What is Endometriosis?
Endometriosis is a medical condition where the endometrial lining, which normally lines the innermost part of the uterus, also develops outside the uterus. This ectopic endometrium can cause pain and complications during the monthly menstrual cycle and may lead to the formation of cysts (endometriomas) on the ovaries or surfaces between the uterus and the rectum. In rare cases, endometriosis may even occur in areas such as the lungs, or inside the bladder causing bleeding during urination while menstruating. This condition significantly impacts the quality of life for those affected, often causing pain during periods, intermenstrual discomfort, pain during bowel movements, pain during intercourse, or infertility.
Types of Endometrioses:
Endometriosis comes in different types, with some being less severe and others more complex. The mildest form is surface endometriosis, where there are no cysts involved. On the other end, some cases can include cysts, and nodules between the rectum and the uterus, and may spread throughout the pelvic area. The seriousness of the condition helps determine the best way to treat it.
Endometriosis Diagnosis:
Diagnosing endometriosis involves a combination of clinical evaluation, medical history assessment, and diagnostic scans. Often, symptoms such as chronic pelvic pain, pain during intercourse, and infertility lead to seeking treatment. Diagnostic tools include ultrasound scans and laparoscopy which is considered the best standard for accurate diagnosis.
Endometriosis Treatment:
According to the doctor, the primary treatment for endometriosis is surgical removal of the endometriotic lesions, especially in cases of severe pain or infertility. Minimally invasive surgeries like laparoscopy or robotic surgery are preferred over open surgery. Post-surgery, treatment options vary based on the age of the patient and patient goals. Fertility treatments may be pursued immediately if desired, or medical management can be employed to prevent disease recurrence. In perimenopausal patients, injections or tablets may be recommended to manage symptoms and prevent the return of the disease.
Endometriosis Tests:
Regular monitoring through tests such as CA125, although not specific for endometriosis, can provide information about the prognosis of the disease post-surgery. These tests, along with periodic scans, help in assessing the effectiveness of the chosen treatment and ensuring that the disease remains under control.
