Today, I want to discuss a compelling article I recently came across in one of our trade journals. The article delves into the use of epidural steroids for the treatment of lumbar spinal stenosis. But before we dive into the details, let's have a quick overview of spinal stenosis.
Spinal stenosis refers to the narrowing of the hollow tubes or canals through which the spinal cord and nerves pass. Think of it like a buildup of calcium deposits in water pipes, occupying valuable space that should be available for the nerves within the spine.
Now, let's talk about epidural steroid injections (ESIs). You may have heard about these injections in the context of treating back pain, and I'll show you how they work on a model. In the spine, we have the vertebral discs in the front, the spinous processes in the back, and the spinal cord in the center. The space around the spinal cord is the epidural space, where we administer the steroid injection. Using fluoroscopic X-rays, we inject the steroid into the epidural space to bathe the contents of the spinal canal and alleviate inflammation and pain.
In a recent article from the American Academy of Orthopedic Surgeons, Dr. Bobby Kalantar of Georgetown University made an important point-epidural steroids are not meant to replace surgery, as surgery can be highly beneficial. ESIs are a non-surgical option to address lumbar spinal stenosis.
Dr. Kalantar highlighted the natural history of lumbar spinal stenosis, noting that untreated patients showed varied outcomes over a period of around four years. 70% remained unchanged, 15% improved, and 15% worsened, for no apparent reason. This natural history underscores the need for effective treatments.

Ask these Doctors what "steroid" they're using...First they're using a corticosteroids which is an entirely different pharmacological animal, and if they tell you they're using anything other than Pfizers DepoMEDROL Acetate Injectable Suspension preserved with Benzl Alcohol they are not telling you the truth! Pfizers DepoMEDROL Drug Monograph that every doctor has access to states clearly in bold capital letters that it MUST NOT be used for Intrathecal, Epidural or Intervenous use! The corticosteroid this doctor is speaking of, was from a New England "Compounding Pharmacy" in which was further contaminated with a fungus in which patients died, had severe cases of Meningitis resulting in chronic meningitis and chemically induced Adhesive Arachnoiditis. ALL Corticosteroids are toxic to NERVES and should NOT be used in any Spinal Injections for this reason as the manufacturer Pfizer WARNS! SO Doc?, what you gonna do when you miss?! Everyone needs to go into their pharmacy (call before) and pick up the drug monograms that ALL Healthcare providers have for Pfizer(s) DepoMedrol Acetate Injectable Suspension and the SAME Drug Monogram from the manufacturer of Kenelog. Search and Print out "The Burton Report" including EVERYTHING HIGHLIGHTED IN RED. This is a thorough factual report on the nuerotoxins that are used by what are called "Proceduralists" because they're usually a mixed bag of doctors who are making literally millions and millions of dollars off of the backs of patients. Pfizers DepoMEDROL Acetate Inectable Suspension is NOT FOR I.V., Intrathecal/Epidural Use! "NOT" should clearly state that this corticosteroid has had to go further than print "Contraindicated" for... Yet every Proceduralist is using this nuerotoxin and injecting it into patients spinal anatomy. Myristal-Gamma-Pictolinium Cloride and Acetate should be absolutely obvious NOT to use, yet these they do? The time is nearing when these procedures have done enough severe irreversable damage to perhaps milliins of parients, that there is NO CURE for and will be involved in heavy law suits if this continues to be taken lightly. I Agree. We should have mastered this a long time ago. We can put a space station in space & fill it with humans. We can build The James Webb Space Telescope which is 150x better than the Hubble telescope. We can land 3 booster rockets back to earth at the same time....... But we cant solve sciatica pain........ I was 35 when i was diagnosed with Sciatica caused by an L4 L5 bulged disc And im just so happy i chose epidural injections instead of the surgery (i was only 35!) In the end, i needed 2 injections (each after 6 months) which helped a lot with pain relief. i couldn’t walk before the injections for more than 5 mins without “bending over” to release the pain Now, i am completely pain free for the last 2 years without the op (which i declined) 🙂🙂🙂

Have had 5 of them in last 3 years . They work and pain is 90% gone for weeks then tapers down to 40% after couple months . Dr Girgis at Cleveland Clinic does mine . Procedure is about as easy as seeing Dentist for filling . Level 2/10 . 😐

@@livenotonevil8279 They got 90% relief from the pain for a few weeks and then it wears off and they only get 40% relief from the pain. That’s exactly how it works for me too but I’d say it tapers down to only about 20% relief after 3 weeks. Which really sucks when the side effects of steroids comes into play.

