Ill try to answer some of the questions you had, without spoiling anything specific about the show. The series in based in the same world as the fallout games, but it isnt specifically based on a game. Its set a few years after the events of Fallout 4 (2287), so the show is set in 2296 In the lore of fallout, a Ghoul is a human who has been irradiated so much they become, well what the Ghoul is. They are essentially immortal, but also kind of dead. They still have all their human traits but they look all messed up and are bery hard to kill. Something can happen to Ghouls where they turn feral and loose their mind, basically very fast radiative zombies. I think someone else mentioned it in a comment, but the beginning of the show is set in an alternate future where microchips (which are basically in everything today) were not invented, but nuclear technology took off, so everything is nuclear powered. Nuclear powered cars, weapons, robots, vaccum cleaners etc. Back before the bombs dropped, The USA had been at war with China and communism for a long time and the threat of Nuclear war was getting closer and closer. Culture kind of stagnated from around the 50s, so thr world was a mix of 50s tech powered by nuclear stuff. Then the bombs dropped and everything kind of got stuck that way and old technology is hard to replicate. All the music and stuff that exists is from before the bombs dropped, which is why its all old, real life 30s-50s music. The fallout games take place in a buch of different locations, the main places being California, Nevada, Washington DC, Boston and West Virginia. The entire world is post apocalyptic as far as we know, and people from inside the many vaults were more or less cut off entirely from the above ground. Many different factions and people's inhabit the surface in the US, from the Brotherhood of Steel, to the Enclave (remnants of the US government), the Institute (a underground scientific organisation) and many many more thay may or may not be mentioned in the series. As well as lots of other things that have survived hundreds of years of radiation on the surface among kther things. The tone of fallout is kind of horror/gritty/morbid/extremely graphic and violent comedy. Have yall seen Westworld? That's another Jonathan Nolan production. Falls off after season 1, but it was a nice ride in the meantime.

Although it looks like the 50s in the beginning of the show, it's technically the future. It's an alternate history where technology advanced for nuclear tech not for microchips and computers.
