I was around 50/51 bmi. I had the GB almost 8 months ago. Went on Keto about the second month after. Started at 315, I am now 189 and still going strong. No complications. Best decision I ever made.

@@Dweller415 It has now been around 1.5 years. I lost a total of around 140lbs the first year. I have been able to keep it off. No major complications. I have been lifting weights for a while now and I have put on some decent muscle. Still doing healthy Keto.

This is the best video I've seen explaining the differences. I'm scheduled for gastric bypass on 1/29/20. Acid reflux and the diabetes plus sleep apnea have been the major concerns. He really broke things down.

@@melanietorres3287 HI. I'm doing great. But will be honest with you. Right after surgery I was miserable from the pain. And for the first few days I kept wishing I never had the surgery. But that passed. Now I'm down over 100 pounds and feel great. On January 22 I was 327lbs. Day of surgery I was 316 lbs. I still weigh myself once a week. Right now I'm 225 lbs. I can't even wear the clothes I use to. The people I didn't tell I was having it done still ask me if I'm OK, lol. They wonder if I was sick. But physically I feel great. I use to have sleep apnea and needed medication to sleep through the night. The apnea was instantly gone. Im a tall guy so I actually have not been trying to lose any more. My goal weight was 235 to 240. I thought it would take a year to get there. I got there in four months. But the strangest thing is you have to learn to eat again. Kind of just like an infant.

@jonathan Collins. 100 lbs in 7 months ???? Wth . All I want to lose is 100 lbs . Or at least 80. . I don’t want to look sick . This is what scares me . I look sick at 180 so I can imagine going down to 160 . I don’t want to be looking. All disproportioned and skeleton like. Will you keep loosing. Is it something than can be controlled? @@melanietorres3287 I don't think I look sick at all and my skin is more clear because of not eating the junk. Its just that I was such a big wide guy, people thought something was wrong. The one good thing that's been a blessing is I don't have a lot of loose skin. Its more like my body just slimmed down. But I could use some firming up. A close friend who I did tell before surgery said she was surprised I was getting the surgery. Even though I was big, I looked more like someone who lifted heavy weights and ate a lot. But my health numbers were terrible. My doctor suggested becsuse every appointment I seemed to be getting heavier and heavier. It never ever crossed my mind to get surgery. But I think the end results are well worth it! I think you won't be disappointed.

1. Gastric bypass - I have diabetes so I think the bypass plan would definitely work for me. 2. Gastric sleeve - My very first option at the beginning was the gastric sleeve, but there is empirical evidence to suggests that the stomach can stretch back to its' larger-size' if you go over the number of calories. which seems like you have wasted your money and you are back to get square one again. 3. I heard that the gastric band is very effective however, there are risky complications, the band has fallen out and has become stuck in the body, or it may be to tight around the stomach causing eating and breathing problems, you may have to go back and forth for a check-up which may seem like a headache!! I would not reccomeded the band, it only works for a small period of time, please don't waste your money, time, and energy, in fact, the 'gastric band' should be annihilated.

The lapband is taking the same road as 8 track tapes did 30 plus years ago. It seems like a waste of time doing that. Go for the VSG Vertical Sleeve Gastrectomy or Gastric bypass. Best of luck to you all in your decision.

It's very simple - you don't lose enough weight. That being said, many surgeons frequently perform sleeves on patients with BMI>50.

I am also opposed to the lap band. I had the lapband surgery eight years ago. To start losing weight I needed multiples fills at a point that I was able to drink my meals only twice daily. I lost 60 pounds and kept off with the liquid diet. After 8 years the band slid and had to be removed. My stomach is 5 times bigger than a regular person. My doctor, after the surgery, said goodbye and good luck. He knew was going to gain a lot of weight back and refused to do a sleeve. Don't accept the band!

Your story is very common. Just because your surgeon could not do a sleeve at the time he removed your band does not mean that it can't be done now.