I had the linocane injections which provided immediate relief and about 2 weeks later I had the steroidal injections at L4/L5. Wait,wait, wait….no effect. My question was why can a simple chemical like linocane give me immediate relief and the steroids nothing. I asked several doctors this question and get vague, evasive answers. I take small does of pain meds that I have to buy off the street. Just enough to get me moving in the AM. I get grilled and lectured about the dangers of such a practice. I ask them what are the risks of stay in bed for the rest of my life, becoming morbidly obese, developing diabetes, heart disease, and also take up smoking marijuana for a small relief. Which is more dangerous doctor? I have been told that there isn’t anything they can do to help me and I should seek help else where. I ask “where?”. “We don’t know”. How’s that for modern medicine. I don’t know why insurance companies pay them for that kind of service. They don’t like to have there conventional wisdom challenged. When you are 73 and reasonably articulate you can do this.

Thank you for a informative video. My pain is not that bad. after watching some videos, I have decided to cancel the procedure and just do natural methods of stretching and exercise. I do not want to take any chances, as I suffer with anxiety and insomnia.

I have this ejections 4x a year and they help me tremendously! It gets me through the pain for a few months! The actual injection takes 5 minutes, I had my L3,L4&L5 surgery

Suffer from back pain. And for years. I sat and did nothing and it never healed. 2 years now of regular movement and stretching. Keeps the flaring down a bit. Definitely keep moving. Or try even if you think you can not. Get your mind right. And fight !!!!

Had an epidermal spinal injection, not worth it was the most painful thing I have ever done which now includes a spinal fusion. Issue is because of the spinal stenosis the epidural injection just lingers and doesn't go anywhere and creates more pressure creating more pain.

My wife has had those shots it did nothing but prolonged back surgery. Insurance company requires this before surgery. Finally after two fail back surgery she got a surgeon that new what the hell he was doing and that make a difference. I’ve talk with a lot of people that have had those shot they said the some thing. The shots are nothing but a bandage. Don’t take them it’s just a way that the insurance company postponed surgery and that save them a lot of money. It’s All about the money don’t forget that people.

I've had too many to count at this stage. Wish I knew then what I do now. A needle should never be in the epidural space. The damage can be catastrophic and any relief is temporary. They also inject pain med so you think you feel better. One injection temporarily paralyzed a family member. They were taken from the pain clinic to the emergency room by ambulance. While they recovered, there is lasting damage. ESIs have been the cash cow of the pain industry at the expense of our fragile spinal cords and our health. Injections can not expand the canal space. Only surgery can do that. Arachnoiditis is a word to remember when thinking about injections.

Most of the time, these injections are used to help figure out which nerves are being aggravated. Knowing this will help tremendously in your treatment plan & road to recovery. No Dr has ever said these injections are a cure. Every back injury is different, so finding out how to fix YOURS is crucial...& they must begin with injections.

Thank you Dr. Mork for the informative session. My mother suffered from chronic lower lumbar pain for many years from injuries at her youngage and was healed through specific stretching exercise. However, she took a fall a few years ago and the returned with symptom progressively worsen. She took another fall last week and now the pain is unbearable that the only relieve is when laying on her back. I had given a thought to consult medical professionals for ESI as an option for her which set me off to start research which lead me to your video. I have learned and understood this topic a lot from the internet, in particular from your presentation and explanation. I now realized that there is a real risk attached which may outweigh the benefit in this option. Thank you again. With kind regards.

I wish, for your mother's benefit, that you would reconsider the LSI. I fell 25ft. In late 2003 and landed on my back in roots and mud. I should absolutely be dead! Even though it didn't feel like it, an Angel broke my fall just enough that I actually drove myself to the emergency room. It took a couple years before I could physically be as active as before, but with the years my lower back pain got overwhelming! I was referred to pain management after several mri's and examinations revealed LSS. It was in several vertebra, but surgery was never recommended. LSI was administered, and I felt like a new man! I have several per year, and have had Cervical Injections also. Please don't believe everything that may agree with your opinion. Your mother would get Instant Relief from a LSI!! Relief she surely Deserves!

