I would be interested to hear about the types of questions any SIMGs who have undertaken this process have received at the interview. The feedback that I have collected is that it can be similar to a PESCI (Pre-Employment Screening Clinical Interview) i.e. there are some questions around your CV and then they ask you about common scenarios. For e.g. in the radiology interviews, they apparently have scans for you to look at.

Hii Antony, which is best time to apply for specialist pathway.. I am about to complete my speciality training (3yrs)in hepatology from India in 2 months..applying just after completion is fine, or to get experience as consultant for few years n than apply..

Thanks for the invaluable information. I just had a small enquiry. When your specialist qualifications have been deemed partially comparable and have managed to receive supervised working post, will there be any form of monetary compensation for the supervised work that you do for the specified 1-2 years of compulsory supervison?

Unlikely to get SC. The reason I say this is that RANZCR assess about 95% of all applications as Partially Comparable.

In your case the primary qualification is your B Med e.g. your basic medical degree. The postgrad qualifications are the other things you have mentioned.

Great insight to the pathway sir. If one has a Drnb (Directorate of National Board) 6 year Neurosurgery training (surgical neurology) program from India will one be able to make it to a substantially comparable category of specialist registration pathway?

I wouldn’t want to speculate. It comes down to a range of factors. Suggest you book a call with me. Getting a post on neurosurgery here is quite hard.

It’s maybe slightly better if you are deemed substantially comparable. But it really depends on what the demand is for your particular specialty.

hi Anthony! really helpful videos, thank you so much. some questions i have, if you are kind to answer them. my husband has finished his MBBS followed by MS general surgery (3years post graduate course) followed by M.Ch urology (3 yrs degree course in urology in India) completed in 2017. Since then till present he is working as an consultant in urology (5 years experience ) .I just want to know if he applies for specialist assessment pathway, what are the chances for being partially comparable and how easy is to find a job in urology for supervised training . I have done diploma pediatrics (DCH) and 10 years experience in pediatrics, which pathway i would be eligible to apply?

You would both have reasonable prospects with the assessment. Finding an initial job for your husband will be the tricky bit.

Hi Emily your PR will be an advantage to obtaining a position. You may want to consider applying for an Area of Need role. For example you will see there are many listed here for Radiologists. www.health.nsw.gov.au/AoN/Pages/specialists.aspx In relation to specialist assessment there is some general information in the video series already. I plan to do some more so keep watching the channel.

@@CareerDoctor Thank you for your quick answer. I have already tried to apply some positions, but it seems that IELTS score must meet the requirement for register. So I plan to get 7 in four parts of IELTS, then apply to specialist assessment. Do you know some radiologists from China who get positive results in the assessment?

It is generally best to try to score better than the minimum on the IELTS. A score of 8 overall for example is a much stronger indication to employers that you have the level of English communication to perform the job well. Each year there are only generally a handful of Chinese specialists seeking recognition in Australia. For example in 2017 2 sought initial assessment and both were found partially comparable. Sorry, I don't know which specialties they applied for.

1.that would be enough training time to be worth applying. 2. Area of Need is done at the same time as specialist pathway assessment. 3. You need to do an English test.

Would it be true to say that partially comparables get specialist registration just 1 year late than substantially comparable ones ? .... as supervised work is 2 years(inclusive of fellowship & assessment exams) for former and 1 year for latter.

Yes this is true although it also simplifies matters a bit. SC doctors have fewer requirements to demonstrate than PC. Whilst the majority of candidates progress to specialist registration a small number don’t get recommended and it’s likely that relatively more PC doctors don’t get recommended in the end.

I am a fresh MBBS graduate from India..should I apply via standard pathway or complete my specialisation in india and apply via specialist pathway?..which is better?..as far as I know..in standard pathway..I have to clear AMC,redo my internship(which I already did in India),do my residency..then apply for specialisation..all of which takes minimum 5 years But in specialist pathway..I should finish my Indian Pg exam..study MD..apply for Australian specialist college..undergo training..all of which takes minimum 4 years.. I need not attend AMC,I need not redo my internship..and I feel specialist recognition better than AMC..what's your opinion sir?