Hello Dr. Weiner, thank you for this video. Very informational. I came across it while searching for other videos. I am in the early stages of researching the best surgery for me. I was trying to play a DVD from Health Dialog on weight loss surgery but for some reason it would not play on my laptop?? (Luckily I have their book). I tried finding their information here on KZfaq but to no avail. I am 52 years old, 5'6 1/2, and I weigh close to 335lbs. I have high blood pressure, high cholesterol, moderate sleep apnea, edema, swollen feet, swollen ankles, swollen legs, and I am experiencing back discomfort, well, I am not able to stand that long when I cook, clean, or even just walking short distances. (and much more). I am considered "Morbid Obesity." At first I thought the band would be perfect because I don't like the other illustrations for the other procedures. As I watched, I'm thinking the GBP would be the suitable choice. I know that you can't diagnose online but it just feels like right choice at this time. I've been battling my weight mostly since I was in my 30's. I would loose like 75lbs, gain it back, and then some. I enrolled in college in late 2016, I lost a lot of weight prior but again a second time I gained it all back, and then some. I am the heaviest that I have ever been in my life. I told myself that surgery would be my last result. 75% of me wants to do this and the other 25% does not want to. I will say, I am kind of scared.

I had the gastric band put in 2014, a year later I was swollen, in pain and constantly vomiting with nausea. I went back to healthier weights and told them I want it out they told me to pay £5000 to take it out. I spent £7000 putting it in. Dr Aston told me to get the band instead of the balloon which is temporary. He didn’t give me the option of the bypass. 2020 I’m going next week to get the bypass and the band taken out privately. Thank god 5 years suffered and struggled with the pain, reflux, vomiting. 2m ago I was diagnosed with diabetes. With a 17m old son I have to look after myself so I can be a better mum to him.

I had the mini sleeve done back in December of 2017. At my heaviest I was 335lbs. I was told by my doctor that with this operation I should be able to reach a weight of around 225. I never did the lightest I got was 269 and I was going to the gym Monday through Friday doing cardio for one hour and another 30 to 45 minutes of weight lifting. And as time went by I was able to eat more and more and vomit less. My stomach had stretched and so here I am back to bn big again. I now weight about 288 or so ofcourse this year with covid didn't help but still doctors should be more honest and tell you this important thing that my stomach can stretch again had I known I wouldn't of have done it. I would want something more permanent. Or atleast for us the patients to know all the truth. So would I recommend it no I wouldn't.

Please don't get the gastric band (Lapband) unless you are super extremely disciplined, type A personality who loves to exercise and eat like a saint! I had the gastric lapband put in eight years ago and it has done absolutely NOTHING for me! My stomach was never "fooled" by the band and by eating a smaller amount... I was hungry all the time no matter how many fills I had and how tight the band was. In fact, lapband surgery can cause very poor eating habits as you try desperately to make it work and you think a magic "fill" will make a difference and make it work like you hoped it would - then the band is too tight and you cannot eat hardly anything solid, so you start eating chocolate and ice cream...anything soft that will go down to give you some calories! I think the band did more harm than good for me personally. You are always searching for this elusive "sweet spot" that is never there for the majority of us. Again there are a select few that it works for but of 3 people I know that it has worked for them they all workout 2-3 hours each day and religiously portion their food - they are food Nazi's and extremely disciplined people! If you are a person who is human and make mistakes then reconsider! I am now going to my doctor for revision surgery from the lapband to gastric sleeve. I hope this review helps someone with their decision.

My dear friend had the lap band flip on her (it turned her stomach inside out) she was vomiting blood and ended up having emergency surgery to have it removed. Her previous Dr says he will not perform any other surgeries on her.

Dr. Weiner, I came across your channel while researching my revision options after having the lap band removed in 2017 (it was placed in 2010) after developing GERD from the band, terrible eating habits and vitamin deficiencies secondary to the poor eating habits (could hardly tolerate healthy food only ice cream and sweets). I am 5'2'' and the heaviest I have been in my life at 199lbs. I have moderate sleep apnea and metabolic syndrome. My question is, do you have a video explaining the pros and cons of both the sleeve and the bypass after lap band removal? I do not have any scaring or esophageal issues (per a recent upper GI series). In your experience, what is the best surgery? My husband had the GBP in 2007 and he did great on it. Only issue is that during the first 6 months post-op he looked emaciated. I am leaning towards the GBP but fear that I will loose too much weight (bet you haven't heard that concern before). Is that a thing given my wt and height? Thanks so much!

