I’ve had 12 surgeries over the last 16 years and every surgery gets more and more complex. It’s quite hard to live with to be honest. I like that this doctor does not sugarcoat over everything. I’ve been told mine will never go away it is not curable and to expect more surgeries.It’s what kept me single for the last 15 years. That’s my choice

Wow i felt like i was the only one going through this. I was misdiagnosed for a year. Was told cysts then went elsewhere found fistula was cause. Now have setons in and i feel like a disgusting person and life sucks dealing with these. I got 2 myself and they not much fun

Just had a fourth Seton placement yesterday. Looks like a pony tail holder. They come in fashionable colors. These little things are amazing. I have Fistulizing Crohn's so the name says it all. Has never been a negative impact on my daily activities.

Hi. Please tell me. Is it painful how much painful. I'm scheduled to have it on Monday, but I am not sure. I'm scared. I had hemmoroidectomy 6 weeks ago and developed a possible fistula and abscess. Still to be confirmed. My BMs still pain after hemorrhoidectomy.

Heavy drinking causes mine to drain like crazy . Heavy drinking is what kept causing my abscess. When I eat clean and stay away from alcohol I’m good. I can’t wait to get this seton out.

Would love to know if you had it out??? It’s usually risky and 50% likely to become infected and have to be put back in again… I’d greatly appreciate knowing what happened with yours.

Don’t live with a fistula like I did for two years. Get the damn surgery. It’s an outpatient procedure and not that bad at all. I was out of surgery, on a plane, and back home in less than 5 hours. I was so afraid of fly home the same day I was having anxiety attacks leading up to the surgery. It was a waste of time. I felt no pain leaving the hospital and felt no pain till two days later. Advil every 6 hours did it for me. It worked fine. I was so afraid of the pain during recovery I had my surgeon prescribed oxycodone just in case. The surgeon prescribed me 12. I didn’t take one. I was tripping out of my mind. Don’t let being afraid of pain/recovery stop/postpone your surgery. It’s not that bad.

I had abcess drained 5 weeks ago in er 4cm deep, and now a seton inserted today for a few weeks as fistula discovered .then they need to decide on a surgery procedure to heal it.this is just too long of a treatment and discomfort for any being! Not forgetting a skintag removal in march which possibly caused all of this. 😅😢i wish i could wake up when all is healed, it can mentally drain you. They need to discover a better way to heal these.❤I really feel for all that suffer with this.

I have had put a seton 4 months ago and it all went well until a few days ago where the seton has been giving me a lot of pains. I take painkillers for it, but it is very annoying. I am a student so i sit down a lot to study but has been rough. So when I heard in this video that it is normal that the pains can come back, it relaxed my mind.

Panicura doesn't work well on chr fissure. Most people waste money on those medications in the hope to avoid surgery but ended up having a procedure.

I've been wearing a Seton since I got Necrotizing Fasciitis. I survived that, now it's been 2.5 years. My Seton is between my bottom and I use a combine gauze. I hope to go swimming again. Horse riding is out of the question.

I have severe anxiety about keeping clean so I either shower or use a bidet squeeze bottle EVERY time. I have a go bag with the bottle and extra underwear for any accidents. Yes farting takes on a whole new meaning because the fart will find EVERY opening and it pushes out that nastiness where it can

Man.. I got 4 of them right now. And my life is hell. I cry myself to sleep and wake up to immediate pain every day. It’s infinitely more painful than the 3 abscess that were there.

Personally I’ve never had my sessions removed. My surgeon says if they’re not causing any problems we’re gonna keep them in because they’re working. Some doctors removed them and it could cause a re-inflammation. Every case is different and it’s a horrible disease

My partner had a seton placed in his fistula. The fistula never healed even after 6 months post op. You know what he did? He went to India to have DLPL done. He stayed there for 4 months. The proctologist there said the seton wasn't even in the right spot. He had the surgery done, had a doctor's follow up 3x a week initially for cleaning and checks, then down to 2x and then finally just once a week. 10 days before he flew back home here in Canada he was already fistula free and fully healed after 3 failed surgeries, months and months of ostomy follow up, and daily dressing changes which I did for him. I could not fly with him to India because I dont have enough vacation hours. Thankfully the care he received there is top notch. 2 years of dealing with this ended after he went there.

Can a Laser treatment be done after placement a seton? If i understood correctly treatment with seton takes long time but how about combination with a laser treatment?