10 yrs ago. After phys. therp. 3 epidural injections with no relief. My Dr. did a 20% lamonectomy this procedure was succesful in relieving my sciatic pain.

I had the TFESI procedure done today. I can't believe the people who said they felt no pain or very little pain as the procedure was pretty painful for me! I had opted for the nitrous oxide (laughing gas) but the pain I felt was certainly "no laughing matter!" The procedure was tolerable for the first 10 minutes or so with the pricks and stings of needles, BUT towards the end of the procedure, I felt enormous stabbing, throbbing pains inside on my left side. I have been suffering for 5+ years with excruciating back pain, so despite the painful epidural injection today, I am praying for a miracle of at least 50% pain reduction. I'm glad for those who had success with this procedure and I surely hope that's my outcome too! 🙏❤🙏❤

Very intesting. I had the Transforaminal Epidural injection (L5-S1) today at Bon Secours Amblatory Surgery Center in Virginia Beach. I've had lower back issues since teenage years (I'm 62 now) but it's gotten much worse over the past 11 months. I hope to be in that 75% of the group who saw improvement. I realize the injection is just a stop-gap measure and I will eventually need surgery but I thought I would try this first. Thank you for making a great video.

I’ve had a total of three over a two-3 month time period I had some pain reduction from the first injection and each injection after was less and less pain reduction. Eventually ended up having back surgery on L4 L5 ten years later it’s now my L3 L2 narrowing

Did epidural steroids more then 6 times - it just makes your pain worse - its very short term relief- i had chronic back pain for 10 years and back surgery three years ago - i DO NOT recommend steroid injections nor do i recommend surgery (microdesectomy L4 L5) ruined my life and now getting stem cells to fix all the damage

I can’t wait to try this. I need relief from such severe nerve pain from 6 herniated discs. It’s had a drastic domino effect in the wrong direction for me and surgery is not an option. I may need a nerve block as well. I’m hopeful these 2 options will help with most of these bad symptoms

@@KVX21 these doctors will be held responsible for the massive damages and disability they are directly responsible for. The known neurotoxicity of using nuerotoxic chemicals in human spinal injections is building towards massive class action law suits never seen in the history of medical proceedures. They all know they're going against the manufacturers drug monograph and they all also know Spinal Epidurals are NOT FDA approved. Every single doctor who preforms these Epidurals is directly responsible for the catastrophic events and the intractable 24/7 pain they leave patients behind in has no excuse to be beyond their compression - a few of these Epidural pushers need to volunteer themselves because it seems that's what it would take for them to stop!

@@jmorse281 I suggest you insist on seeing the vile of corticosteroid your doctor is using BEFORE you consent. Write it down and do your research. That small "epidural space" take note...*the doc called it a "potential space" which is precisely correct. Spinal Epidural Injections are not FDA approved for not only this reason but there are NO corticosteroids made by anyone, anywhere that are not preservative free and that alone makes these chemicals extremely toxic. Pfizer DepoMEDROL is a ACETATE with Benzl Alcohol as the preservative, one tiny eensy weensy 'oops? and you are screwed! You do not want to know the adverse events let alone try to live with a nuerotoxic chemical inside of the most prestigious part of the human body, the Spinal Cord. These doctors are playing Russian Roulette when it comes to jabbing needles full of nuerotoxic chemicals anywhere near the human spinal cord. @Bea Lowe the main problem that patients have with chemically induced Arachnoiditis is its progressive nature that produces inflammation and adhesion of the spinal nerves to the dura matter that results in Adhesive Arachnoiditis leaving the thecal sac "empy" aka "empty thecal sac" and/or "cauda equina syndrome". Pain that burns like a "blow torch up the rectum and genitalia and down the legs",,,see utubes under EDNC acronym for End Depo Now Campaign. These utubes should be shared far and wide as Pfizers DepoMEDROL Acetate Suspension is the corticosteroid used while Pfizer states clearly in their drug monograph NOT FOR Epidural, Intrathecal or IV use, yet Doctors ignore this and do not give patients near adequate "informed concent" which leaves these doctors personally accountable for the horrific harms done. Great video!! I have been having this treatment & at first it was Excellent 👏👏👏 However my Doctor told me that one day it wouldn't work anymore😓 The chronic pain never really goes away and the things I find hard to do is 1 Stand in the one place too long! like peeling potatoes. The pain reaches a stage that I have to sit to get any relief at all 😩🤬 2 Walking any distance is impossible, the pain in my back and nerve pain radiating down the front of my leg ( the shin) and soles of my feet 😭 I try to keep moving from the time I get out of bed until I can't go any longer at night👎👎👎👎👎 It's a revolting pain that you wouldn't wish on your worst Enemy.. the last too epidurals I had really affected me like Never Before! I found I couldn't Sleep the first time this happened it was nearly three days before I could close my eyes🙄 The last one wasn't quite as bad but still couldn't sleep for 24 or so hours. Not Nice 😭