I've gone to my first couple appointments and am needing to make my choice this coming week between VSG and bypass this video kind of helps me. My surgeon suggests that I get gastric bypass because of my current weight 325 and because I have history of GERD (not so much in the past year and half) but I've had 3 csections will that impact having the bypass? If not I'll probably get the bypass 1) because doctor suggested 2) because of history of GERD 3) evidence of long-term success And that 3rd reason is BIG for me I've done weight loss X3 in my life first time I lost 60lbs second time I lost 80ish lbs and 3rd time over 110 lbs but each time I eventually gain the weight back plus some and at this point its become harder for me to lose any weight and being active is getting really hard (painful) 😢 I have 3 young boys and I want to actually LIVE life with them not hold them back or just watch them from the side lines so I'm going to push myself and hopefully having this tool (whatever one I go with) will help me meet my goal and stay there. Lowest weight since I WAS 13YRS OLD- 168lbs Highest weight- 325 Good luck all!

I have a BMI of 35, acid Reflux that disappears when I get leaner, also high collesterol. I was thinking of the Sleeve but my doctor is pointing me towards the Gastric Bypass, but i heard there are a lot of things that you can't do after gastric bypass since there's no longer a path through the stomach, such as medication that works in that organ and also I was told that alcohol and sugar are not advised at all after the procedure (even years after)

+Debra Shea the criteria is a bmi higher than 35 with a medical problem, or 40 without one for insurance coverage. I perform around 2-3 sleeves per year on patients with a bmi less than 35, but they pay for the surgery themselves, rather than go through insurance.

i am curious doc about the gastric bypass i am a type 2 diabetic for 14 years i also suffer from gerd for many years.. i read that the gastric bypass cures or puts diabetes in remission for about 84% of diabetic patients i read since i have been a type 2 diabetic for so long and i am on pills insulin and trulicity diabetes may not go into remission but will get better.. in your practice have you had patients like me and what were there outcomes...

Which surgery is better if you still want to be able to not he limited to a super small helping of food I.e thanksgiving and occasional work Gatherings? I have a BMI of 40 with a mild case off sleep apnea and my doctor recommended the gastric bypass, but I am Leaning towards the sleeve after watching numerous videos. The other hiccup in this is my insurance just approved the gastric would I be able to change to the sleeve?

jody scott my friend was 430 lbs at around 5’10, and he went down to 260 with the sleeve, so it definitely is very competitive versus the gastric bypass, he went from like a 61 BMI to a 36-37 BMI, a huge difference, and he’s already two years out, so it works, and he’s not even a exercise kind of guy. I’m 305 lbs at around 6’1, from a max weight of 325, and I’m getting the sleeve in a few weeks, I’ll be happy at 205, toning by body the best I can along the way.

Im supposed to be getting my surgery next month im 32 yrs old starting this process i was at 280lbs and today i am at 254lbs i have pcos my nutritionist put me on a low carb diet and im doing well on it. My reason for getting this surgery is my rapid weight gain but i also want to be a mom . My surgeon feels that the sleeve is the surgery i should choose. But im leaning towards the rny. Ive had friends that have done the sleeve and a few years later they have gained the weight back. Any suggestions..

I share your concerns about weight regain after the sleeve. Consider the rny, but ask your surgeon how many he/she does, many surgeons have moved over to only sleeve and don't have as much experience with rny

If a person starts their journey at above 50 BMI but loses enough weight before the surgery to be below a 50 BMI do you find that the surgery still has the same success rate or do you find the stats stay the same and the person still doesn’t lose enough weight after the surgery? I have the lap band currently (approximately 8-10yrs). I had to lose 30lbs to qualify for the surgery and lost about a total of 60lbs all together. I plateaued and maintained for a long time but now Im over 330lbs and Its extremely frustrating. Im looking at having revision surgery because I have alot of side effect of the banding and none of the benefits. Ive been thinking about getting the sleeve in conjunction with the revision surgery, Im terrified of removing the band and gaining even more weight! But at the same time I think the band is causing harm.

It's not big in the US, but it's a reasonable surgery. My understanding is that lots of patients have discomfort from the reflux of bile into the gastric pouch, something that doesn't happen with a standard gastric bypass

Hi I had mini gastric bypass in Australia in August. I’ve lost 27kg (59lbs) in 4 months and have had no reflux but take Somac each night and it is my surgeon’s protocol to take for 6 months post surgery. I did have some reflux prior to surgery but during pre surgery endoscopy it was found I did have a hernia which was repaired during my mini bypass procedure. I’m very happy with the procedure I had and the results are amazing!! Dr Weiner thank you for your vids, they are very interesting and informative.

