How can a spherical wave move only in x direction? As it is a spherical wave form it should move in every direction symmetrically

You are right. But here the motion of S' reference frame i.e. x',y',z' and t' coordinates motion is along the X-X' axis only and hence the Lorentz Transformations happen for the x' (or x) and t'(or t) coordinates only. The y,z axes remain unaffected that is identical in both the frames.

Yes. It was a mistake done in the time transformation equation, which apparently was corrected (off line) as ( beta x gamma) in the matrix.