I’m in England and have the stenosis. My doctors won’t give me the steroids anymore,why? I don’t know,maybe it’s saving money. I am in total agony with it and the only time I get relief is if I can get some sleep for a short while.

I've have 6 epidurals, 4 of which were 10+ years ago & only one worked. Sadly,i fell, flat on my ass, just 2 short pain-free weeks after that last round. As of 2018, I had 2 more.... The last one was horrible, something went very wrong, & I've never been the same again, except for worse. Imo, these injections were a huge waste of time & money. Don't even bother. Or you could be in my boat, worse coming out than going in.

Spinal Stenosis is not so easy to treat. The Epidural shots for some people, have to have at least 3 or 4 in various months. Then depending on your physical make up, and you may be experiencing other medical ailments not yet known to you or your doctor, you can get the shot, but afterwards experience other malfunctions and or an infection. I think exercise is good, stretching, a decent Chiropractor who knows how to stimulate the spine, and for those who have CYSTS, perhaps surgery (not very invasive) pop that cyst and remove it. The only time I heard of Epidural's is for women in pain during child birth. But their shot doesn't last a year to two or three. It's a personal decision. If you feel it's good for you, go for it.

Dr. Tony Mork..It is quite a while since this video was done. I have moderate lumbar stenosis in L2-L4 both in the central canal and the foramen. My question is this..why are pain clinics now insisting that you do nerve ablation for pain? I have been having a great life for many years on the same amount of an opioid medication. It is the only medication I use. I am healthy..do not smoke or even drink. I am 67 and I know it is being done out of fear of reprisal from the CDC.

"Doctors" have made addicts out of millions of people by overprescribing opiods. Now there is a backlash against opiod use. Anyone who needs these meds, uses them responsibly and benefits from them is looked at as "drug seeking" now that the medical establishment is attempting to correct their "mistake" by withdrawing from opiods. In the facility where I work, everyone gets Tylenol or maybe Tramadol for pain. Doesn't matter if they have had major surgery or trauma.

Only good for for the first 12 months. That's nice, considering doctor's spent six months trying to figure out what was wrong with me (focusing on the shoulder pain so diagnosed me with bursitis, muscle tear, and rotory cuff tear before realizing it could be the spine, then telling PT, and PT and even more PT is the answer, then finally giving up and sending me to a spine specialist, which I will not get in til after my 1 year anniversary of this pain. Good going docs!

Dr Tony Mork, I really hope you will reply. Nerve block injections were never APPROVED by FDA to be injected into the spine RIGHT? I saw myself testimony before congress on this matter? These injections caused my brother's hip joints & tissue to deteriorate, not to mention the pain & trembling in his legs; the nerve blocks cause problems in other areas of the body & can/will cause greater damage in the body eventually, as the POISONS/injections could spread inflamation to other areas of the body where the meds are not even needed,right? 1 tiny slip can cause paralysis and even death, right? After the HORRIBLE side effects I suffered after having these epidural blocks in my neck twice, I decided to do more research. I have been told by different physicians over the past few years to never allow these injections in my spine ever again. I thought I was dying!!! Did these nerve block steriods go thru pre-market approval (PMA) since FDA hasn't approved this to be injected in certain parts of the body????. I dont know you& so I am not judging you. However, we KNOW that there are physicians that receive THOUSANDS of $$$$ by pharmaceutical companies to push their meds on patients that have not been FDA APPROVED. It is a sad thing when greed is more important than the quality of life for human beings!!!

