Shoulder impingement is a prevalent term used to describe shoulder pain, but it falls short as a diagnosis. In this video, I'll debunk the notion of shoulder impingement and explain why it's an insufficient explanation for the root cause of your injury.

External impingement describes the pinching of the bursa (a small fluid filled “air bag”) or rotator cuff tendons against the ceiling created by the acromion bone. The subacromial space is the space between the acromion and the rotator cuff/soft tissue structures. Internal impingement occurs when the tendons of the rotator cuff are pinched between the “ball” of the humerus and the edge (usually the back side) of the socket “tee.” Because these classifications are based on where the impingement occurs, they often bring out pain in different parts of the shoulder.
External impingement for example often creates pain on the front side of your shoulder as you raise your arm overhead. Internal impingement creates pain on the backside of the shoulder as the arm is elevated and externally rotated.
To understand WHY your shoulder impingement is occurring we'll need to uncover:
1) Mobility imbalances
2) Stability imbalances
3) Coordination imbalances

Dr. Horschig is an Innovator, I'm 55 and I wish I'd known all these drills 20 years ago to avoid these shoulder issues today. Following the advice given on these informative videos, I can confirm that my problems had always been in the stability side and have seen a hell of a lot of improvement with the techniques given. Dr Aaron you really bettered my quality of life. I watched this video like a year ago bc I had that front impingement. Found out i was unstable and weak on the external rotation. Worked on that. I forgot about it, but in the past two-three months started feeling an internal impingement. All on the right shoulder. Now i saw this video again and found out my right lat was way tighter than the left. Did the stretch and ball excercise, the test and retest, and bravo! I can do pushups without pain! Crazy.. i bet now with the lata unlocked im gonna find something somewhere else. Little by little. Thanks dr Aaron!

Can't be overstated how light you might need to go on these. I've had to do these movements with zero added weight -- just the weight of my arm. Progress is slow and the little cues (like retracting first) make such a difference.

Great coaching as always! I have had total shoulder replacements that are performing great (hockey and football injuries)! I am concerned about wear of the implants over time. Maybe a video that speaks to those of us with shoulder or hip implants and the importance of stability and strength? Just thinking out loud for content! Great day!

I have had an impingement for 2 years also, never done weights but this is really good at explaining the differences. I have all of them from computer use. Been seeing 3 physios over the years and this video is by far the best.

I think this is what I have! after watching this... I am a heavy bencher and for the past couple of years have had constant pain in the left shoulder and into the front of the upper arm, it's affected my bench as well as pretty much cancelled out my shoulder exercises to the point I dont/cant train them anymore, I can feel and hear a popping sensation in the shoulder, I've still trained in the gym, not as heavy benching and with the pain and weakness it doesn't allow for heavy benching now, a couple of weeks ago it has now started in the right shoulder, I will try those exercises towards the last 1/3 of the video... Thank You! I sincerely hope this helps as I cannot sleep on the left side at all and now having problems on the right...

This was very good for me cause I’m getting into body building and I watch your videos for bench press now I’m having issues with lat close grip pull downs soon as I get a good weight my left shoulder would make a popping sound so this will be diffidently be in my next warm ups thank you brother

The lat stretch helped reduce my lh shoulder impingement. I was able to do light shoulder row into external rotation without pain. Range of motion also improved. Thank you!

another great video! very appreciated. you also talked alot about not letting the upper trap take over to shrug during this video. a good friend of mine has very tight upper traps and cant feel his midback stabelizing during any type of pressing/pulling. his traps hurt even so bad that he isnt able to highbar squat. the only thing that seems to help is squeezing a tiny plate between his romboids so he can actually feel them work during dips, pushups, pullups etc. do you have any tips on how to fix this?

Thanks for all the great content, Dr H. One question on this type of routine (the lateral raises and band exercises demonstrated) - is it something that should be done 2-3 times a week? More or less frequently?

Daily until you have progressed to the point we’re your joints and muscles can’t recover in time. Than go to 3 times a week and than maybe down to 2.

Hi Aaron, I just want to say thank you. I’ve had shoulder impingement for years and had no luck even after going to a couple therapists. I tried that lacrosse ball massage on my lats and found instant relief. I never knew tight lats could cause shoulder pains. Thanks again
