How to Create a Fillable Form in Google Docs

Learn how to create fillable forms in Google Docs including form elements like text fields, dropdown list selections, and checkboxes. In this step-by-step tutorial we will create a fillable form in Google Docs from scratch. See how to insert and place a logo on your form, then how to use tables and borders to create fillable text boxes on your Google Doc. You'll see how to insert and set up dropdown list selections and even format the color options. Then you'll learn the two types of checkbox checklists in Google Docs to either tick or select a checklist item, or how to set it up to cross out the list item selected. Finally, we will create a large open text box on the form.
It is important to note that fillable forms in Google Docs are not able to be protected or restricted to filling in forms, which means users may inadvertently edit, modify, or even delete parts of the document. If you need to protect the form, you'll want to convert it to a PDF and add PDF fillable form elements, or create the form in another tool, like Microsoft Word, Google Forms, or Microsoft Forms.
Also, unless you have Google Workspace, you won't be able to save your Google Docs form as a template. However, you can make a copy of the form file each time you want to fill in a new form.

This is Great, I learned alot!! I do have a question, how can we create a conditional value, so for example if applicant chose the Region to be East, then only eastern states would populate in the next drop down value, so that we don't get a submission where they have chosen Eastern Region and California as the state. it is not allowing me to select multiple segments of the table while holding control. Do you know if this is something that could be fixed? Thank you for all your help

Thank you for this tutorial. My question is once Im done making my form, how do I save this and in what format, in order for my clients to complete this form. I emailed it to myself and it didnt work to complete it. The drop downs didnt work or the check marks didnt appear.

Hello - if you create this in Google Docs, it will save as a .gdoc and the functionality may not work if you open the document in Word or another application. This is best if your users will also be opening in Google Docs. However, if your users use Word, it is best to build the form in Word, or as an online form (either Google Forms or Microsoft Forms).