I agree with you. I did research on this as well. I went to a spine dr and he told me the complications this could have. I saw my neurosurgeon yesterday and he gave me 2 options go back for this injection or surgery which I need 3 discs replaced. I will not get this injection. My sister got them in her back half didn't work. She had 3 surgeries on her lower back which finally worked. I refuse to have these doctors make money off of me. Like you said its not approved by the FDA

Absolutely they get kick backs.. when doctors are asked about the quantity of procedure done.. they boast- they don’t mention the quality of life that have improved. These numbers trickle into the grey area where they’re no longer examined and for a lack of a better term- patients become a statistic and a prisoner of their injury & disability.

Dr. Tony, I was just diagnosed today with a slight bulging disc in my T3-T4 region on my MRI, but my pain is lower around the T-8- T-9 region radiating to my left ribs causing numbness in my rib cage. My Dr. stated that my bulging disc was non impressive and it wasn't touching my nerves. I have had physical therapy for 6 weeks including the electro patches, (don't know the proper term), massage therapy, and acupuncture. My pain has been going on for almost 2 years with no physical injury to the location. I have just for the first time been prescribed actual pain medication this last week because it is getting to be to much. My orthopedic Dr has now told me we should do this epidural injection and we are scheduled to do the first one March 2. If the first doesn't work, he wants to do a second. Is it safe for someone to get done these injections without lower radiating pain to the legs in the upper spine?

I am pretty sure my Dr. messed up. I had a S1 L5 epidural injection. About 3/4 of the way through the procedure, the Dr. hit a nerve that sent lightning bolt like pain from my buttock to my ankle. I thought I knew what a level 10 pain was until today. The pain was excruciating and lasted about 20-30 seconds. That doesn't seem long, but when you are in that much pain, it is an eternity. I've had many procedures and accidents that has caused horrific pain, but nothing like that. My best description was an electric shock with tingling throbbing pain. After that my whole leg went numb. Ankle stayed numb for about 30 min. I am curious if anyone else had a similar experience with this sort of pain from this procedure. Again I believe my Dr. made a mistake. Dr. Mork, is this typical or was there a mistake?

Dr,Tony, after a year of ozone shots in my lower back, i am back again at the gym doing exercises but with a lower intensity...,,according to this dr, ozone has oxidated (diminished) my bulging disc, therefore widening my stenosis. I hope my comments help people.

Joe Schock I had one yesterday I got a major herniation at L 5 and a previous surgery for spinal stenosis every time I have gotten them by legs cramps are terrible but they ease up I stop getting them but had to have one in the hospital like I said yesterday pain was just too bad I would had back surgery if they would have done it the pain was so bad but the shot helped the pain just have stinging in my foot

Good question Before the pain of horrific and paralyzing episodes I had 5/8 horrible cramps a day all over my body My nightmare lasted 4 mos. I was taken to the hospital four times with paralyzing agonizing pain. One day I could not move for more tan 12 hours all I did was scream I could hardly breath than God a friend told me about CBD. Praise the Lord. I’m pain free. Today in fact is the first day I feel like myself I’m doing exercises since I got very week. Not habit forming. I read a lot about it. I found many friends drs had recommended I and are feeling great. Medicine is going this way. More research needs to be done on back problems I had issues since I fell doren a mountain when I was about 8 years old. Had a ten hour surgery at Mayo Clinic 15 days in intensive care. All went well until I over did myself and I’ve been “to hell and back”. God help everyone who experiences this pain.

I had 3 esi early on with my diagnosis. They struggled to get the needle into the space saying it was very tight. I specifically remember dr saying that he couldn't get it in. The esi's didn't help . I did a great deal of physical therapy & everything they asked me to do & it got me where I could work & actually walk about a block without extreme pain. I was given pain meds as stenosis is only part of my spine problem & I have severe pain from a nerve impingement. My understanding is a nerve is involved. Now my meds were cut in half & I've rapidly aged from having so much more pain to deal with everyday & qol is gone. They want to do ESI again which is stupid considering it's been 12 yrs since I originally had them & they didn't work. I also have osteopenia so I don't care to lose my hips from useless steroids. I will absolutely refuse them as I know they are dangerous & steroids according to FDA are not to be used in spine. I will most likely be cut back way back on pain meds to the point of stroke or heart attack or complications from pain. Joyous future huh?