Dear Dr. Chung, I have had a recurring anal abscess for the last 6 years. It comes in the exact same spot every time and I notice that it is aggravated by certain dairy products, as well as if I touch it too firmly when checking to see if it has come back. I have needed to have incision and drainage 6 times and I am losing my mind, as it seems to be starting up again just over the last 2 days. I have had exploratory surgery under general anaesthesia once last year as my general surgeon was convinced there was a fistula. He could not find one. In the past I also had a colorectal surgeon in his office look for a fistula, but he could not find one. I am wondering if you have any opinion on the matter. Of note, I did have trauma to the inner part of my rectum when a drunk woman used a sex toy on me (without lubricant by accident) and a few days later I did develop the initial abscess and it is from the same side as the trauma. I can no longer and definitely wish to no longer engage in that anymore. Please Dr. Chung, please help me. Thank you, Joey

Hello friendly proctologist! I’m wondering if you could do a video on complex horseshoe fistula? I’ve had this for almost 15 years and I’ve had more surgeries than I can count. I am currently in a very bad flare with an infection in the size of an orange. I’m on antibiotics but I have a feeling I’ve got another surgery coming my way. My surgeon is recommending a stoma surgery. Anyways I’m just wondering if you could share your thoughts on Horseshoe fistulas. As there’s not much information out there. I basically had it go from my right side around my tailbone and over to the left side and I had a litre of infection. You’re just always so positive about this horrible disease and I thank you for that❤

Hope you've had intervention since. But get the stoma. Get the bag and then have the procedures done on your fistulas. Once they've healed, consider keeping or removing the stoma. You might find the stoma easy to live with. I had mine for 5 years, no problems. Reversed it and 3 months later had a flare up of crohns

Hello Dr Chung. Hope you are doing well. I had a Perianal abscess about 7 months ago. I had an incision and drainage performed, however it turned out to be fistula, following an MRI. I consulted my surgeon, and decided to evaluate and decide on what procedure to perform. About 2 weeks ago, The surgeon performed fistulectomy, and noted "low fistula -- non communicating". I'm just worried about what is"non communicating", did he miss the internal opening, or was it just a sinus, or anything else. I am just worried about it coming back.

I had a fistula /abcess that i could not get looked at ɓecause even tho i couldnt sit, my wbc count was elevated the 3 doctors i encountered at ER dismissed it because they couldnt reach it. Finally i was bleeding profusely ie 3 toilet bowels of blood in an hour at Er they finally admitted me a dictor saw it said i dont like this cat scan stat. I was hooked up to liquids as i had not voided or had a bm in 7 days. I had surgery seto. Now 6 months later with constant on the hour diareahea the genersl surgeon wants to do a fistuloytmy. He said the wound is 3 inches long right now. My q is with my super frequent bms will wound healing be a possible ptoblem ? I want to keep the seton. Anyone ??

Hi Dr have you seen the knotless seton drain from a company called AMI in Austria. Only Seton drain made for Fistulas.

I’ve had two incidents with my fistula, but each time it ruptured by the time I got to the doctor and he says it would be more difficult to find a canal. If it’s not infected, on the second incident is ruptured so he wasn’t able to do the procedure now this morning I have a smaller filled sac so I’m going to see him at 10 o’clock and I’m hoping that he could just do it today. I wanna get it over with.

Hi, not about this topic, but I really need your help. I had a hemroidectomy surgery done exactly a week ago, and am still in agonizing pain. I REALLY HOE YOU READ THIS. I wish I had you for my surgeon, you seem to care so much. My surgery has been the most excruciatingly painful thing in my entire life, and Im only 7 days in. I had to go to the ER for pain, and they didnt do anything but give me Lidocaine. BUT MY BIGGEST ISSUE, is that I go to the bathroom to much. I have about 4-5 bowel moments per day. Its hard to judge the quantity, but it seems average sized poop. I am on Mirilax stool softener, and the poop itself is very soft, almost like soft serve ice cream constancy. But its sooo painful as its passing, it feels like razorblades. I can handle 1 or 2, but I poop at least 4 times a day. Practicly every time I walk by the bathroom, my body wants to go. I eat a normal amount of food. A bowl of bran flakes cereal in the morning, a pp&j sandwich for lunch, and some chicken breast and green beans for dinner. With little variations like broccoli and carrots and peas, and a home made burger. Also some snacks between meals like apples and bananas and whole grain granola bars. I also drink the occasional Ensure Protein drinks, as well as much water as I can stomach. I was told to eat a high fiber diet (but not overly high), so I sticked with bran flakes and fruits and veggies with fiber. But I just keep pooping, it feels like I go a lot more then what I eat. What can I do? I feel like I wont survive another day, if I have to use the bathroom so often.

