In this video I go over what the most likely cause of Ford E-Series vans and cutaways being stuck in defrost and or defaulting to defrost is and this "most likely" cause is faulty vacuum check valve and or vacuum reservoir. These are known for failing. Usually the vacuum check valve fails and allows air flow in both directions and the vacuum reservoir develops a crack in it causing loss of vacuum. These components can fail individually or together. Regardless of this it is my opinion that they be replaced together when a repair is performed based on their individual track record. In the video I also explain how I personally diagnose this situation so that I'm more confident in my choice of repair path.

Note: This video is a superficial informational video and does go over what needs to be done to diagnose and repair this concern. However, it does not contain all information and instructions needed to properly and safely remove and replace the vacuum reservoir and check valve. The video is meant to point you in the right direction as far as what could be causing the symptom your vehicle is experiencing and what needs to be done. The use of a repair manual is needed for further instructions. Also, special equipment and certifications may be needed to perform this repair such as an A/C recovery machine and a refrigerant handling license.