I feel for you Karen. I am 78 & have some similar problems as you. Have had the epidurals, nerve block, etc. no surgery yet but legs are so weak, I can barely walk & not without a cane or walker. Wife is handicapped to so it a question of who will take care of who.

Dr Tony, I will be getting my first epidural injection this week but after watching videos I realize it is not FDA approved is that right? I had back surgery in my teens and have a metal rod in my back and so my discs are compressed. My dr said I'd become paralyzed then said he was egr what kind of doctor says that? That isn't something to joke about. I know you don't know me personally but what are the pros and cons? Will I be okay 6 months or 30 years from now?

First 4 days pain relief on l5s1 right disc bulge, after that the pain reverts and you start from scratch rebuilding pain tolerance. Improvements is iffy vs sticking with stretches and lumbar exercises.

I would rather have some form of pain management then face Failed Back Surgery. Those failure percentages are not acceptable to me and many others. RFA and epidurals have helped. I am looking into Ozone therapy and Stem Cell.

@@johnmitchell8925 Too late, I had spinal surgery in April. My back pain was cured after a 3 level nerve decompression via laminectomies and micro disc dissection. 5 weeks later I was able to return to work as a respiratory therapist.

@@jasonhenderson4376 Hey Jason, sorry to hear you need surgery, but mine was successful. I had an ortho that wanted to fuse my spine but I didn't want that done. That would have been a career ender for me. I honestly didn't want an orthopedic surgeon to operate on my spine. I wanted a Neurosurgeon and that's who I went with. It's been about 19 months since I had my surgery and I am feeling great. He removed part of the spinal bone that's called the lamina at 3 different bones L3/L4, L4/L5, and L5/S1. After I woke up it was a little sore and it's gonna be sore but it is manageable. Physical therapy will go by the next day and will help you stand and walk. I kinda felt like I didn't have a spine (lol) but I managed to walk about 100 ft. You will hurt when you try to adjust yourself in bed. So just let them help you. I was in the hospital 4 days. At home I had to put a plastic chair in the shower. By the next day I was able to go down the stairs and rest on the couch. You'll need someone to help you stand up. Anyways the only physical therapy I did was walk. I increased my distance a little to the garage, the mail box, around the block etc. Then to grocery store at week 2. Anyways I was able to return to work about 6 weeks after my surgery. Was sore for a few months, even felt like the pain was gonna return but it was because I was bending to pick up stuff that I would drop, but Motrin and rest helped. Just don't (BLT) Bend, Lift heavy things, or Twist. Feel great today, I feel like new. Oh after surgery you're gonna have an area of pinched skin above the incision that is gonna feel like someone is pinching you skin between two fingers. It will go away after about 8 months. My neighbor told me about that one because he too had the same surgery. So that put my mind at ease because it felt kinda weird for a while. When you wake up you might feel some numbness to your legs, that should go away in time, it's because when they operating in that small space they have to push nerves aside while they remove the bone to relieve the pressure on the nerves.

Another thing is that depending on how many disks are bulging/herniated will determine the size of the incision or approach. I had bulging at 3:discs so my incision is about 5 or 6 inches in length. If yours is in one area then a micro laminectomy would be the best approach because the essentially insert a tube about thumb size and with a microscope they do the surgery (ask the surgeon if he is trained in this approach). For me they did that and the regular way. They will also trim off the area of then disc that is bulging without removing the entire disc. I know that they can sometimes go thru the abdomen to reach the area. One other thing to mention is that if you only need a small area done you might go home that same day (out patient ) procedure. I'm now 51 yrs old and I work as a respiratory therapist in Las Vegas. My surgeon was Dr Gary Flangas. He was great and won't BS you. If there is anything else you wanna know, don't hesitate to ask. Oh ok was also able to stop taking the hard core Norco pain meds. Just regular Motrin was enough after a few days. Depending on how long you have been taking pain meds, you might have some withdrawals from the pain meds, you ll feel like you need to take one cuz you're gonna ache, and have trouble sleeping. But Melatonin 10mg for me helped and Motrin PM will also help with that. I think I went thru withdrawals for about 2 weeks. It will go away. Just fight the urge with the meds I mentioned all over the counter. Those achy sore feelings will go away. I returned to work 6 weeks after my surgery and like I said today I feel great and it's like I never had anything wrong with me.

