Thanks for these info, Dr Luo - very helpful; especially the Q&A about the aftermath. My hubby is to undergo this procedure hopefully in a week's time. He had a back surgery 13 yrs ago (L3/4/5: 6 screws and 4 cages) and is in constant pain. He also had a decompression at L2/3 some 2 yrs ago (the compression was some 70%) with no improvement - it actually got worse. This Epidural Injection is our last option. I just hope we're doing the right thing!

I was suffering for 3 years due to a herniated disc. Tried everything. Exercise. Cold and hot patches. Physio. Chiropractor. The pain would not go away. I could not sit, as I had this pain in my buttocks. After walking or running, the pain escalated all the way to my thighs and to my calves, so it felt like shin splints. I had the epidural shots 3 times, but they would not help at all. So finally the spinal surgeon did the operation just 3 days ago and I feel great. Finally for the first time in years, the pain is gone. It will take 6-12weeks to fully recover, but I can tell now, that it did work. Can’t wait to start living the kind of life I lived before and go running again.

I am scheduled for steroid injections in two weeks and reading some of these comments I am not sure if the injections are going to work. I have pain on the left side starting at the hip and down the front of my quad. I have had these pain issues off and on for over 35 years and now for the past 3-4 years, it has been constant. The pain doctors have diagnosed my pain as stenosis and are giving me two injections in separate locations along the Lumbard. All I hope is I get some relief from this pain. I will try to follow up on this channel after the injections.

I call that pain the Andaconda. It feel like stabbing in my hip then is coils around the groin to make my thigh quiver then continue s to my ankle THEN a pain like a hot branding iron goes inside the bone all the way to ankle

Dr.Luo, thank you so much for this video. I'm getting ready to go in and having four needles put into my lower lumber. I have to do this three times before they go anything further. I have three bulging discs, and one herniated disc. This is creating some pretty bad nerve pain throughout both of my legs. I have suffered with this pain for 10 years now.

Thanks for the video doc. My pain is on the Left glute and goes down to lateral calf only. Never get it all the way down the leg. MRI of Lumbar Spine shows Left S1 root nerve impingement. MRI of SI joints and hamstrings are completely normal. I heard you say if it is radiculopathy pain then it should radiate all the way down. Is that true for all the cases?

it's definitely possible to have your symptoms being lumbar radiculopathy without touching the foot.

L4-5 disc desiccation and broad based, central & B/L Paramedian disc protrusion / extrusion with larger central component indenting the anterior thecal sac and causing B/L L4 - 5 lateral recess / subfacetal space stenosis. What do you suggest???

Dr. Luo. I've always been very scared about doing this. My bones in my lower back shift several times a day. I have had serious problems for 50 years. I adjust my back in the morning and evening. Without it I can't sleep or stand. My concern about these shots is for those people who have major movement with the bones/discs, would a shot be too risky if it ended up being put in the wrong spot? Also, is this risky for heavier, obese, people?

The injection is theoretically hard to perform for heavier and more obese patient, so it's possible that you can have more pain with procedure, the other risks should be similar.

I herniated a disc in the military and was unbearable pain down my right leg to my toes and got typical military help with just a chiropractor and Motrin so never really got better 12 years now and still have irritating pain in my leg and back so considering going to the VA hospital and trying this, really hope it works so I can enjoy things again as I’m very active and love the gym but it’s hindering everything , if you have any info to help guide me to start the process I would appreciate it greatly, thank you!

I would ask your pcm to see if you can get a referral to the pain management branch at your local VA, if that is not possible or the wait is too long, you can ask them to out source to a private pain clinic, you should have done some physical therapy with VA first though.

I don’t anxious to get the procedure over but my blood pressure was too high for him to do so he said I’d have to see my primary to make sure all is well I was very anxious and knew my heart would be pounding. Sometimes we can give some medication for anxiety prior to injection, but if the blood pressure is too high, then it might be better to see your pcp first

Is the injection cortisone? I’ve had gastric bypass and if Steroids are used it can lead to osteoporosis ? I’d love to be out of pain.

It usually contains dexamethasone which is a steroid. A small amount of local steroid in a specific area usually does not cause significant osteoporosis. However, we know people who take oral steroid daily for a long time can have osteoporosis due to steroid use.

After 2 years of spinal decompression I have no pain but still have tingling right hip down leg to foot, last three toes tingle and or are numb daily. I had mri 3 yrs ago L4/L5, L5/S1 herniation and large mass bulge, moderate to severe stenosis. A new mri a year ago L4/L5 5mm bulge, light to moderate stenosis. Question is with your history when or how do I get rid of this tingle? I feel a nerve is probably still being pressed, just not sure if spinal decompression is going to fully cure or if I need something else

I had racing heart 3 days which mad legs hurt and could not sleep! Really hard time to sleep, but pain did get better after. 2nd shot same thing for 2 days, but then constipation set in, and real bad gas for 10 days. Is this normal? Odd effects.

racing heart is a common effect from steroid and so is insomnia. Constipation is not commonly reported in my experience.

I usually use midazolam (versed in clinic) and 0.25% bupivacaine with dexamethasone for the injection. The Level will depend on the pain, imaging.

I have spinal stenosis - I have had three epidural injections. The relief lasted 3 weeks at the most. The last injection relief lasted one week. Tomorrow I will be having spine surgery L2 L4 Leminectomy and -L4 L5 TILF

I am sorry to hear that. The epidural steroid injection is not magic, sometimes for people with significant lumbar stenosis, it just does not last very long. Good luck with your surgery!

Most patients getting a relief from the 2nd injections. I always get a relief from the 2nd injection.

it's going to be very hard to predict, I usually would recommend 2 ESIs, if neither one give you any pain relief, I would not recommend ESI in the same area again for the same pain. Hope that helps.

