I was retained as an expert witness in a laser vitreolysis case where a patient had a complication that could have been treated promptly and without consequence. Instead and because of the surgeon's lack of experience, the patient had severe consequences. A long video, but some valuable information for those thinking about laser treatment, or even if you had treatment with your local doctor and had unimpressive results.

The Floater Doctor, James H. There are lot of excellent doctors in your area, but laser vitreolysis is a very specific skill set that is not taught anywhere so caveat emptor.

@@Thefloaterdoctor1 I agree I think your expertise and experience is unmatched probably within the United States, My doctor made no mention of your process but I'm sure she's aware of it, I don't think I'd be comfortable with anyone doing this unless they had the similar amount of hours like you do performing this procedure used to be extremely careful

The cat is a chimera. Two individually fertilized eggs that combine a few cell divisions later. Two sets of DNA. That slit of color right down the middle of the face is a tell-tale sign.

I had cataract surgery on both eyes a couple of months ago (at my local ophthalmologist). The results were GREAT, but recently I experienced a PVD with a Weiss Ring in my right eye. The ring appears in the upper right quadrant of my vision most of the time, but if I pan my vision from right to center, the ring appears in the middle of the macula momentarily. It is then followed by a cloud or loss of contrast as the ring returns to the upper right. The cloud eventually clears if I continue to stare straight ahead. I realize it has only been a couple weeks since the PVD, but I am afraid this loss of vision in my right eye will not resolve any better. What would be your recommendation as far as floater treatment now, or waiting for several months to see if it gets better. Thank you Bob Hibbard

If you are not experiencing peripheral flashes, then the PVD is likely completed. Generally I suggest waiting at least month after the abrupt onset of PVD and longer if still getting flashes. Other than that, there is not advantage, or disadvantage to waiting longer.

I listened all the way through 🙂and enjoyed your story. I live in Ecuador, but fortunatly I have family in the DFW area, so you may be seeing me one day. Out of curiosity, relatively speaking, how much does the presence of a multifocal IOL toric lens affect your ability to treat floaters? I'm 65 and recently had a Tecnis Synergy toric IOL implanted, and have enough floaters to be rather annoyed, although I could live with them if I had to.

I treat through these lenses all the time. I think I just like to complain about them mostly. They might decrease the efficiency a bit, but would not add risk to the procedure.

Hello again, Interesting story you don't in the video. When I was watching it I thought you were going to say that that doctor hit the retina with his laser causing blindness not that he made a miscalculation with the patient's eye pressure. I didn't realise there were other causes of damage to the eye when using the laser other than it striking the retina or the lens.

If the laser were to hit the retina it could leave a very small damage area, like losing a few pixels on your screen or in your camera's sensor. In this case it was much much much worse because it damaged the optic nerve affecting all vision.

Are outcomes with higher pressures correlated with patients who got more shots or is it more of just their eye being more sensitive to the energy?

It is related to the type of floater, anterior location, previous cataract surgery, baseline higher or high-normal pressure which tends to be the main risk factors. It at higher risk, then, 'yes., a high shot count aggressive treatment likely pushed this person over the edge of safety.

Possibly, using gold nanoparticles could increase the efficiency of treatment. It would, though, require an injection into the eye and would not likely allow us to treat close to the retina which is the main concern with younger sufferers.

Does it not cause oxidation when using any lasers for Eye Floaters in the Eyes and causes Cataracts then you lose the natural lens of the eyes, which eventually you will need a new lens replacement?

Laser? No. Surgical vitrectomy though removes the native antioxidants present in the vitreous and causes cataracts very commonly.

I enjoyed your video Dr. Johnson. You are just the same in person. If any of you people think he's not telling the truth about not charging that lady it's true because he did the same for me. I had 3 treatments and a pesky floater that kept moving around and the Dr couldn't pin point my little artistic shard (Dr. Named it that) so since he couldn't get it he didn't charge me. I will go back maybe next year and try it again. My shard is still floating around and bugging me but my other floaters are mostly gone. Thanks again Doc. Vicki. P.S. loved the cat 🙀

I've always been reluctant to talk about the situations where I have refused payment or deeply discounted, because I didn't want it to seem like a marketing gimmick, or risk someone taking advantage of my. In Southern California? You can find many with a laser, but I don't think there are any that have significant experience, unfortunately.

Last month I got a vitreous detachment and now I am with a huge weiss ring in my left eye, it is very bothersome and unfortunatelly the Ophthalmologists in Brazil dont care about floaters. It is impossible to say without an examination. You are at that in-between age for laser treatment.

Hello doctor, I have a question please. I m 31 years old. How do we know if the floater is close to the retina? In the first time I sow it in my right eye, it was sharp and small, after 2 months, it become more big, blurry, hairy and transparent with lot of filaments.

Based on your age, they are likely to be close to the retina. If the floaters have very distinct borders and edges and look like crystal worms, then likely close to the retina.

Now it has become blurry, bigger, more transparent, and like a cloud. olso I started to see a lot of filaments like crystal worms connected to this main floater. perhaps it moved.

My entire medical practice has been based on the treatment of eye floaters mostly with a laser. For good candidates it can be very safe and effective. Younger patients are not usually good candidates because of where the floaters are located (too close to the retina) and for them I have low dose atropine eye drops.

Hi Rizia. These symptoms do not sound like classic eye floaters. Floaters cause 'moving' shadows, clouds, and densities. Double vision, glare etc., are more optical issues from the cornea or lens. Get back to the treating LASIK doctor, or get a second, independent opinion if you are not getting straight explanations. "Give it more time" would not be acceptable to me.
