit from Sculptra which is a biostimulator as opposed to HA filler. A lot of people consider both biostimulators and HA fillers to be in the category of fillers.

A surgeon who understand morphology and art. The two are relevant as smile lines are really important as give definition. Filler is a bogus concept as is Botox. Seeing more and more people erupting in reactions and Botulism and having near death experiences. Your surgical expertise really is the most important factor as underatndfing blood vessels and reactivity is vital. Dissolving is super horrific as much as putting it in. Best thing to so is have a face lift with an artist like you. If I ever make it to Cali I will come to you as I have EDS and would love my collagen elasticity looked at.

So, if I want my lips refilled, is it wise to have hylaruonidase injected before another application in order to prevent stacking of remaining filler, that may not be apparent? I want to avoid sausage lips and loss of perspective of what is normal.

Just in small amounts all over the face? I want to do some treatment just to give life, nothing drastical,. I'm in my mid-50's. After watching your video, It sounds like getting filler in lips and around the small lines where smile sags down now wouldn't be the best option b/c of my age and sagging? She's a dermatologist with a YT channel and she has different opinions on some things, I'm specifically interested in the topic of heat and energy treatments. There is a strong connection between dermatology and plastic surgery, I would love to see 2 docs that I respect from both fields discussing and explaining different procedures. I'm sure you have followers that also consume her content (or other derms) and for viewers it can get overwhelming when there are opposing opinions. 😊

I had Juvederm in my n-l lines, marionette lines, and to lift the corners of my mouth when I was 63 with *excellent* results that lasted a good year. When I tried it again at age 68 in my marionette lines only, the first week (of swelling) looked incredible, and then it looked like I had nothing done.

Personally won’t do filler but have done Sculptra and it’s been a game changer! Huge improvement! The only place I ever had filler was in my lips and never again! It migrated and dissolving process was not only incredibly painful for me but we ended up having to do 2 full vials of Hylanex over a period of time. 4 treatments in total! And it was mostly Kysse. 🙄 I also work in aesthetics. Lol

Plz tell me what exactly was in your lips? I have problem with biopolymer many years. Aesthetician lied to me and told this filler will dissolve in 6 month and more than 10 years it still there.

@@svetlanawilson6831 it was restylane kysse 3 times and 1 Juvaderm product but I can’t remember what one. Over the course of 5 years. I currently work in aesthetics but prior to had no knowledge base of the products and how they work. I will NEVER put that garbage in my face! So many Injectors are terrible about educating people. No one ever mentioned that my lips should be dissolved after so many times of filling. And the best part is 3 out of the 4 times I had them done at ay very reputable plastic surgeon’s office. 🤦🏼‍♀️

@@rhonda6791 temples, cheeks, jawline, and a little in the nasolabial and marionette lines. Fanned it throughout the lower portion of my face. Can’t be used in the forehead or a lot around the mouth. You need to research and find an experienced injector. It’s been amazing!