Really? I'm shocked the Philippines know what a seton surgery is. There's literally no cases of Crohns or UC in the Philippines. The Gastro doctors in the Philippines are nowhere near as experienced or have seen as much as Western gastros have seen. I just hope they make many advances in the Gastro field in the Philippines and Asia as a whole.

I've just been told that I have a complex fistula. They have no idea what the treatment will be as they need another MRI and other tests to rule out chrons. They said I'd definitely need seton

Just wondering how you’re both going? I was just wondering if either one of you were diagnosed with Crohn’s and whether it was actually a complex fistula. I have a complex fistula due to Crohn’s and I’ve had a seton for two years and each time I go to the specialist for a check up to see if it’s healed and whether he’ll be able to remove the seton he tells me that there’s a likelihood that it will become reinfected within two weeks and that there’s a 50% chance that this will happen I am so distressed fearing that I may end up having to have the seton in place for the rest of my life :(((((((

@@virtuallyme2518 Still waiting for treatment on the NHS. It's gradually got better, so much that I don't notice it anymore. The things that helped me were COVID and buying a portable electric ass spraying machine (portable bidet) The colonoscopy showed no chrons or other anolomolies

I had operation for fistula around 7 times, im currently recovering from surgery last week using seton by my doctor. But when it was dec 21 i went for FILAC METHOD SURGERY, The weirdest thing from FILAC METHOD was it occured once a month the abscess, so on january the abscess exploded by itself and on february my doctor made an incision on my recovered skin that already closed, he cut me open again forthe pus to come out... I thought that will be over right, but on march came back again, abscess every 25- 30 day exactly... My doctor said oh dont you worry it will healed up within a month, then at night the wound getting bigger and painful, so i swithed hospital and doctor using seton method. But the question is during december - january i drank a lot of alcohol then boom abscess, jan - feb i ate a lot of spicy food, feb - march yes i drank alcohol but only a bottle. Do spicy food and alcohol allowed during recovery? Or do i have to ban those 2 all my life?

Nurses told me food really matter with what seens to be a complex fistula. I was on a restricting Diet 8 months ate brtn 700 to 800 calories list weight BUT developed n abcess fistula. My instructions eat 20 fibre day, eat adequate calories, avoid alcohol and spices foods. I've done this have been regular ea h day since surgery.

@gosu84moba74 I was restricting to 4 ounces chicken 4 egg whites per day that's not recommended. Now I eat fruit veg and small amount chicken I still lose weight at higher calories. The fustula us not cured the surgeon only drained the abcess in emergency surgery and placed a drain in the fustula that will be in me many many many months.

@virtuallyme2518 hi there since I made that comment the surgeon who was gonna do a fistulotomy just cut the seton out at a check up in may. I've been fine ever since so my guess no I don't have chroens. Dang it was one the most difficult physical experiences I had tho.

Hello! So i under went a fistulectomy surgery then i had a seton placed. What i don’t understand isnt a fistulectomy suppose to be an incision of the fistula tract? Then why do i still have a seton? Shouldn’t the tract be removed during surgery and closed? Please can anyone answer and help me understand thanks 🙏🏼

Hi dr., ihad a blind ending sinus tract . First i had an anal abscess. 1 month after drainage, got a fistula but it didn't had an internal opening. My dr. Did fistulectomy + placed a cutting seton on the inner end so that it can drain as well as cut the inner end as surgically it might have damaged my Spinchter muscles, so a seton was used. Now in few days, there will be seton tightening, will it hurt? Anyone else?

Helpful hint: don't wait for your doctor to do all the tightening. Within your comfort level, try to "play" with it, jiggle it around, pull gently, etc.

Doc I remember using soap on anus before 2-3 years but after that I experienced anal itching and moist down there hence I searched about it and knew that Soap is probably the cause so I stopped using it , but since then I have dead-skin flaking on the anus area which I haven’t experienced before , when ever I rub that area or just touch it some dead-skin would be there , I don’t know if this is normal because I grew since that incident but like is it normal to have some dead-skin coming from the anus area when you rub/pat it ?