Are there any other options available? The reason I ask I am looking for the option that has the least malabsorption competent to it. I have to go back into surgery to fix a slipped wrap and since I have to have surgery again I would like to have bypass surgery done at the same time. I am a hard case but from the research I have done not impossible, when I had my GERD surgery done in 2015 it was found I have liver cirrhosis, I am compensated, I also have type 2 diabetes which I take no meds for but am noticing my numbers rising lately which is not good with the liver disease. Since I have CVID I am not keen on having anything placed inside me. I have struggled all my life with weight I can lose it but I always end up gaining it back, I have found I am very carb intolerant but I am also finding now that the higher fat intake from doing LCHF is working for me anymore I just can't handle it but to increase my carbs rises my blood sugar. I really do believe that at this point to keep my blood sugars under control and to keep my liver compensated for as long as possible I am going to need weight loss surgery, my liver is so fatty right now. Which option do you feel would be the best option for me?

Hi Dr. Weiner!, I am close to the date of my surgery and I have decided to get the Gastric Bypass, mainly because the long term data and the better results in weight loss, but now I am having doubts.. my surgeon recommends me the sleeve but he said that at the end its my decision, which surgery would you recommend me? Glorimar, I can't give you specific advice without seeing you as a patient and it's important to remember that I won't be the one doing your surgery, so I always defer to the operating surgeon. If you're having trouble with the decision, it means that you're giving it all the proper consideration since it is a very hard choice. Best of Luck.

Huh. This video has me second guessing all the research I have done. I thought vsg was the best option for me because of its minimally invasive method (“keyhole” surgery) because as a cancer survivor I have massive nerve and tissue damage, and most of my right abdominal wall is now made up of mesh. BUT, this damage also leaves me with chronic pain because of the nerve damage. So I have a significant mobility issue. I used to have a very labour intensive job and obviously ate less because I was busy working, but I’ve put on at least 50 kgs (over 100 pounds) since I got sick. I don’t have diabetes but I do have limited mobility. I was fairly certain that vsg was the best option for me but now I’m wondering about it. I will be presenting my “case” to my surgeon at our next regular check up in hopes that he will agree to sign off on weight loss surgery but he’s very reluctant to operate on me unless it a necessity as I had near 40 surgeries while I was sick because of massive complications. I need to have my facts straight and present a good case to my surgeon so he will agree to surgery. so, the fact that I have an extremely complex surgical history suggests VSG is the best option, but my limited mobility and obviously slow metabolism (no matter how well I eat I can’t seem to get any weight off) suggests the gastric bypass is more appropriate. I realise that it is not appropriate for you to give specific medical advice and I will absolutely be discussing this with my own surgeon but I need to present a good case to him from the get go so I am wondering, what would you recommend for a patient with no diabetes or signs of diabetes developing, slow metabolism, limited mobility and complex surgical history? I ask purely so I can present my own findings to my surgeon so he knows I’m absolutely serious and have put serious thought into the effects it will have on my life. I have bmi of over 50. 5 ft tall and about 140 kgs (around 300 lbs I believe). I’m only 23 years old but have had two children (if that makes a difference) sorry for the rambling message, and again I’m not asking for specific recommendations from you but more a general approach on what you believe would be the best option for a patient like myself. Thanks for your time in making this video.

Surgical history trumps all other factors, you are probably only a candidate for a vsg

@@apoundofcurewow what a blast from the past I was at the very start of my weight loss journey then, it ended up taking so long to get in to see a weight loss surgeon because that's just how it works in my country but while I waited I watched and learned and lost 36 kgs on my own, then quit smoking as part of my pre op, had the sleeve surgery and lost a further 38 kgs. I started my journey at 146 kgs and now sit happily at 72 kgs 😁 a total of 74 kgs lost and I'm a whole new person (we joke that I'm half the woman I used to be!) I get picked on some times and told "oh you're one of those" when I talk about carbs or calories you really can't win with some people it wasn't okay that I was fat but now it's not okay that I read all my labels and limit my calories lol 🤷 I'm super happy though I feel great I only wish my flaps of loose skin would disappear as they ruin my self esteem. I'll learn to love it. Thanks for commenting again I probably would never have seen this again otherwise.