Pfizer, issued a PLANET wide warning that they are not to be used for spinal injections. There was a 15-7 vote against) the FDA over ruled it, because, of course, the FDA is partially funded by the pharmaceutical companies.

One thing that was not mentioned in the controlled case study; was the group that was observed. There age, gender and general background of there life style. The number are surly generated to be bias. Think of it.. percentages of the people who got better/worse etc suffered a lifestyle change ie work, leisure etc. This info was not incorporated with the conclusion. FDA doesn’t even approve of this particular procedure so who benefits and use these determinations as a rule of thumb? Not the patient.

Doctor, I know first hand. I’ve had- next month will be my fourth injection for cervical radiculathy from an auto and work injury. Still have symptoms from 2010 auto accident where the other drive rear ended me and left the scene. 2 separate along with 3 separate PRP injections for a partial cuff tear. Along with another round on back (l5-s1 injections from 2017 injury @ work- it’s mentally depleting me while the limitations are hindering me. On the brighter side, I was told when I was young I was a “pain in the neck” and “pain in the ass” jokingly of course by my parents. But I’d didn’t expect this..thanks for your response. Your dedication and explanation of the procedures are nothing less then perfect

With complications attributed to surgery, such as failed back surgery syndrome, and formation of adhesions one should be very cautious about surgery. This should be the last resort. Surgery works by amputation of the back wall of the spinal canal to make space. Often times there will be extensive adhesions after 2 years. I myself know a neurosurgeon who has had 13 injections over several years and still doesn't accept the surgery for himself! Meanwhile there is inflamatory processes with spinal stenosis! At present, spinal stenosis is a dilemma. Neither surgery nor injections can solve the problem. I personally have seen catastrophic surgical results with loss of continence and paralysis of both legs, especially after the 2nd or 3rd surgery that commonly follows the first!

Either way you go you're almost screwed. Spinal stenosis can get so bad until the pinching ruins your motor skills or causes temporary paralysis. I did trigger point injects then ablation after 4 injections spread over 4 months. The ablation worked for 2yrs. That was 2016. Ive been looking at find a surgeon that uses the sonar equipment instead of traditional cutting tools. I probably will not get surgery anytime soon but the sonar methods seems promising.

really great seemingly objective video. got my first shot a few days ago. .however was ill with cold which became much worse afterwards. Probably won't get another shot as I really believe the issue for me is one of musculature

Edit.. I’ve since discovered the term Back Mice and believe this is a major contributor to my pain and shots not helping. Has anyone else experienced Hives 2-3 weeks after? I think it’s happened twice or maybe I manifested it from the trauma of the tailbone injection site unmedicated. Doesn’t work anymore anyways.

Im 28, and I’ve had herniated disc since my teen years, now I have two. L4-5 and L5-S1. This would be my 3rd major flare up that has kept me out of work. Tried many pain meds and doing physical therapy. Will see a spine dr next week to discuss ESI, but am terrified of the outcome but also exhausted from this pain MRI notes say L4-5: disc, facet disease, and ligamentum thickening. Central disc osteophyte extrusion contributing to severe spinal canal stenosis likely impinging passing spinal nerves. No significant neural foraminal narrowing. L5-S1: disc, facet disease, and ligamentum thickening. Left paracentral/foraminal disc osteophyte protrusion causing left lateral recess stenosis impinging the passing left S1 nerve. Mild spinal canal stenosis. Moderate left neural foraminal narrowing.

Did you have the injection? I've had 3 injections and it really didn't help me and my new dr is going to try another one. The first 2 I had I wasn't even sedated and it wasn't that terrible but the 3rd one I was sedated and that makes it easier.

