HOW TO CREATE FILLABLE FORMS IN EXCEL // Learn to create a fillable form in MS Excel using interactive drop-down lists that populate text in other areas of the form. See how you can pull in data from another table using VLOOKUP and how you can program conditional text to appear using the IF formula. "Shawn Spencer - Communications" LOL! Love all the Psych callbacks. You didn't give poor Lassie a title. :D I was looking for a form to input data into a spreadsheet for later data analysis. In the case of your employee engagement survey, you'd want to record the responses. For my needs, I want to build a db for my beehives. Inspection date, queen present Y/N, Honey yield, that sort of thing. Something that can easily be manipulated with a tablet out in the apiary.

Sharon, the information and presentation are Amazing! I started into a project and you answered MANY of my questions in this one video, and it just happened to be the first source I found. I do have a question on how to disseminate the form to at team an then retrieve the form ideally via submit button that sends form/file to admin with time stamp). Is this possible utilizing a Microsoft 360 account? It would be great to give users unlimited access to the blank forms, then once its submitted have the finished form locked, but accessible to the user and the administrator.

@Sharon, this is fantastic. I do have a question though. I created a form using Microsoft Forms, I love the branching capability. However, I don't like a lot of the issues with compiling results and my process after receiving them. Is there a way to have multiple lines populate based on the drop down answer? So if the drop down is A, B, or C and they choose "C", can 3 different Yes/No or Text answer questions populate?

Thank you for this! Exactly what I’ve been looking for. Can this excel form be made into FORMS and be accessed on my iPhone? I can’t bring computers into my meetings, but would like to collect data on my iPhone while I’m in a meeting.

QUESTION 1: If you send out this form to several staff to fill out, once you get them back, how do you collect all their individual answers (from several employees) into a single Excel sheet database? QUESTION 2: Is there a way to automatically feed a spreadsheet with the data from each employee form done? Thanks for sharing your knowledge!

This video was so clear and well done, and I'm excited about the possibilities. I was wondering, however, how a user would submit the form and whether it's possible to compile the submissions into a spreadsheet. I see other users have asked similar questions below, but I don't see an answer.

I can't see why you couldn't compile the results into a tab on the same spreadsheet, like having a summary sheet as the top of the tab list. But I'd like to hear from more of an expert as well. I'm not one.

People below are asking about populating the answers. I did a project 20 years ago populating the answers behind the scene in the same workbook, with one click you see all the answers and calculations. My problem is I lost my backup of my project, and don't remember how I set up. I am trying to remember. I do remember that I even wrote down the password, the name of the Manager to whom I made the project. The original project was done manually by somebody 17,000 questionnaire forms with 50 questions in word format. Each question has 5 answers. I had to assist the dept to retrieve the data/results of the questionnaire. So I decided to set up another method to replace the original manual labor project. I transferred everything to excel. After I accomplished the set up, it was a breeze, you open the answered form, and done. Saved a lot money, fatigue, and time. I just don't know where I put the backup disk, I saved in a floppy disk, even if I find it, where I am going to insert the flipflop disk, unless I go to those antique computer store and ask them to retrieve it. I have a habit not to backup my project inventions, a great procrastinator.
