I've been doing all wrong all along. I thought the pad went under the actual painful part. Thanks for explaining so clearly and most of all for demonstrating.

Tape the pad in first until you are sure of proper placement, draw around it and then use the adhesive.

I've been nervous about proceeding in case I aggravated my sore foot due to improper pad placement. Seeing just the insole in your hand with the pad placed farther back than I thought made it super clear what to do. Many thanks.

I've been suffering from this for about a year now. I walk an average 25,000 to 35,000 steps a day at work. I have have been buying pads that sits over the ball of my feet. I have never done it your way. It makes so much sense. Thank you so much, I'm definitely going to try that. Cheers.

Update? I bought these slip on gel things that go over the balls of the foot. It’s only been a day and I think it’s working, there is less pressure from my feet to the ground. However I feel as if this video is the better way? Idk. This is my first time having this issue it sucks

3:50 Really helpful to see where to place the pad. I’ll probably use a marker pen to draw where the ball of my foot sits on the liner.

I've been suffering from tremendous pain in my feet from cycling. I've spent thousands over the years on orthotics, various high end shoes, but nothing has worked; I figured I was doomed. And then I tried this - instant relief. It's competely in believable. I cannot believe it, just so thankful. Thank you.

A few weeks ago I started feeling like I was getting sharp electric shocks in my feet while on the bike trainer. I used a pair of Dr Scholls met pads and also a pair of gel arch supports. I placed them under the insoles to smoothen the transitions. Night and day comfort difference plus a measurable power increase at a given effort level! This video was very helpful in confirming the location of the met pads. Thanks!!!

Thanks Dr. I am a bespoke shoemaker and I often get clients with problem feet. Usually their podiatrist sends information to fit an insole but it's good to know more about what I am actually doing.

Thanks for your video! Whether DIY or store bought, I'm curious why the pad wouldn't go across the full width of the insole? Seems like the pear shape almost doesn't support the proximal & distal metatarsi.

I had this made at my podiatrist yesterday. He taped it to my foot. I had the heads of my fifth metatarsals removed as a young woman. One podiatrist did one foot and another podiatrist did the other foot at the same time. One foot I've never had problems with and the other I've had nothing but problems. On my problem foot I had neuroma surgery many years later. That foot has been sensitive since. Most recently I've developed pain that gets fairly bad where my toes join onto the foot at the second, third and fourth metatarsal. I already wear orthotics.

Dear doctor! 🤓 What a great detailed explanation, which took the mystery out of this problem. I understand WHY the foot pads did not alleviate my pain (between 2nd & 3rd bones) before. Just bought some expensive Brooks but I'm sooo hoping it will help my metatarsal pain. Since Covid hit Ive been furloughed n walking 5 miles a day. I am going to try your ideas on another pair of shoes! I need some exercise ideas for my feet too!

The clearest explanation that I've seen so far! Could the pad go under the insole though, or is it better on top? And could it be modified to support the arch of the foot too?

What do I have if I have itching along my metatarsals, where my toes meet the foot? No signs of redness, no gout, no rash, no athlete's foot. I'm guessing because I'm overweight but recently lost weight and am running about 3-4 miles now, that it's possible inflammation of the nerves around my metatarsals? Will these pads help?

I've been using something like this for a while. I wonder if it can put any stress on the foot that might damage it in other ways. Also, is it important for it to be in that exact teardrop shape rather than, say, an oval shape placed width-wise behind the ball?

Hope you have found the best position. You want it just back from the sore area, so it relieves pressure on the sore spot. Just try a few different placements until you get it right for you. 😊😊 You can vary the size too, to suit your foot.

Thanks for the video. Question: The top of my foot starts to have a burning (2nd toe/3rd toe where they bend) after I've been running for about 5 miles. Once I get to 5 miles I start to feel the pain/burning. I've gotten wider shoes and have tried all sorts of things including toe socks. What do you suggest? I'm willing to try just about anything. I'm training for a half marathon. The pain goes away as soon as I stop running. Thanks :D

I would recommend you see a good podiatrist. Bring your running shoes to the appointment. Have you tried lacing your shoes to take pressure off the dorsal cutaneous nerves? If I have time I'll make a quick video on how to do that later this week - I don't think I've done that video yet. It could also be extensor tendonitis - if your feet are very flexible then you may be gripping the ground excessively when you run. Just a couple ideas - you can run these by your own foot doctor.

Hi Dr. Dave I was wondering if you can answer my question. I have custom orthotics and decided to replace the top cover myself I ordered Tread Labs foam covers off Amazon. the thing is its a steep 3 or 4 mm drop off from the poly shell to the front ball of the foot area. Can I use like an adhesive backed felt under the top cover and taper it with a scissor cut? or is there a better material or method to smooth that transition?

This sort of tape is so incredibly expensive in the UK for such a tiny, tiny amount. We're being gouged.

Try foam carpet underlay (a shop may give you some free offcuts) which you can attach under the insole with double sided tape. The carpet shop should also sell that. If not it is readily available in hardware stores or Ebay etc.

I got met pads and on day one of using them (at a desk job) they feel kinda bulky and bothersome, no pain. Is that normal or could they be too big? Do they do a better job if they’re a bit bigger?

Placement is very important. Also, the nice thing about using the felt to make your own pad is that you can make the pad bigger by adding more felt - or smaller by peeling the felt.

Would you, could you, Be so kind, Dr Dave and address the significance of the TRANSVERSE ARCH? No one I can find, will address the fact that there are 3 arches of the human foot. I feel that my Transverse Arch has completely fallen, like my main arch in both my feet! You have no idea how much I would appreciate your personal addressing of this question!

I just bought a pair of Spenco pads, but they were too bulky and tight in my road cycling shoes. This is a great idea of helping me formulate something on my own. I suffer from 'hot feet' after about 2,500m of constant hillclimbing. If I spread the effort out over 100+ km it doesn't bother me, but when I compress the effort by doing hill repeats, my foot doesn't get a break and I get 'hot feet'. Any suggestions, besides making sure my cycling shoes aren't too tight?

Ric: It happens to me as well on hot rides. I use a Louis Garneau sock that is pretty thin and I loosen up the straps on my Sidi shoes and it seems to do the trick. I've also moved my cleats further back so that I don't put pressure directly on the met heads.

I wish just one podiatrist spent more than 5 minutes with me and told me that a metatarsal pad and OTC orthotics are going to throw out your body's natural gait and cause another injury. It's almost a sure thing if you wear them long enough if one foot is elevated higher than the other with every step due to the pad. It's going to mess up your knees, feet, and especially your other foot. You'll get all kind of soreness in your toes and foot muscles. It is not the answer long term.

Many patients tell me I should invent such a shoe. Unfortunately few individuals would wear them. Most people prefer shoes that prioritize fashion over function.
