We think that roughly 20% of patients who undergo ACL reconstruction surgery eventually suffer a second injury. How can you prevent tearing your graft or the ACL in your opposite knee?

Great blog. Thank you!
I am a 25-year-old female. I had my ACL reconstruction about 2 months ago using a patellar bone-patellar tendon-bone graft. Physio and rehab has been going really well, and I have started doing more heavy weight exercises. I am constantly worried about my graft failing or stretching because it happened to a friend of mine. What are the signs and symptoms that I need to watch out for which could signal that I’m harming my new ACL?
Thanks a lot!
I think that working hard with a physical therapist on regaining full strength, function, sport-specific activity and more can help you prepare for your return to sports, but it also gives you the best chance to avoid a second injury.

Hi Dr. Geier! I had a bilateral congenital absence of the ACL and suffered from my knees buckling from time to time. 5 months ago I tore my medial and lateral meniscus on my left knee. So my surgeon also did an ACL reconstruction using my patellar tendon. I am almost 4 months post op and still suffering from knee stiffness especially now that it's getting cold. I still have a difficult time going up and down the stairs and still struggle with my running program during rehab. My surgeon said it's possible that my body is also adjusting to a new ACL. I like watching your videos, I find it very informative. Thank you!

I had partial manisectomy of left leg and ACL reconstruction of right leg around one month ago. my concerns are - 1. as around 40% of my maniscus is removed from left leg, what are the chances that I eventually tear the remaining one, and I need another surgery for that ?. generally how much time I can survive with that much maniscus? How can I avoid that? 2. I had ACL reconstruction after 3 years of injury. I had adequate strength before surgery in my knee, now I have very less.what time I should be looking for to be able to run ? thanks in advance :)

Hi I'm Hasan from Bangladesh. I had an ACL reconstruction surgery on my left knee on last 20th January 2018 and currently going through physiotherapy. Till today I achieved around 120 degree range of knee motion and can walk slowly. My question is, how do I understand that the graft is okay and healing properly? besides, what are the activities should I avoid during this healing process? Thank you Doctor David :)

I was afraid of that during my rehab process and even today... I tore my ACL, PCL AND MCL in the first injury, then after few years ACL again. Still struggling with that. I think that my recovering time passed in a fear of that possible injury and that that was the reason that I didn't successfully achieve full potential of rehabilitation...

Hi - I had a combined ACL reconstruction (allograft) and meniscus repair surgery on my right knee on 12/7/2017. I was very disciplined with my PT: 3 times/week for 8 months. My right leg atrophied quite a bit from the 6 weeks of non-weight bearing (for meniscus repair) and from the moment I could put weight back on, I would feel a slight "separation sensation" with certain lateral movements. Sometimes, as I gained strength back, it went away during a particular exercise. For example, with the sliding board: I felt the sensation for the first week of the sliding board, then it went away. I went to formal PT for 6 months post-op and then continued on my own. 8 months post-op, I felt this sensation again when doing a jumping jack. My orthopedist ordered an MRI and everything came back normal. I thought maybe I hadn't prepared myself well for agility drills and it was weakness in my leg and decided to return to PT. I went to a different PT from before (better location). At 9 months post-op, I was performing an agility drill I had performed often: jumping in an L shape. I would hop from my left leg onto my right in a lateral movement, wait 1 second, then jump forward from my right onto my left -- thus, forming an L shape. One day, my PT said to go faster. I performed 2 fast jumps on the right side (landing on my right and immediately pushing off with my right leg and landing on my left leg) when suddenly, my knee buckled. I felt this awful crunching sensation. Pain and swelling ensued and an MRI report showed that I had torn my meniscus. The orthopedist who completed my surgery said the cause of this incident was the meniscus that never healed, even though the MRI report from 8 months confirmed healing. A second orthopedist, who confirmed that I have objective instability in my knee, said that the allograft was to blame and that it had somehow stretched and thus, was causing instability. A third orthopedist said that the placement of the graft on the femur was too vertically aligned -- that the graft should be inserted into the femur at more of a 45 degree angle -- and thus, the ACL is not doing its job in providing the necessary stability for lateral movements. And finally, a fourth orthopedist, said that unless my pain is unbearable, not to undergo revision surgery or meniscus surgery and instead, focus on strengthening the quad. I was also wondering if my PT perhaps progressed too quickly? Maybe my strength was not good enough to be doing those kinds of agility exercises? If the answer to this question is yes, then it doesn't seem that revision ACL surgery is necessary and I can instead focus on strengthening my knee (and maybe have my meniscus repaired or taken out?). However, if the answer to the question about strength is no (that strength was not an issue with this agility movement and the ACL is, for whatever reason, simply not doing its job), then I should look to have revision surgery (and fix the meniscus). Thoughts?

