We have patients travel nationally and internationally for the ESG procedure, though travel costs may negate any procedural cost savings. More important than cost, however, is that you have your procedure performed by a physician highly experienced in ESG. With ESG, technique matters.

Hi great video and information. I’m at about 180 and my feel good weight is 130-140. I believe I would qualify for this but what happens after wards like after years? Do the stitches come undone if not does the part of the stomach that isn’t being used die or rote? Do we remain the same weight if we keep the lifestyle of better eating habits? Thanks

In general, if the procedure is performed properly, the goal is for scar tissue to form and "fibrosis," which helps to keep the stomach stiff and smaller. We are not certain whether the sutures even need to remain in place after that occurs. But in general, they do. The goal is to use this procedure as a tool to make long-term behavioral changes. Then it will be much easier to keep the weight off.

Dont know if you'll answer. Pure curiosity: Why isn't the upper portion of the stomach reduced as well? I can imagine the food getting stuck there longer causing discomfort, but what do I know 🤷‍♀️. I have hypothyroidism my weight shot up from 134 to 184 then 195, I've tried to loose weight went down 165 then went back up to 176. I'm only 5' 3". I have acid reflux, hiatal hernia, small ulcer and cholesterol. Stress and depression trigger my poor eating habits, lack of will to do exercise has been at a high since I gained all the weight and cause more pain in my joints hurts to walk, feet and hands almost always swollen. what would you recommend?

We intentionally leave the fundus "untouched" during ESG. This leaves a small reservoir or pouch at the top of the stomach, which will fill with food then slowly empty. As the fundus fills and reaches capacity, nerve signals are sent to the brain telling you that you are full. So we are basically taking advantage of the satiety pathways in the stomach. It's quite a bit different than a surgical sleeve. Great question!

Can a patient combine the balloon and the esg procedure? Start with balloon for 6 month then do the esg after some time? Also, what about a patient with history of clotting disorder. Would esg be a great alternative?

Ideally, the one procedure you choose would be the best fit, and a long-term solution for you. But yes, you can definitely have an ESG procedure after Orbera. However, we would advise waiting 6 months or so after your balloon is removed, prior to having ESG. As for clotting disorders, this would need to be reviewed with a medical provider. The procedure itself is brief when performed at an experienced center (30 minutes average at our center), and you are immediately ambulatory, so the risk of blood clots should not be elevated. But again, we would want to review your medical history to help advise you.

What about food entering into the fundal area which is not plicated.... near the short Gastrics That'll behave like a diverticum and food can get trapped in that ...

Yes! we routinely perform endoscopic revisions of prior sleeve gastrectomies. It's basically an ESG, or "sleeve in sleeve." trueyouweightloss.com/services/revisions/sleeve/

There are no head to head, randomized studies, though overall the laparoscopic sleeve gastrectomy likely has a higher total body weight loss than the endoscopic approach. This varies somewhat based on the center and patient population. In our program, the average weight loss at 12 months is 23% total body weight and 66% of excess weight, which is quite comparable to the surgical sleeve.

Hi Sheila, in our program, the consultation fee is applied toward the procedure, should you proceed. We try very hard to ensure that anyone reaching the point of consultation is an excellent candidate for the procedure, so that money is not wasted. But these are important considerations!