Dr I am considering having this done been approved but it all seems a tad crazy to me. The problem isn’t my stomach but my brain. And the surgery isn’t a lobotomy.. I heard in the group sessions a patient was sent home after surgery and blended a hamburger. His stomach exploded and needed emergency surgery to fix it. Also the sleeve is absolutely insane when in fact they remove 85/90% of their stomach because it’s almost a full stomach amputation. What about the Ghrelin? They call it the hunger hormone in the group sessions but I’ve found out that it also contains human growth hormone that body builders call the fountain of youth. Is that why when people break something it sometimes takes them 3 years to heal instead of weeks/months. Wounds heal slower and so on. Isnt the part of what they remove called the Fundus where Ghrelin is produced, they say you will produce some of it again but it will never be the same amount as it was. I’m doubting this all. I weigh 147 kg ( I’m from the Netherlands so it’s kg) and came from 161 I lost a few kg on ozempic. Isn’t it better to lose the weight naturally? They say with bypass you don’t take up nutrients as good and vitamines. They warned us to take it serious because someone who was operated didn’t take his vitamines and his spine completely collapsed😢 How about the nourishment when people get older? And boneloss? Also what if a patient gets cancer and can’t digest the medicine properly? Does he/she get ulcers because they need to take heavy medication 💊? I’m asking All this not to criticize your work but I’m generally considering surgery but want to weight out the pros against the cons. I find some people may loss the weight but end up aging immensely is that because the lack of Human growth hormone? And how is it possible so many lose their teeth due to boneloss ?when they first hade good teeth I saw this woman on KZfaq and found many more on Facebook. There are all kinds of groups on Facebook talking about the complications after weightloss surgery. Im in need of a good advice and honest opinion on this topic from someone who is experienced. Is it possible for me at 147 kg and 41 height is 1.72 cm and a female to weigh 72 kg and lose the weight naturally? Or is weightloss surgery my safest bett? Thank you greetings from The Netherlands

Bariatric surgery should be considered a last option. If you've tried everything else to lose weight and you just can't lose and maintain weight loss, you consider surgery. I have a book called A Pound of Cure. In the book is a diet called the Metabolic Reset Diet. You may be able to lose the weight without the surgery if you eat according to this eating plan. I am 33 and 375 at 6 foot 2. All of a sudden in my early 20's I started gaining weight. Still no answers why. I went from a size 14 to 24 in 6 months. I was walking 12 miles a day plus working 8-16 as a nurse. I should not of gained that much weight that fast. Also around that time I started experiencing extreme fatigue, heat and cold intolerance, concentration issues. Now I have a diagnosis of lupus. I've been trying for 3 years to lose weight due to allergies I have very limited diet I mainly eat fish and chicken and vegetables. I still have a healthy workout regimen I ride an exercise bike I on bad days put in 10,000 steps a day on good days I can hit 20 or 30 thousand steps. There's no reason why I shouldn't be able to lose weight it's very frustrating and my doctor wants me to after I get my gallbladder removed which will be in 11 days go and see people at the bariatric Clinic because he said that without my gallbladder is going to add more stress and strain to my body and most likely I am going to end up gaining more weight after losing my gallbladder. I'm scared I'm worried about excess skin I have a lot of Scar Tissue on my skin due to accidents and abuse my skin doesn't rebound like it used to I have a history of skin infections lesions and MRSA. I don't want to lose a ton of weight too fast and then I'm not going to be able to afford to get rid of the skin Tom. I'd like to do is slow and steady that way so my skin has a chance to regain some elasticity so it can go back to some form of normalcy

duodenal switch has a 99% cure rate of diabetes while the bypass is only 84% still good but not as good as the duodenal switch..

I have. You just have to weight a little to be able to eat enough protein. But before that you should be focusing on losing weight anyway. You have the best weight loss the first year after the surgery. You should not be wasting that opportunity. Focus on getting used to eating healthy, while losing a ton of weight (not so bad, lol).

Ask your surgeon what percentage of his/her surgeries are Sleeves? My guess is that it is the only surgery he/she really does. If you go to a Ford dealer, they're not going to recommend a Chevy...

It is not cosmetic surgery; it is covered by many insurance plans. It IS an "elective" surgery but not cosmetic.

My mother died from Gastric Bypass, I don't recommend it at all! She was 86lbs before she completely died from hospitalization. I had to see her going from healthy to a total skeletal woman, gave me trauma for the rest of my life. Please don't get Gastric Bypass!