I also experienced all the things that you have shared. I had my surgery on 2015..n 2017 a started actively doing sports then it happened the you had described. Now i am on my way to do my second acl surgery. I had two orthopedic diagnosed my knee differently. The first one said that my knee are too vertically aligned so..its not doing its job. The other one said that..i might have tear my acl. I m doing arthroscopy on my knee next week to repair my left knee meniscus..and to see if i really had torn my acl. I hope i get to know your story as well since we have similar problem with our knee.

what is the percentage of re-tear possibilities after you have gone through all the proper PT, specific sport training and have been released to play? it's been two years and some month since the reconstruction and most recently I cut, heard a smaller pop and my knee swelled up. However ,MRI shows no tears. Any thoughts?

Hey Doc, thanks for the video. I had an ACL tear in 2021, rehabilitated it, and got it stable.. In July 2022, I had a reinjured the knee and therapy failed. So I guess I will get cut. My question is about the graph site. I have two options, the patellar or quad, with the Dr. Recommending the quad. My therapist says people she has seen with the quad graph has issues with getting the quad activated. Is the quad a good choice that won't mess with my quad strength? Thank you

Thanks Dr. David. This video is informative. I had a ACL REPAIR last year in October. The rehab went very well and i recovered really fast. However I tore the ligament again and then I had my ACL RECONSTRUCTION little less than 2months back. However I find my operated left knee little loose compared to right knee. Is this something I need to worry about ? I asked my doctor and he said we can wait for hamstring to gain the strength back. However I am worried that even after recovery this looseness could cause trouble in future. Please let me know your thoughts. Thank you.

Many people can play at 10 months. Your surgeon and physical therapist should be able to guide you. Good luck!

Listen to this man and also. Get your leg strength back. Do LOTS of machine hamstring and quad strength. hamstrings hold the knee in place. And also do mad squats. Get powerful legs in the meantime

Hello Sir,I'm 21 years old got a reconstructed ACL.The surgery was done on the year 2015 & I used titanium screw (cannulated interference screw) for the same..Then I came back to my sports life & I was doing well.Now after 2years of the surgery I am feeling little problem on my knee & found after doing a X-ray that a screw has little scrolled down from my tibia..Now if I decide to remove those screws,is there any chance to having problems to fix my ACL or its already fixed on it's place ?? Sir,my question is, 2 years is enough to fix my reconstructed ACL on its place or not ?? Help me with an answer,Sir.

How about that thing called 'stem sells injection' does it really work? Is it better than reconstruction surgery?
@eliwhitesell5415
@eliwhitesell5415 6 жыл бұрын
Hey man, as long as your complete tear is non-retracted. Also make sure you go with stem cells that are your own stem cells as they are completely safe and work like a charm. I tore my ACL and meniscus and had a procedure and the MRI shows almost complete cellular regeneration. It worked for me so I hope this helps.

Hi, I am 3 weeks post opp of a right knee ACL Reconstruction. Last night whilst asleep flat on my back, I pushed my leg out to extend it straight and I felt part of my knee moving back into place, as though it was out of place somehow. This followed sharp pain which lasted for a few minutes and subsided. I wasn’t standing and nor was I pivoting, but worried as it was very similar to when the knee would give way before surgery and cause pain. Do I need to be worried? Does it sound like the graft has failed? I will see the surgeon and PT when I can get an appointment but any advice or support will be very much appreciated. Regards,

I injured my knee playing Football/Soccer 3 years ago and tried my very best to get by without surgery. My leg was never 100% stable, but around 6 months ago I stepped off something around 14” tall and my leg give way again, it felt as though something else had happened, it had!!!! Now I had severed my ACL completely and damaged my meniscus, at this point surgery was my only option especially only being in my mid 30’s. Hoping I may be able to play sports again I had surgery 6 weeks ago(Hamstring Graft), the surgeon opted for a no brace policy. My question is, is it normal to feel a tightness, almost pressure and pulling across the centre of my knee from left to right when walking and is walking the stairs without crutches OK to do?

I did sports specific rehab w cardio, lots of squats and deadlifts and still retore my ACL due to surgical error. Now I have to have another surgery and i will never play sports again besides biking. I’m 20 and I want to make sure I never have to have another knee surgery cause if I do I will go insane since all I’ve ever done is sports and play outside and this injury is just a sick reminder of what I can’t do anymore. If I never play any sports that require cutting, build my strength and coordination back, is it reasonable to expect to never injury it again? Besides a freak accident of course. There’s no way I can go through this a 3rd time. I’m struggling to even do it a second time. I wish I knew how easy it was to retear.....

Thanks Dr. Thank you.
