Choosing a track for your MSN is confusing. Can be agonizing if you have paralysis by analysis like myself. In this video I discuss the program and specialty options with a focus on the age-old ACNP vs FNP debate. I talk about the LACE consensus model and how that changes certification, and by default, where you can practice.

Great content! I think a great but unpopular choice is nurses becoming pa’s instead of np’s. It may not be as smooth as becoming an np but it gives you the most lateral mobility eliminating time and commitment of going back to school again. If independent practice or entrepreneurship is your forte then I don’t suggest becoming a pa.

Bree, I am so glad I’ve found your channel! I’ve been a RN for 10 years in an acute care setting and have been thinking about going to NP School for about 5 years but I wasn’t ready for the added responsibility until more recently. Now I am researching schools and getting ready to apply for the next fall semester in an AGACNP program. I have watched so many of your videos and truly find you very inspiring and intelligent! I think of you as a role model in the profession and thank you for your content!

I came across your channel by googling things related to acute care NP which lead me to some KZfaq videos then after watching many different Influencers on KZfaq I found one of your videos which I really liked and continued to watch your videos as I feel that you have the BEST content related to my interest! I am almost obsessed with preparing myself as much as I can to be successful in getting admitted into my local university program and preparing myself for beyond the program. Thanks again for providing support and insight into the advanced nursing profession.

Thanks for the video Bree I felt like you were talking to me directly hahahaha....I've been accepted into both fnp and acute care programs for this upcoming fall. I'm currently an ICU nurse, initially my goal was to become dual certified and that's what I ultimately think I'm going to do. I'm just debating which path to pursue first! Decisions....

Thank you for this video; it has given me a lot of education about the difference between the ACNP and FNP roles. I just started my FNP classes last summer not really understanding that ACNP was even an option. I don't regret my decision to start my FNP, and I'm hoping I enjoy it, but it's good to know there are other options in case it's not for me.

Wonderful video, very insightful and thank you for that. I’m 45 and start my BSN program in a month. So I’ll be probably around 50 if/when I go to NP school. I figured FNP was my best choice given my age, but feel very drawn to acute care. I wasn’t aware there were so many options along that pathway. Luckily I’ve got a ways to go before I make my choice and I’m sure I’ll have all kinds of experiences as a student and as a new nurse that could change my direction. But thank you for opening my eyes to the many possibilities for ACNPs.

Yay!! It really is the best profession, you get to lay down at night with so much reward! Congrats on making the decision. I agonized sooooo much being older and going back to school - all for naught. Many years still ahead to make it worthwhile and honestly looking back it was the best hung for my kids.

I stumbled upon your channel via your interview with the Respiratory Coach... I'm really enjoying all of your content so far!! Thanks for helping to shed light on this topic. I'm a newer nurse (1.5 years) working in a PCU (MSD) at a level 1 trauma center... will likely head to the MICU in the near future. Prior to working in the hospital, I always envisioned myself as a primary care NP, practicing in the functional/integrative/precision medicine realm. While I still think this is my ultimate path, I've become increasingly interested in the field of critical care. As you mentioned, I too tend to overthink future possibilities and hypotheticals, but I'm hoping to start an NP program in another year or so. Are there NPs that work in a private practice setting and also in-hospital? I'm mostly only interested in working with an adult population, but my divergent interests in primary care and critical care seem to be odds with one another.... perhaps time in the ICU will make things more clear? Any advice on how to narrow this vision or potential NP focuses that may be able to fulfill both?

Yeah that's a tough one Justin. There are plenty of people who have their acute care and work for private practice within a specialty (surgery, ID, nephrology) who do both clinic and in-house rounds. But you have two very different roles you are looking to fill. Primary Care education is very different than AC. I would suggest starting with what interests you the most right now. You can always go back for a post-cert in 2 semesters if you decide it's not for you.

Prior to nursing, I was a personal trainer and health coach for about a decade, so prevention and health promotion are core to my values... hence the interest in primary care. Given this, I never expected to become so fascinated by AC/CC. In addition to time in the ICU, I think I need shadow a primary care NP to discover if this path feels like a true fit. Thanks again for all of your educational content and time in responding.

I just applied for NP school. I would like to stay in the hospital (acute care setting) very much but I have very limited ICU experience, mostly PCU and telemetry.

I’m actually interested in both. I have always wanted AGACNP but wanted FNP in case I wanted to do something different later on. I actually found a school that offers a dual program for both certifications but I’m worried about working in hospital setting for years then later wanting to work outpatient as FNP and needing to brush up on skills.

I’m very torn on which route to go. My experience is in the icu. On one hand I want to specialize in Lyme disease / chronic infections / chronic illness due to personal experience… on the other hand I want to continue with the Icu

It’s tough. Big agony time for me as well. So many interests, who knows what I’ll like in 10 years… start with what speaks to you now. You don’t have to have it all figured out. Just start somewhere and follow where the journey takes you.

I sm glad I found your videos. I am a AGACNP-BC with ANCC. I am struggling getting hired into a critical care setting. I have 16 years ICU experience. Please do you have any advice getting into the ICU as an AGACNP-BC

It can be tough. you likely are going to have to start with a specialty and get to know people in the ICU. Lots to be said about word of mouth. Or look at a less desirable hospital/shift. Or move.

@@BreeJuskowiak I’m surprised to hear this comment…is this common Bree? If so why is that? I know a hospital system I use to work for in the cvicu for mid level providers they usually went with pa’s but that’s not to say there were no nps…just not as many.

@@goldenopportunity31 some regions are quite saturated. Supply > demand and icu is a narrow niche so not quite as many positions available. I still do not think this should be a reason not to pursue it if this is what you want.

Can ACACNP’s work in Emergency room observation units? Or would that be an FnP?

AGACNP definitely can. I don’t think FNP. FNP in ED setting does fast track/urgent care type of work. Not inpatient care. I get that it’s obs but it’s still in hospital care.

I started off in an oncology clinic as an RN and I really enjoyed my time there. I saw NP's work there and it instantly came across as a position I would happily take on. Does that mean I should be an FNP? It seems like FNP has a lot of options, but I would like to have the option of working as an NP on a trauma floor in the hospital or acute care.

As acute care you would have the option to do either hospital or specialty clinic including oncology (just not kids).

@@BreeJuskowiak I did just now! I immediately love the answer she said of more autonomy in oncology. I noticed that too with the NP's I worked with. It does sound like a lot of studying and mental work so I might do the same track of working in a hospital setting first. Get some experience before settling into outpatient oncology. But I'm glad that I can do both with a Advanced Care NP rather than FNP.

Do acute care Np have to work in critical care? Can they work in non critical specialties like med surg?

No, they can work in many settings both inpatient and outpatient. As long as it’s not caring for mommies/babies/children or doing primary care. Think: is this an active/acute problem versus health maintenance. You can not anywhere that is considered a medical “home” for patients. Like health maintenance and promotion. You will be caring for people with a specific problem or acute decline of many problems. All of the interviews on my channel (with exception of those that are labeled fnp) are example of what acute care NP’s can do inside the walls of the hospital.

Im struggling with finding which specialty. I need a remote job due to health issues which makes me lean towards psych but my true passion lies in chronic illnesses but FNPs make the same as RNs in my state and it is oversaturated so not worth the return of investment :(

Strongly recommend state schools near you so try a good reputation. Easier clinicals placement, some states offer a tax incentive for local schools which make a good “selling” point for finding preceptors.
