Why do you use the term horizon line and not eye level line? The terms are interchangeable but I am curious why you use one term over another. Thanks.

For the guy running I'm just wondering. Would you change the head too, to look up and follow the elipses? Or is there a reason you didn't?

yep, the head should change with it, youre right on that one, but you can see that the head actually tilts quite a bit downward so it would need it's own vanishing points and ellipse guides which is really complex. I think he just skipped it as it was too complicated to fix and it wouldn't really be a good example for heads in perspective, a much better example would be the next one he mentioned.