What would you recommend Doc. Surgery or injection if I have lower back lumbar spine injury with osteophytes. Surgery was not recommended 5 years ago. Now it's painful standing up from sitting position

The problem is, isn't it rare that a patient has stenosis shown on MR and no other degenerative findings? Usually the study will show stenosis, some bulges, maybe a small herniation, facet arthritis. So how do you know the epidural is just treating stenosis. Older people (like me) often have a grab-bag of those findings, so the epidural may not be treating "only" stenosis. In other words, the stenosis may not be what is causing the symptom.

"Epi" dural..if they pierce the dura around the cord then they have failed the procedure.

For two years every two months I was receiving Epidural injections, but not Transforaminal, through the side. March if this year I stopped, bc my pain got worst, let the office know and my Pain Mgmt doctor or NP did not return the call. I came in after tolerating as much as I could, and told the NP what happened. An MRI was done to see what changed, and now I have a rare condition called, Epidural Lomatosis.. this is unacceptable, pain Mgmt is supposed to help with my pain, not make it worst. When I went back, the NP said, regarding my pain, "They could not help me and referred me to another doctor, a Neurologist.. I just want my life back and go back to work.

Lipomatosis seems to be mainly induced by corticosteroids, but will also improve after ceasing corticosteroids. Only found a paper on children. Corticosteroid-induced spinal epidural lipomatosis in the pediatric age group: report of a new case and updated analysis of the literature, 2011 Thinking about this and so many patients having to endure corticosteroids injections, maybe this is an epidemic of progression. I remember seeing some weird mri of slipped vertebrae supposedly causing an issue when it was clearly a mass on the opposite side which most likely was lipomatosis.

What is the difference between this epidural steroid procedure and having Ablation injections where there is a drug that kills the nerves with a chemical? It supposed to directly into the nerve. Can you please clear this up for me?

It seems to me every medical drug or treatment is that way. Some people get the treatment and get the promised benefits, the second is, they pay the money and get NOTHING, or like me, I get the high Cost and I acutally et worse. I had a doctor tell me EVERY treatment and drug going works like that. It is all a crap shoot, BB

This is the standard distribution. For any medical treatment, including prescription drugs, the success rate falls thusly- 33% full success w/ no side effects; 33% partial success with some symptoms; with some heavier side effects; 33% no success, with multiple side effects.

I have spinal stenosis, and have had about 6 doctors say I needed the surgery to infuse L2,L3, L4 and L5. I have two vertebrae 1 going to the left and 1 going to the right. Causing narrowing of my spinal cord. I have nerve damage which causes me to fall alot.

In my much research of reading, listening and viewing, I cannot find an "answer". My impeded central canal has only a 25% channel for the nerve bundle to pass through. Exercise, stretching, alternative treatments, etc. can possibly OPEN or RELAX "MANY PINCHED AREAS" in the body, PROMOTING MOBILITY and PAIN MANAGEMENT. But non-surgery options WILL NOT REMOVE CENTRAL CANAL HINDERANCES or SPREAD THE CANAL BONES/OPEN THE CANAL ..... in order to enhance mobility and pain management. Is that correct? If so, it seems to me (if generally high success rate) that common sense dictates that limited invasive surgery should be an early option instead of a last option. Clean out the pipe first and then add then add stretching, exercises, etc. Is this faulty reasoning? Thanks for any help!!!!

Help please? I have asked for a second opinion multiple times, my MediCal group (PIH Downey, California) just sends me back to the same Orthopedic Surgeon who tells me the same thing; “You need a four-level fusion!” When I reply; “Yes, I agree that my neck is indeed broken! However, the recovery plan of one night in the hospital, and six months at HOME…might be a problem!?” Because I am homeless, unemployable, and living in my 2001 Ford Expedition? His answer is basically, “that’s not my problem, I’m just the mechanic.” I’m paraphrasing, of course, but sadly he did actually refer to himself as a mechanic-not very comforting. Is there somewhere I can send my CT SCAN to, so some more creative surgeons can offer ANY reasonable ideas? Thanks, T. Sullivan.

I live in the uk 🇬🇧 and waiting for a spinal block injection I have had two spinal operations on my spine for spinal stenosis each lasting 10 yrs but I'm suffering terrible with sciatica pain down my leg numbness in leg and foot painful standing and walking will this injection work or should I have another operation?