Hi Supriya. The best answer is it depends. If you are thinking you want to establish yourself as a specialist in India first then I would do that. If that is not your goal then maybe #standardpathway is better. Both approaches have their pros and cons. Your analysis of both pathways is generally correct. With #standardpathway you do not need to do internship again. There is a provisional registration year which is similar to an internship. But its not an internship.

Hello sir , I have done my MS general surgery in 2013 following which I did my M.ch urology from a reputed institute in India. I completed my urology training in 2017 following which I have been working as a specialist in urology in a government medical school. My wife is an MD in dermatology she completed her training in 2014 and has been working as specialist in dermatology in government medical school since then. So I have 6 years of experience in urology since my residency and my wife have has 5 years of experience. So do I need to give the AMC exam and the IELTS exam and which college should we apply to ??...please guide us....thanks in advance

Hi Sir. I have done my MBBS and MD in Dermatology from India with 1 year experience after MD. Should I apply for the specialist pathway or the standard pathway? Also would MRCP(SCE) help in any way?

I don’t know why you would want to apply for standard pathway if you are already qualified on dermatology on your own country. The MRCP won’t help very much. It would be best if you worked as a consultant for a couple more years.

Hi, I came over to Australia via competent authority pathway and now have general registration since 2018, while here I applied for and got SCP in psychiatry, now I am in a bit of a problem as I cant get provisional registration for supervised practice as a specialist, because I have Gen reg, I cant get specialist reg as I need 12 months supervised practice before fellowshipI don’t know as yet what my prospective employer will think of this as credentialling for me might be an issue, also I cant work as a “ specialist” while on gen reg , I am not sure if there is an appropriate way around this issue. Any words of advice will be helpful

Hi ,I am from Iraq .. I had finished 5 years of training in orthopedics and trauma and became a fellow of iraqi council of medical specialities 4 years ago with practice during these years and now is in a local spine surgery fellowship .. I haven’t had a lot of applications from SIMG plastic surgeons. I imagine it will be more difficult than most other specialties. As plastics is very competitive.

Hi David thanks for the question. It depends on what your actual goal is. If you want to specialise in an area and you are already training in that area. Then I’d recommend finishing your training. Then at the least you are recognised as a specialist in your own country. If on the other hand your goal is to live in Australia as soon as possible. I would consider going the other route. Ultimately there is actually no easy route to working as a doctor in Australia these days.

Hello, it will mainly come down to the length of your training in India. In general, I would recommend going for the DNB in India rather than doing the FRCS, because the FRCS on its own does not mean much here in Australia and also it relates to surgical training rather than Ophthalmology (i.e. its the wrong College).

My training period for proper neurosurgery is 3 years. but to do neurosurgery it is necessary to do general surgery in most of institutes of india. Hello Anthony, Just a small question, I'm planning to take up DNB OBGYN in india which is a 3 1/2 year course , To apply for specialist pathway later I will need atleast 5 years experience to be substantially comparable right? So in this case what would you suggest to do in the meantime after DNB to add up as experience 1.MRCOG and gain experience of 2-3 years in UK 2.fellowship or superspecialisation in india in a subspecialty in OBGYN which is again of 2-3 years ? As both will give me extra 2-3 years experience and in total 5-6 years experience in the field of OBGYN

Thanks Amrutha on the one hand lack of access to dermatologists is a common complaint in Australia. On the other it’s a very difficult specialty to get into both as a trainee and IMG.

I would suggest completing ur MD going for the DNB and working at least 2 years as a Consultant. That’s a reasonable level by which most Indian specialists succeed through the specialist pathway here.

Is it true that with regards to surgical specialties like Obstetrics and Gynecology, it is much more difficult to be considered as comparable even with good knowledge of English and good work experience? Is there less need for these types of specialties?

Hi Mansoureh. First off the College of Obstetricians and Gynaecologists (RANZCOG) would not take kindly to being referred to as a "surgical specialty". For the public data that we have from 2015-2017 the College with the lowest rate of deeming applicants comparable was the College of Dermatologists with 38.71% and the College with the highest rate was the Royal Australasian College of Physicians with 85.48%. RANZCOG was 4th hardest on the list but still with an overall rate of 67.21% deemed comparable on assessment.

DNB Is seen as doing something additional and higher standard on top of the MS. Because Indian masters are only 3 years you need to demonstrate more training and assessment. The MRCS is less likely to be helpful.
