1. There is no dress code for regular class days; you can wear anything you like. For conclave, you will be required to wear suits paired with solid ties (no prints or designs) and plain shoes (nothing fancy). Other than that, no specific dress code is needed. 2. Two pairs of formal wear should suffice. While generally unnecessary, having them might be useful if you wish to dress formally for presentations. 3. In the required document, they would have asked you for a medical certificate as well. If you are having trouble generating it, don't worry. I faced the same issue, and what I did was get the certificate generated in Amritsar. Even the hospital staff knows you are from IIM Amritsar, so they will generate it for you without any issues.