John : if you have spondylolisthesis with lumbar stenosis.. surgery is better option for you. Repeat steroid injection will also fail.

it should not be common to hit the nerve unless you have severe stenosis, even then we don't hit it routinely. If that does happen, patient can get post procedure nerve inflammation after the procedure.

Ask the doctor if ESI’s are approved for Epidural administration by the FDA for safety and efficacy? Most incorrectly say “yes they are” when in fact they are not for reasons that will scare the pajamas off your back. Oh and BTW, Pfizer recently announced that Fluoroscopic guidance does not reduce errors in their latest DataSheet for Depo-Medrol (Methylprednisolone Acetate) … it just helps the doctor bill for more $ while it informs him he just misplaced the dye, punctured the dura, or just poked a hole in your spinal cord.

Am I not a doctor but those things have been said to me and explained!the lack of meds is so when they prepare (give you the medications for specific procedures)you you are safe from any medications from crashing with one another which can cause your death!and the eating? Well they like for all of your pipes(intestines,bladder etc to be free of any and all substance) so in case anything goes wrong they have a better chance at working on reviving or bringing you back up to 100% in health!

I am not sure about not taking any of pain meds one week prior to the procedure, that seems a bit extreme, unless you are on a crazy amount of pain medication. The not eating part is for anesthesia/sedation, due to possible aspiration risk.

I had 1 right transforaminal epidural steroid injection of the L3-4. It has reduced the amount of severe pain in my leg and hip area. While I am pleased it has helped so far I don't know if I can withstand the pain of having the another injection if or when I need it, it was excruciating, is this normal for it to be so painful?

Can you explain what you mean by the pain during the injection? It's possible you can feel the pressure while the medication is being injected, or the nerve was irritated during the needle placement.

@@DrMaLuo It wasn't pressure that I felt it was terrible pain almost a cramping pain, it seemed that when the Doctor was injecting the medication in to my back it caused severe pain from the hip all the way down the leg, then he would pause for a few seconds then release more medication which again caused the pain, he did this several times. I'm really afraid to have it done again.

They are usually for different things. Esi is more for disc bulge and stenosis centrally and peripheral nerve block are done for more joint pain and extremity pain.

@@abid-ali1235 yes you can use lumbar picture needle, usually the needle we use is a 22 gauge or 25 gauge for transformational and 21 gauge for interlaminar. There is not one steroid that is superior but most physician use dexamethasone for transforaminal for safety reason. Position wise, it depends, different people prefer different positions.

Update; this morning I had a second round of injections. I had a phone talk with the doc and he said he thought a second round would be easier because one had already been done. I asked for Ativan and took 2MG before procedure. It was tolerable to the point where I will consider injections on an on-going basis. I wanted to let people know this so they wouldn't not get shots because of what I said. Also, I had played tennis 16 hours before first time and that probably aggravated the hell out of my spine. Just wanted to tell you that your update encouraged my hubby and me to go for it. He's had everything (see my previous posting) and was just getting more and more miserable. Finally we went for the ESI 3 days ago and he's feeling an improvement already. We're to expect a set-back we were told and then it should go steady for- and upwards. I hope the docs are right. I only witnessed the agony but it must be one of the most excruciating pains. Hope you're doing better too and thanks again for your positive update!

I have DJD, with multiple fusions in both c spine and lumbar. I have never had any relief from steroid epidurals, except for the short period , following the injection. It's very disappointing, when you are hoping for relief. 😥

Mine didn’t kick in for 14 days after injections. Then it lasted with 75% pain and mobility relief for only 6 months. Going in for second round of injections under sedation next week.

i usually try not to do more than 6 a year, but when you do more, you increase the risk of getting too much steroid induced side effect (weight gain, mood swing, osteoporosis/fracture, suppression of your own steroid.

This is a long discussion, but the short summary FDA has not approve epidural steroid injection due to the fact hat in the past, there were significant adverse outcome with particulate steroid injected into the epidural space and none of the catastrophic events with non particulate steroid (dexamethasone) injection in the epidural steroid. I only use dexamethasone because of these concerns.

Facet joint injections are not the same as epidural steroid injections. It's rare that we do fact joint injection now in the US since they are not diagnostic. We usually do medical branch block and rfa for fact joint arthritis.

@@DrMaLuo okay understood. Thank you. I will assume my spine Doctor -DR Mazzafaro will do an examination to determine where he will give injection if my pain from the L4 L5 facet joints gets me no relief.

So medial branch rhizotomy are for osteoarthritis and ESI are more for nerve pain. ESI won't help everyone.

@@DrMaLuo in the mean time my back pain is much better by using the Atheq System, which most doctors knows nothing about. I skipped the injections and the surgery and the PT, and of course health insurance will not cover Atheq System because it works and the insurance system cannot milk patients of their money.

I am not sure there is a direct correlation of ESI with Tarlov cyst. However, still sorry to hear. Symptomatic Tarlov cysts are no fun.

@@DrMaLuo No, they have ruined my life….three TC surgeons in the whole country. Read up..go to Tarlov Cyst Foundation, Dr Frank Feigenbaum in Dallas, TX (clearly has indicated they are, especially myelograms, heavy lifting, trauma, and all those injections. Dr. Forest Tennant, expert on Adhesive Arachnoiditis . Many women have gotten symptomatic Tarlov cysts epidurals for C-sections. I will never leet another pain by mgm doctor touch me..

sorry to hear that they put you through 5 of these, I would not recommend to continue after 2 in the same place and it is not helpful.

Unfortunately, sometimes if the nerve compression is really bad or if the problem is not coming from nerve compression, the epidural steroid injection does not help people.
